Effectiveness of Systemic Inflammation Response Index (SIRI) Neutrophil-Lymphocyte Ratio (NLR), Derived Neutrophil-Lymphocyte Ratio (dNLR), and Systemic Immune Inflammation Index (SII) for predicting prognosis of acute diverticulitis.

There are many applications to emergency services due to acute colonic diverticulitis. It is necessary to decide whether these patients are complicated, to quickly predict their prognosis, and to decide whether medical or invasive treatment is necessary. We planned to research effectiveness NLR, NLR, SII, and SIRI values calculated using hemogram data can predict the prognosis of acute diverticulitis. We managed a retrospective scanning with patients who applied with a diagnosis of acute diverticulitis between 06/2020 and 04/2023. Demographic data (age, gender, previous surgery, comorbid disea…

[Diverticulosis: the dimensions of a growing problem].

The etiology of diverticulitis remains unexplained. One hypothesis postulates that a diet low in fiber results in low-volume feces, which in turn leads to a segmental increase in muscle tone with bulging of the mucosa. Diverticulitis then occurs through micro-/macroperforation of the resulting diverticulum. Four grades are distinguished ranging from local mesenteric inflammation (grade I) to fecal peritonitis (grade IV). Asymptomatic diverticulitis requires no treatment. Diverticular bleeding must be carefully distinguished from upper gastrointestinal tract bleeding, carcinoma, and angiodyspla…

Essential updates 2018/2019: Colorectal (benign): Recent updates (2018-2019) in the surgical treatment of benign colorectal diseases.

This review outlines current topics on the surgical treatment of benign colorectal diseases, with a focus on inflammatory bowel disease (IBD) and diverticulitis. Treatment options for IBD and diverticulitis have evolved in the last few years as a result of medical advances in technology and new clinical trials. Therefore, treatment options and strategies need to be updated to provide optimal care for patients. The purpose of this review is to elucidate recent global trends and update the surgical treatment strategy for IBD and diverticulitis based on literature published in the past 2 years. P…

The surgeon's view.

Diverticular disease affects up to 50% of people by the time they reach the age of 80. The major complications of diverticular disease (abscess, perforation, fistula, obstruction and bleeding) have their own management pathways, but the treatment of uncomplicated diverticulitis is controversial. On initial presentation, diverticulitis is always treated conservatively. Whether this should be followed by resectional surgery has been the subject of speculation for many decades. The American Society of Colon and Rectal Surgeons recommended in 2000 that an elective resection should follow two attac…

Laparoscopic treatment for acute diverticular disease.

Diverticular disease of the sigmoid colon involves more than 50% of population over 60 years, and much more in people older than 80 years. Most patients remain asymptomatic, but, about 10-20% develop complications requiring surgery. Colonic diverticulitis represents an acute bowel inflammation, in many cases, confined only to the sigmoid and descending colon. Recurrent attacks and complications of diverticulitis require surgical procedure, although most cases can be managed medically. The cause of acute diverticulitis remains obscure. It has been speculated that obstruction at the mouth of the…

Studies on complications of diverticular disease of the colon.

Out of 1,124 cases of diverticular disease of the colon seen during the past 15 years, 27 cases (2.4%) of diverticulitis and 44 cases (3.9%) of diverticular hemorrhage were found. The incidence of diverticulitis was more frequent in the both-sides colon type, and also in the multiple form having 10 or more diverticula. The average age was higher in diverticulitis of the sigmoid colon (57.8 years) than in diverticulitis of the right-side colon (47.9 years). Twenty-two cases (81%) of these patients were recovered by medical treatment, and in 5 cases (19%) of these, elective colectomy was carried…

Natural orifice specimen extraction in robotic sigmoid resection for diverticulitis is associated with reduced postoperative pain and shorter operative time compared with transabdominal specimen extraction: a retrospective single-center study.

BACKGROUND: Natural orifice specimen extraction (NOSE) has emerged as a minimally invasive alternative to transabdominal specimen extraction (TASE) in robotic sigmoid resection for diverticulitis. However, comparative perioperative data remain limited. This study compared perioperative and postoperative outcomes of robotic sigmoid resection using NOSE versus TASE. METHODS: Fifty-two patients undergoing elective robotic sigmoid resection for diverticulitis between 2022 and 2025 were retrospectively analyzed, including 26 NOSE and 26 TASE procedures. Primary endpoints were operative time, posto…

[Three cases of diverticular perforation of the caecum and ascending colon].

Perforation of the ascending colon secondary to diverticulitis is an uncommon surgical emergency. It is usually diagnosed during exploratory laparotomy for suspected appendicitis. We report three cases of cecal perforation due to diverticulitis. Imaging work-up, especially computed tomography can establish the preoperative diagnosis of cecal diverticulitis and guide its management.

