Sports and occupational injuries of the elbow: MR imaging findings.

Elbow injuries may limit participation in sports and occupational endeavors, as well as activities of daily living. Conventional radiography is the appropriate initial screening technique for evaluation of possible osseous injury or arthritis/arthrosis, but often it offers little information concerning soft-tissue derangement, which is a common source of dysfunction. MR imaging can evaluate these soft tissue to assist in diagnosis, as well as in planning treatment and assessing response to treatment. This essay illustrates MR findings of both the chronic overuse and the more acute traumatic so…

The relation between cubital tunnel syndrome and the elbow alignment.

The authors reviewed forty-three surgically managed cases of cubital tunnel syndrome treated during the past twelve years. Surgery had been performed on thirty-one men and twelve women. Their average age was 46.8 years old. The etiology of cubital tunnel syndrome had been determined as follows: There were twenty-three cases of osteoarthritis of the elbow joint. There were four cases of isolated cubitus valgus deformity. There were three cases of isolated cubitus varus deformity. There were three cases of rheumatoid arthritis. One patient had sustained trauma to the cubital tunnel. In nine othe…

Unreduced dislocation of the elbow: case report and review of the literature.

A case report of an unreduced dislocation of the elbow requiring late open reduction is presented, and the literature is reviewed. Early reduction of elbow dislocation, by closed or open means, is of paramount importance if good functional results are to be obtained. Closed reduction of an elbow dislocation is unlikely to be successful if attempted later than 21 days after the injury. The results are acceptable if open reduction is undertaken within 3 months of the injury, but after 6 months the results are disappointing and arthroplasty is an alternative.

Fascial deformation in the lateral elbow region: a conceptual approach.

In embalmed preparations, the antebrachial fascia in the lateral elbow region is shown to be deformed by load application to the triceps muscle. From this fascia, muscles arise which are primarily concerned with the extension of wrist and fingers. In the case of lateral epicondylitis (tennis elbow), the superficial site of attachment of these extensors at the lateral epicondyle is extremely painful. Triceps training may help to diminish (or prevent) this pain by altering the forces acting at the lateral epicondyle.

Generalizability of grip strength measurements in patients with tennis elbow.

The purpose of this study was to determine the interrepetition and interoccasion generalizability of grip strength measurements in patients with the diagnosis of proximal extensor carpi radialis brevis tendinitis. Thirty-five consecutive patients (mean age = 44.5 +/- 8.6 years; mean duration of symptoms = 3.9 +/- 3.5 months) fulfilling the eligibility criteria participated in the study. Measurements of pain-free grip strength and maximum grip strength for the involved limb and maximum grip strength for the uninvolved limb were taken on two test dates within seven days of each other. We calcula

[Open reduction of the elbow. Apropos of 29 neglected dislocations].

The authors describe results and technical difficulties faced in a series of 29 elbow dislocations using the open technic, then observed over a period between 5 years and 4 months. Results obtained are satisfactory, taking into consideration the primary stiffness. Results on stability are better than those obtained by resection of the lower part of the humerus. Dislocations by the open technic have to be envisaged first in neglected luxations. Indications are limited to patients crippled and limited to non-functional position.

[Extraction of a foreign body from the elbow using arthroscopy].

Arthroscopy of the elbow is now a good diagnostic and therapeutic method, particularly for the removal of loose bodies which are frequently found in the elbow, whatever their etiology. The authors have investigated the advantages and disadvantages of different sites of entry and the technique of removal of loose bodies in five cases. The technical ease of the procedure and the simplicity of the after-care makes it the method of choice in this condition.

Prevention and treatment of elbow and shoulder injuries in the tennis player.

Tennis injuries are common in both the upper and lower extremities. The most common, and often most difficult, upper extremity injuries are shoulder tendinitis and tennis elbow (lateral and medial). Key considerations in the treatment of tendinitis include an understanding of the injury process and the resultant character and quantity of the pathologic spectrums. Tendon degeneration rather than tendon repair is the primary pathologic entity secondary to intrinsic muscle-tendon overload. For best treatment results, the protocols of treatment, both surgical and nonsurgical, must be individualize…

Carcinomatous arthritis of the elbow caused by metastatic breast carcinoma.

We report a case of persisting chronic monoarthritis of the elbow in a 54-year-old woman with previous breast surgery for carcinoma. An open biopsy of the elbow showed a chronic nonspecific synovitis and a poorly differentiated metastatic adenocarcinoma on the radial head. This is the first report of elbow arthritis due to metastatic breast carcinoma.

Simultaneous dislocation of the elbow and the proximal radio-ulnar joint.

Simultaneous dislocation of the elbow and the proximal radio-ulnar joint is rare. Two children with this uncommon injury are reported. One child had a transverse divergent dislocation that was treated successfully by closed reduction. The other had a convergent dislocation (or translocation of the radius and ulna) which needed open reduction. Convergent dislocation is even rarer than transverse divergent dislocation, and this is believed to be the first time it has been reported.

Entrapment of the median nerve after posterior dislocation of the elbow.

The authors present two cases of complete median nerve paralysis due to entrapment after posterior dislocation of the elbow. Previous literature on the subject matter is reviewed, and management and therapeutic indication of these lesions are discussed.

Interposition arthroplasty in the management of advanced haemophilic arthropathy of the elbow.

Six patients with severe haemophilic arthropathy of the elbow have been treated by limited excision arthroplasty using a sheet of silicone rubber as an interposition membrane. There were no complications. The symptoms and signs before and after operation have been assessed using the grading system advocated recently by the World Federation of Haemophilia. In all cases there was relief of pain and increased movement. Moreover, the subsequent incidence of spontaneous haemorrhage into the elbow has been reduced with considerable cost benefit.

[Radius head resection as a reconstructive measure of the elbow joint].

The authors followed up 55 patients to check on the clinical manifestations prevailing ten years after resection of the head of the radius with particular reference to the pattern presented in the radiocarpal joint. Particular attention was paid to functional and roentgenological changes in the regions of the elbow and wrist. Overall results were unsatisfactory, although a clear improvement in function could be seen in old fractures of the head of the radius, although the function did not improve beyond that achieved by primary resection of the head. No convincing improvement was obtained in o…

[A clinical study on arthrography of the elbow joint (author's transl)].

There are many patients who visit a clinic with some complaints at the elbow joint. In such cases, it is customary to evaluate radiographs in making diagnosis. However, it is often difficult to interpret the X-ray films due to structural complexity of the elbow joint. Although in some cases arthrography is performed, there is neither standard technique nor standard method of projection established, hence the evaluation of the arthrogram is often quite difficult. The purpose of this paper thereby is to study the role of the arthrography of the elbow joint as a diagnostic tool second to plain X-…

Dislocation of the elbow and its social consequences for an African elephant.

A dislocated elbow of a male elephant calf (Loxodonta africana) in the Addo Elephant National Park resulted in it being harassed by other elephants and consequently leaving its maternal herd at the age of 6 years, rather than the more usual age of 9-10 years. In the absence of large predators the elephant had survived for 9 years and had adapted its locomotion and intraspecific behaviour to its injury. The humeral joint of the affected forelimb was carried in partial extension while the cubital and carpal joints were permanently in partial flexion. Substantial secondary osteoarthritic changes,…
