[Sensitization to cockroach allergens evaluated by skin tests in children with atopic dermatitis].

INTRODUCTION: Cockroach and house dust mites (Dermatophagoides pteronyssinus, Dp and farinae, Df) are the most often implicated aeroallergens in severe asthma, hay fever and conjunctivitis. Cockroach allergy is still unknown in atopic dermatitis. PATIENTS AND METHOD: 146 children with atopic dermatitis-aged 6 months to 15 years- have been patch tested with the European standard series and some aeroallergens. We have studied the sensitisation to cockroach allergens and compared to Dp and Df. RESULTS: 113 children reacted positively at least to one of the 3 aeroallergens (77 p. 100), 61 childr

Oxatomide in the treatment of atopic dermatitis in breast-fed and very young infants.

BACKGROUND: This study was designed to assess the efficacy and safety of oxatomide oral suspension, in breast-fed and young babies with atopic dermatitis. METHODS: Thirty-one children (11 females, 20 males) aged between 4 and 35 months (mean+/- SD 15.4+/-7.6 months) were treated with oxatomide (2.5 mg/mL) at a dose of 1 mg/kg/day) split into two doses, every 12 hrs; treatment lasted 30 days. Efficacy was evaluated on the basis of the following signs and symptoms: itching (severity, extension and duration), blisters (number, extension), erythema (severity, extension), papules (number, extensio

Severe atopic dermatitis is associated with sensitization to staphylococcal enterotoxin B (SEB).

BACKGROUND: Staphylococcus aureus has been identified as a possible trigger factor in atopic dermatitis (AD). Some 30-60% of S. aureus strains isolated from patients with AD are able to produce exotoxins with superantigenic properties, mostly staphylococcal enterotoxins A, B, C, and D (SEA-D) and toxic shock syndrome toxin-1 (TSST-1). Recently, it was demonstrated that the presence of IgE antibodies to SEA and SEB is correlated with the severity of skin lesions in children with AD. To determine the relevance of staphylococcal enterotoxins in adult patients with AD, we investigated the relation

[Safety and efficacy of treatment for severe atopic dermatitis with cyclosporin A and transfer factor].

BACKGROUND: The atopic dermatitis is a chronic skin disease that appears in patients with a personal or family history of allergic asthma and rhinitis. It is associated to the specific activation of a gene group. In most instances, the response to the conventional treatment is adequate. The are cases, though, know as refractory, where that is not the case. The study of two therapeutic alternatives, Transfer Factor (TF) and Cyclosporin A (CyA), was elaborated for this type of patients. MATERIAL AND METHODS: Patients with severe refractory AD were studied, being admitted to the Allergic Service

Staphylococcus aureus in the anterior nares and subungual spaces of the hands in atopic dermatitis.

We examined the prevalence of Staphylococcus aureus in the anterior nares and the subungual spaces of the hands of patients with atopic dermatitis to determine whether the presence of S. aureus at these sites may contribute to the aggravation of the dermatitic skin lesions. The prevalence of S. aureus in the anterior nares of patients with atopic dermatitis was over five times higher than that in the anterior nares of patients with other skin diseases or in healthy adult controls, and the prevalence of S. aureus in the subungual spaces was 10 times higher in patients with atopic dermatitis tha

Elevated serum levels of soluble CD30 in patients with atopic dermatitis (AD).

The immunopathology of AD is still unclear, but evidence for an immune response polarized towards Th2 activity has been provided. The CD30 molecule belongs to the tumour necrosis factor (TNF) receptor family and is expressed on activated T cells with a sustained expression in Th2 cells. This molecule also exists in a soluble form (sCD30). Elevated serum levels of sCD30 have been found in patients with Hodgkin's disease, chronic hepatitis B infection and HIV infection. Studies were undertaken to compare the serum levels of sCD30 in patients with AD (n=49) and healthy non-atopic controls (n=94).

Retinal detachments with crescent-shaped retinal breaks in patients with atopic dermatitis.

BACKGROUND: There has been increasing number of reports of retinal detachments associated with atopic dermatitis. However, little is known about the etiology of these rhegmatogenous detachments. The authors encountered two patients with retinal detachments associated with a crescent-shaped retinal break combined with another break located at or anterior to the ora. METHODS: Preoperative and intraoperative findings in these cases were evaluated. RESULTS: In both cases, vitreoretinal adhesions were observed on the anterior and posterior edges of a crescent-shaped retinal break. Vitreous tracti

Treatment of atopic dermatitis in children: the importance of skin care and environmental control.