Prospective study on the management of patients with complicated diverticular disease.

INTRODUCTION: Diverticular disease is a common condition with high morbidity and mortality related to its complications. The aim of this study was to assess the predictive role of acute diverticulitis in the development of further complications from diverticular disease. PATIENTS AND METHODS: Prospective assessment of all patients with complicated diverticular disease over a 1-year period in a large teaching hospital was undertaken. All patients had documented evidence of their diagnosis by radiological, endoscopic or histopathological techniques when feasible. RESULTS: Seventy-seven patient…

[Modern therapy of diverticular disease].

New concepts are developed in the therapy of diverticular disease. 5-aminosalicylates can be administered in patients with slight forms of diverticulitis. In moderate diverticulitis antibiotics should be applied alternatively or additionally. In cases of severe diverticulitis patients should be kept fasting with parenteral nutrition and intravenous broad spectrum antibiotics. Small abscesses can be treated conservatively while abscesses larger than 4 cm should be drained in a first step and than treated surgically. A free perforation is still an absolute indication for emergency operation. In …

Diverticular perforation of terminal ileum associated with chemotherapy for non-small cell lung carcinoma: a case report.

A 71-year-old man was diagnosed with advanced non-small cell lung carcinoma and treated with chemotherapy developed ileocecal diverticulitis three times over the last 2 months of receiving second-line treatment. During the fourth diverticulitis event, the patient presented with fever and abdominal pain, worsening after 5 days. Abdominal computed tomography showed ascites and intra-abdominal free air, suggesting bowel perforation with acute diffuse peritonitis. We performed emergency surgery; the surgical findings showed diverticulosis with perforated diverticula in the ileocecal region. We per…

[Solitary diverticulum of the cecum: apropos of 2 cases].

Diverticulitis of the cecum and ascending colon represent 1.5% of colonic diverticulitis. The preoperative diagnosis is made in 0 to 23% of cases. Two typically misdiagnosed cases are reported. A review of the literature reveals that CT Scan is useful when atypical pain is present in the right lower quadrant. If diagnosed preoperatively, the uncomplicated caecal diverticulitis can be managed conservatively.

Review article: the pathophysiology and medical management of diverticulosis and diverticular disease of the colon.

BACKGROUND: The incidence of diverticulosis and diverticular disease of the colon, including diverticulitis, is increasing worldwide, and becoming a significant burden on national health systems. Treatment of patients with diverticulosis and DD is generally based on high-fibre diet and antibiotics, respectively. However, new pathophysiological knowledge suggests that further treatment may be useful. AIM: To review the current treatment of diverticulosis and diverticular disease. METHODS: A search of PubMed and Medline databases was performed to identify articles relevant to the management of…

[Diagnostic pitfalls of complicated colonic diverticulosis].

Irritable bowel syndrome and (or) non complicated diverticulosis, associated with fever, could simulate diverticulitis. Cancer of the sigmoid colon appears the main differential diagnosis, when diverticulitis is associated with an atypical or complete colonic stenosis on opaque enema, with a vesicoenteric fistula or with a peritonitis due to a colonic perforation. Even at laparotomy, a pseudotumoral diverticulitis cannot easily be differentiated from a colonic carcinoma. Acute diverticulitis of the caecum or ascending colon is usually mistaken for acute appendicitis. When massive and life-thre…

[Clinical analysis of 554 patients with colorectal diverticulosis].

Objective: Most patients with asymptomatic colorectal diverticulosis are easily overlooked. However, some of diverticulosis become diverticulitis, bleeding and even perforation, which cause extensive harm to patients. The purpose of this study is to analyze the incidence, clinical features, diagnosis and treatment of colorectal diverticulosis in order to improve the clinical understanding of diverticulosis and its related complications. Methods: A descriptive cohort study was carried out. Clinical data of 554 patients with colorectal diverticulosis confirmed by CT, colonoscopy, digestive tract…

Incidence, factors, and resultant management associated with small bowel diverticular perforation: A retrospective review.

BACKGROUND: Small bowel diverticula are uncommon, often asymptomatic, and typically discovered incidentally. Small bowel diverticulitis with perforation is even more rare and potentially life-threatening. METHODS: Given the paucity of literature and lack of management guidelines, a single-center retrospective study was conducted from 2015 to 2024 on patients diagnosed with small bowel diverticulitis. RESULTS: Of 32 patients with small bowel diverticulitis, 14 (44%) had perforation. Eight of these (57%) were managed operatively, including two initially treated non-operatively. All 32 patients…
