BACKGROUND: Keratoconus has long been considered to be an ectasia produced by stromal stretching. Although stretching should result in increased corneal surface area, previous observations of topography during progression of keratoconus have suggested that surface area may actually be conserved. A novel objective surface area measurement based on corneal topography was tested and applied to data from actual corneas under various conditions for comparative analysis.

A 2-year-old boy presented for evaluation of a systolic murmur. Echocardiogram demonstrated dilated left main and right coronary arteries. Selective injection of the left coronary artery confirmed dilation of the left main and anterior descending coronary arteries. The left anterior descending coronary artery subsequently filled the mid portion of the right coronary artery retrograde, which emptied into a blind fistula. The orifice of the right coronary artery was atretic, with no filling from the aortic root or the fistula.

We evaluated sonographically 162 children who met the criteria for biliary tract dilatation in the past 18 years. Of these, 131 patients were diagnosed as having anomalous dilatations of the biliary tree (including 112 with choledochal cysts and 19 with biliary duct dilatation and biliary atresia). Aortic insufficiency is a well-recognized complication of adult spondyloarthropathy but rare in juveniles, and no report is associated with aneurysmal dilatation at any age. We report two cases of Marfan's syndrome with coexistent obstructive sleep apnea (OSA) in which treatment with nasal continuous positive airway pressure was associated with attenuation of aortic root dilatation, a serious complication of the syndrome. We speculate that coexistent OSA promotes progressive aortic dilatation in some patients with Marfan's syndrome.

The criteria for the detection of fetal pyelectasis are still controversial. Prenatal and postnatal data from 2,170 consecutive pregnant women who underwent at least one antenatal ultrasound scan were prospectively studied. Fetal pyelectasis was defined on the basis of a mean renal pelvis dimension > 5 or 10 mm before or after 28 weeks of gestation, respectively. Pyelectasis was detected in 95 (4.4%) fetuses. Eighty-nine among them were explored after birth. In 13 (13.7%) cases, an obstructive urinary tract abnormality, a severe vesicoureteral reflux, or a megaureter were diagnosed postnatally

We present two infants with ureteropelvic junction obstruction with very large renal pelves, which extended to the level of the urinary bladder. In both cases, the distal aspect of the renal pelvis simulated a dilated distal ureter, leading to the incorrect sonographic diagnosis of ureterovesical junction obstruction. Awareness of this potential pitfall on sonography is important when imaging infants with prenatal diagnosis of hydronephrosis.

This study examined the relations between age, arterial distensibility, and systemic hemodynamics in patients with the Marfan syndrome. The study group included 170 patients referred to a specialist clinic, of whom 55 (age 26 +/- 12 years) were diagnosed as having Marfan syndrome. Three cases of large airway stenosis of different aetiologies are reported. In the first case, tracheal stenosis occurred after endotracheal intubation. The second case developed bronchial stenosis as a sequelae to pulmonary tuberculosis. The third case had localized stenosis of the left main bronchus for which no cause could be found. The first case responded completely to dilatation while the 2nd case demonstrated only partial response. Third case did not improve significantly with dilatation.

Antenatal sonography is an accepted method of evaluating the cervix and lower uterine segment. Dilatation of the endocervical canal can be missed due to compression by an overdistended urinary bladder, intermittent changes in configuration of the cervix, and suboptimal visualization. In order to establish the animal model of papillary insufficiency, 10% formalin solution (0.5-1.0 ml) was injected into duodenal papillae just after cholecystectomy in ten dogs.