A 5-year-old female with severe atopic dermatitis with secondary bacterial infection is presented. The patient had been managed with topical steroids and anti-allergic drugs for several months prior to admission to the National Children's Hospital, Tokyo. This case is reported to emphasize that drugs alone are not sufficient in treating severe atopic dermatitis. Skin care and environmental control are equally important and should form a part in the management of the disease.

Ketoconazole in atopic dermatitis: therapeutic response is correlated with decrease in serum IgE.

The prevalence of specific IgE antibodies to the yeasts Pityrosporum orbiculare and Candida albicans was investigated in adult patients with atopic dermatitis (AD) or with seborrhoeic dermatitis and in healthy controls by means of the radioallergosorbent test (RAST). Of 63 AD patients, 28 (44%) had IgE antibodies to P. orbiculare and 21 (33%) to C. albicans. This is highly significant, since no antibodies were found in sera from other patients or controls. With the intention to treat, 20 patients with AD and a positive RAST to P. orbiculare were given ketoconazole 200 mg daily for 2 months and

[Pigmentation on the anterior chamber angle in retinal detachment associated with atopic dermatitis].

We reported pigmentation on the anterior chamber angle in twenty-two patients suffering from atopic dermatitis. Dense pigmentation on the anterior chamber angle was observed in all eight eyes with retinal detachment and was evident before clinical recognition of detached retina. Minimal or slight pigmentation was also evident in eyes without retinal detachment. Increased pigmentation on the anterior chamber angle is a characteristic and predictive sign of retinal detachment associated with atopic dermatitis. This pigment appeared to migrate from latent breaks because of inflammation of the per

Levels of soluble interleukin-2 receptors correlate with the severity of atopic dermatitis.

Levels of soluble interleukin-2 receptors (sIL-2R) were measured as a marker of lymphocyte activation in serum of 37 patients with atopic dermatitis (AD) and of 16 patients with psoriasis vulgaris (PV), all individuals being hospitalized in the Zurich High Mountain Clinic of Davos-Clavadel (altitude 1,600 m above sea level). Measurements were performed at admission and at discharge. The PV patients served as a control group. As a further control, 20 AD patients from the Department of Dermatology in Zurich (altitude 400 m) were also evaluated. The serum sIL-2R concentration in AD patients at ad

[IgE and peripheral blood T-lymphocyte levels in patients with atopic dermatitis].

The percentages of suppressor T lymphocytes were compared in 111 patients with atopic dermatitis and 100 healthy volunteers. In the former group there was a statistically significant reduction of suppressor T cells, which was not related to IgE level. Similar decrease in T cells has been found in patients with normal IgE as well as in patients having 10-fold increase in serum IgE. Our results were compared to previously published data of other investigators.

[Concomitant psoriasis vulgaris and atopic dermatitis. A study of 1,065 patients with psoriasis].

The simultaneous presence of psoriasis and atopic dermatitis was checked for in 1065 patients with psoriasis and was found in 18 of them (1.7%), which is in keeping with the statistical probability. In these 18 the efflorescence was also examined histologically. All 18 (100%) reported marked pruritus; white dermatographism was observed in 16 patients (89%); in 15 patients (83%) psoriatic phenomenas could be produced, and 11 patients (61%) also had allergic rhinitis or bronchial asthma. Immunologically, 16 of the 18 patients (89%) showed elevated IgE levels, the geometric mean level being 888 I

Pityrosporum orbiculare--a pathogenic factor in atopic dermatitis of the face, scalp and neck?

Pityrosporum orbiculare (pit.o.), the yeast form of Malassezia furfur, though usually considered to be a non-pathogenic saprophyte, in some individuals trigger various types of dermatitis. It is earlier shown that it is of importance in atopic dermatitis in the head-neck area in adults, and that elimination of the yeast by help of treatment with ketoconazole improve the dermatitis. In this large retrospective study of 741 patients prick-tested with an aqueous extract of pit.o., the occurrence of positive prick-test to pityrosporum was related to the patient's atopic manifestations. It was foun

The relevance of inhalant and food allergens to the etiology and management of patients with atopic dermatitis.

Patients with atopic dermatitis have IgE antibodies to common environmental antigens, both foods and inhalants. Such antibodies are probably relevant and exposure to the corresponding antigens can give rise to eczema. Nevertheless, the mechanisms involved and the role of other etiologies, e.g. contact reactions, remain to be elucidated. Patients with atopic dermatitis should have comprehensive evaluations to determine the role of environmental antigens.
