Increased enhancement of the liver adjacent to the gallbladder seen with contrast ultrasound: comparison between acute cholecystitis and non-cholecystitis.

BACKGROUND: This study was performed to evaluate the ability of contrast-enhanced ultrasonography (CEUS) with time-intensity curve analysis to demonstrate an increased enhancement of the liver parenchyma adjacent to the inflamed gallbladder, as seen on contrast-enhanced computed tomography. METHODS: The Ethics Committee of our institution approved the study protocol (Kawasaki Medical School, registration number 1277). From April to November 2013, 11 consecutive patients with acute cholecystitis and 16 patients without cholecystitis consented to CEUS (Sonazoid™) and were enrolled in this study…

Pediatric acute cholecystitis: Risk factors and outcomes.

BACKGROUND: Pediatric acute cholecystitis is a growing concern due to rising childhood obesity rates and associated metabolic abnormalities. AIM: To identify demographic and clinical risk factors, evaluate surgical outcomes, and analyze the impact of complications on hospitalization in pediatric patients undergoing cholecystectomy. METHODS: A retrospective study was conducted on 12750 pediatric patients who underwent cholecystectomies in 2022. Data were collected from a de-identified National Surgical Quality Improvement Program database, including age, gender, race, body mass index (BMI) st…

Percutaneous versus surgical treatment of acute cholecystitis.

A recent meta-analysis by Fanchulli et al. concluded that cholecystectomy (CC) is associated with lower mortality and readmission than percutaneous cholecystostomy (PC) for acute cholecystitis, but this interpretation is limited by confounding by indication to management. The pooled evidence in this article consists predominantly of observational cohorts where crude or minimally adjusted comparisons are vulnerable to case-mix imbalance; even the CHOCOLATE randomised trial, which supports laparoscopic cholecystectomy in selected high-risk but eligible patients (ASA III-IV, age ≥ 70 or comorbidi…

Cholecystitis-related cystic artery pseudoaneurysm: Case report.

The pseudoaneurysms of the cystic artery (CAP) are very uncommon. They usually develop as a result of an acute cholecystitis or after a cholecystectomy. Among the complications, we can find hemorrhaging, biliary blockage, and haemobilia. Limited understanding of the illness makes managing specific cases difficult. We describe a case of a cystic artery pseudoaneurysm complicating an acute cholecystitis that was successfully treated by transcatheter arterial embolization.

Isolated tubercular cholecystitis: A rare diagnostic challenge.

Abdominal tuberculosis, particularly affecting the hepatobiliary system, presents a diagnostic challenge due to its rarity and its tendency to mimic neoplastic conditions. Tuberculosis of the gallbladder, especially in the absence of pulmonary infection, is exceedingly rare. We present a case of isolated tubercular cholecystitis in a 41-year-old female initially suspected to have neoplastic etiology. Radiological imaging revealed thickened gallbladder wall with heterogenous enhancement extending into the subcutaneous plane. Histopathological examination post-cholecystectomy confirmed tubercula…

Portal vein thrombosis associated with acute cholecystitis.

Portal venous thrombosis (PVT) is an uncommon clinical problem and is rare following cholecystectomy. This article describes a patient who developed PVT after an initially uneventful laparoscopic cholecystectomy. The patient was successfully treated with IV antibiotics and anticoagulation.

Hemorrhagic cholecystitis afflicted with glanzmann thrombasthenia patient.

Hemorrhagic cholecystitis is a rare and potentially life-threatening condition that requires immediate medical attention. If the gallbladder perforates, it can lead to even more severe complications. We present the case of a 27-year-old man with Glanzmann's thrombasthenia, a rare platelets disorder, who visited the emergency department twice before being diagnosed with this condition. Initially, he complained of ear pain, and later, chest pain. Despite a normal ECG and cardiac markers, he was discharged with non-steroidal anti-inflammatory drugs (NSAIDs) for musculoskeletal pain. However, when…

Acute cholecystitis with double gall bladder diaphragm.

Gall bladder diaphragm is a rare congenital malformation of the gall bladder, entailing its segmentation in several chambers. Functioning as a non-return valve, it interferes with drainage, leading to gallstone formation and cholecystitis. We are reporting a rare case of acute cholecystitis with a double vesicular diaphragm.

[Risk Factors Associated with Acute Gangrenous Cholecystitis].

INTRODUCTION: Acute gangrenous cholecystitis is a serious complication of acute cholecystitis, which is difficult to diagnose preoperatively. AIM: . To assess risk factors associated with acute gangrenous cholecystitis. MATERIAL AND METHODS: Case-control, retrospective, observational and analytical study of patients with acute cholecystitis, diagnosed between January 2017 and January 2022. Patients were divided into two groups according to anatomopathologic diagnosis: acute gangrenous cholecystitis (cases) and acute non-gangrenous cholecystitis (controls). RESULTS: A total of 159 patients w…

Percutaneous Cholecystostomy for Acute Cholecystitis During Spaceflight.

INTRODUCTION: Acute calculous cholecystitis is a common surgical emergency and cholecystectomy is the gold-standard treatment. However, alternative drainage modalities such as percutaneous cholecystostomy tube (PCT) placement have been proposed for poor surgical candidates or in remote environments, such as space. We reviewed the literature to assess the theoretical utility of PCT to treat acute cholecystitis during long-duration spaceflight or on the Moon or Mars. METHODS: A systematic review of 16 peer-reviewed articles published since 2018 was completed to describe the terrestrial efficacy…

Acute cholecystitis treated by direct visualization endoscopy.

The common treatment of acute cholecystitis due to cholestasis is percutaneous transhepatic gallbladder drainage (PTGBD) or EUS-guided gallbladder drainage (EUS-GBD) or cholecystectomy. The new generation of direct vision endoscopy represented by Spy glass has successfully entered the gallbladder duct and gallbladder. On this basis, we apply similar direct visualization system for treatment.

Imaging acute cholecystitis, one test is enough.

BACKGROUND: Patients with right upper quadrant pain are often imaged using multiple modalities with no established gold standard. A single imaging study should provide adequate information for diagnosis. METHODS: A multicenter study of patients with acute cholecystitis was queried for patients who underwent multiple imaging studies on admission. Parameters were compared across studies including wall thickness (WT), common bile duct diameter (CBDD), pericholecystic fluid and signs of inflammation. Cutoff for abnormal values were 3 mm for WT and 6 mm for CBDD. Parameters were compared using chi…

Perforated Emphysematous Cholecystitis: A Race Against Time.

Emphysematous cholecystitis is a rare infection of the gallbladder that stems from acute cholecystitis. It can rapidly progress and perforate the gallbladder, which would require urgent surgical intervention. A perforated gallbladder can be diagnosed using an abdominal computed tomography by confirming the presence of air in the gallbladder lumen with adjacent extraluminal air. The causes of ruptured emphysematous cholecystitis include, but are not limited to, diabetes, atherosclerotic changes in blood vessels, and infection with Clostridium perfringens, Escherichia coli, and Klebsiella spp., …
