The effects of cochlear implantation in Japanese single-sided deafness patients: five case reports.

CONCLUSION: Cochlear implantation (CI) for Japanese single-sided deafness patients resulted in improved speech perception, increased sound localization accuracy, and reduced tinnitus handicap. OBJECTIVES: This study reports results for five adult SSD cases with CI, focusing on the benefits they obtained in terms of speech recognition, sound localization, and tinnitus handicap. METHODS: Five Japanese patients meeting the eligibility criteria were included in this study. All patients were implanted with a fully inserted MED-EL Concerto FLEX28® implant (MED-EL, Innsbruck, Austria). Speech perce…

Cochlear implantation via round window or cochleostomy: Effect on hearing in an animal model.

OBJECTIVES/HYPOTHESIS: Cochlear implantation in patients with residual hearing has increased interest in hearing preservation. Two major surgical approaches to implantation have been devised: via the round window membrane and through cochleostomy. However, the advantages of either approach on hearing preservation have not been established. Due to the great inter- and intravariability among implantees, the current study used a normal-hearing animal model to compare the effect of the two methods on hearing. STUDY DESIGN: Animal study. METHODS: Thirteen fat sand rats were studied, in which 13 e…

The Relationship Between Cochleovestibular Orientation, Age, and Sensorineural Hearing Loss: Implications for Cochlear Implantation.

OBJECTIVES: To determine if spatial orientation of the cochlea within the temporal bone is related to age or sensorineural hearing loss (SNHL) and describe the implications for cochlear implantation. METHODS: Five angles of cochlear orientation were determined from computed tomography (CT) imaging of the temporal bones in adults with (n = 55) and without (n = 27) sensorineural hearing loss (SNHL) and children with (n = 45) and without (n = 12) SNHL: facial recess versus basal turn, posterior semicircular canal versus basal turn, round window versus basal turn (axial view), round window versus…

GDP Matters: Cost Effectiveness of Cochlear Implantation and Deaf Education in Sub-Saharan Africa.

HYPOTHESIS: Cochlear implantation and deaf education are cost effective in Sub-Saharan Africa. BACKGROUND: Cost effectiveness of pediatric cochlear implantation has been well established in developed countries but is unknown in low resource settings, where access to the technology has traditionally been limited. With incidence of severe-to-profound congenital sensorineural hearing loss 5 to 6 times higher in low/middle-income countries than the United States and Europe, developing cost-effective management strategies in these settings is critical. METHODS: Costs were obtained from experts in…

Insertion trauma and recovery of function after cochlear implantation: Evidence from objective functional measures.

Partial loss and subsequent recovery of cochlear implant function in the first few weeks following cochlear implant surgery has been observed in previous studies using psychophysical detection thresholds. In the current study, we explored this putative manifestation of insertion trauma using objective functional measures: electrically-evoked compound action potential (ECAP) amplitude-growth functions (ECAP amplitude as a function of stimulus level). In guinea pigs implanted in a hearing ear with good post-implant hearing and good spiral ganglion neuron (SGN) survival, consistent patterns of EC

Results of a prospective surgical audit of bilateral paediatric cochlear implantation in the UK.

OBJECTIVES: Since being approved in 2009, bilateral simultaneous cochlear implantation (CI) has been the standard treatment for children in the UK who meet the criteria for CI. The aim was to report surgical outcomes of bilateral CI in the UK. METHODS: Between January 2010 and December 2011, 14 UK CI centres collected data prospectively: demographics, aetiology, use of imaging, device type, surgery duration, use of intra-operative electrophysiology, length of stay, and post-operative complications. RESULTS: 1397 CI procedures in 961 CI recipients were included; 436 bilateral simultaneous, 39…

The pathologic basis of facial nerve stimulation in otosclerosis and multi-channel cochlear implantation.

HYPOTHESIS: Unintentional electrical stimulation of the facial nerve by cochlear implants occurs when advanced otosclerosis invades the endosteum of both the upper basal turn of the cochlea (UBTC) and the facial nerve canal (FNC) and all the bone between these 2 structures. BACKGROUND: A complication of cochlear implantation is facial nerve stimulation (FNS) known to be more common in otosclerosis. Otosclerotic involvement of the enchondral bone of the otic capsule results in areas of bone resorption, new bone formation, vascular proliferation, and a connective tissue stroma. This may reduce …

Pneumococcal meningitis post cochlear implantation: development of an animal model in the guinea pig.

In 2002 an increased number of cochlear implant related meningitis cases was reported by the U. S. Food and Drug Administration (FDA). The most commonly identified causative agent was Streptococcus pneumoniae. Although most cases of meningitis were related to a special electrode design, the risk for post-operative pneumococcal meningitis might nonetheless be enhanced by opening of the cochlea during implantation. In the present study, a threshold model for middle ear inoculation of S. pneumoniae was established in the guinea pig after cochlear implantation to assess the post-operative risk of …

Temporal bone histopathology related to cochlear implantation in congenital malformation of the bony cochlea.

HYPOTHESIS: Histopathologic findings in temporal bones with congenital malformations of the bony cochlea may provide insight into cochlear implantation planning, surgical approach, and complications. BACKGROUND: Patients with congenitally malformed cochleae account for an increasing percentage of candidates for cochlear implantation. Few studies on the relationship between histopathologic findings of temporal bones with malformation of the bony cochlea and cochlear implantation have been reported. METHODS: We studied 21 temporal bones from 12 cases with congenital malformations of the bony c…

The prognostic value of promontory electric auditory brain stem response in pediatric cochlear implantation.

OBJECTIVE: To test the hypothesis that children with clear promontory electrically evoked auditory brain stem responses (prom-EABRs) would outperform, after cochlear implantation, children who had no prom-EABR preoperatively. DESIGN: A prospective study was undertaken on 47 implanted children assigning them to two groups (group A: 35 children with a clear wave e-V in the preoperative prom-EABR and group B: 12 children with no prom-EABR). Speech perception and speech intelligibility were assessed annually up to 3 yr after implantation with the IOWA sentence test (level A and level B), Connecte…

Surgical complications of cochlear implantation: a report of three cases and their clinical features.

The most common complications of cochlear implantation are flap necrosis, compressed or improperly placed electrodes, and facial nerve injury, paresis, or stimulation. We report on three recent complications experienced at our institution: 1) a flap ischemia and partial-thickness necrosis without infection or device exposure, 2) facial nerve stimulation, which was managed with a new programming technique, and 3) a postimplant cholesteatoma in a patient with chronic eustachian tube dysfunction that required reoperation and explanation. We discuss the pathophysiology, likely etiology, and recomm…

An evaluation of per-scalar cochlear electrode implantation techniques. An histopathological study in cats.

This study was carried out to evaluate the effects of infection and trauma on the organ of Corti and spiral ganglion cells following per-scalar cochlear electrode implantations in cats. The results showed that the incidence of infection with a per-scalar electrode inserted through an opening made directly into the apical and middle turns is low, provided the procedure is carried out aseptically with an antibiotic cover. Trauma could also be kept to a minimum by inserting the electrode into the scala tympani, taking care to insert the electrode without applying force, and ensuring that the firs
