BACKGROUND: Atrial septal defect (ASD) is a congenital heart lesion that often remains undiagnosed until adulthood. The reasons for this may be multifactorial. It is, however, known that closure of a hemodynamically significant ASD improves exercise capacity. This study aimed to explore whether the aerobic capacity in late adolescence is associated with time to diagnosis and intervention in adult men with late diagnosis of an atrial shunt.
METHODS: The Swedish Military Conscription Service Register contains data on exercise tests performed in late adolescence. By linking these data with the N

The adverse physiologic effects of pectus excavatum and subsequent resolution following correction have been a subject of controversy. There are numerous accounts of patients reporting subjective improvement in exercise tolerance after surgery, but studies showing clear and consistent objective data to corroborate this phenomenon physiologically have been elusive. This is partially due to a lack of consistent study methodologies but even more so due to a mere paucity of data. As experts in the repair of pectus excavatum, it is not uncommon for pediatric surgeons to operate on adult patients. F…

BACKGROUND: Little is known about physical activity in daily life among patients with sarcoidosis. Fatigue is a frequent and disabling symptom that might negatively affect physical activity levels.
METHODS: In patients with sarcoidosis, we measured physical activity (steps per day) by accelerometry (SenseWear Armband) for 1 week. We assessed lung function (DLCO, FVC), exercise capacity (6-min walking distance [6MWD]), health-related quality of life (St George's Respiratory Questionnaire [SGRQ]), generic quality of life (12-Item Short-Form Health Survey [SF-12]), and fatigue (Multidimensional

BACKGROUND: Pure exercise intolerance associated with exclusive affection of skeletal muscle is a very rare phenotype of patients with mitochondrial myopathy. Moreover, the exercise intolerance in these rare patients is yet not well explored, as most of known cases have not been assessed by objective testing, but only by interview. We report a patient with a mitochondrial DNA (mtDNA) mutation that gives rise to an exclusive myopathy associated with exercise intolerance and ophthalmoplegia. We quantified the patient's exercise intolerance through detailed exercise testing.

BACKGROUND: A number of reviews have reported the influence of exercise therapy (ET) for the treatment of intermittent claudication (IC). However, a complete overview of different types of ET is lacking. The aim of this meta-analysis was to study the effect of supervision on walking capacity in patients with IC. It was hypothesized that there was a positive treatment effect in relation to the intensity of supervision and improvement in walking capacity (i.e., a "dose-response" hypothesis).
METHODS: A systematic search in the Cochrane Central Register of Controlled Trials, MEDLINE, and EMBASE

Spina bifida (SB) is the most common birth defect in United States that results in permanent lifelong disability according to the Spina Bifida Association. Advancements in medical care have led to a longer life span and an increase in the risk of secondary conditions, for example, obesity, with age. The need to maintain a healthy and active lifestyle is even stronger in adults with SB than the general population. Our objective was to fill a gap in the literature by highlighting the current state of the literature on health-related measures of fitness, exercise, and physical activity (PA) in ad

We present a case of Ebstein's anomaly revealed by symptoms occurring at high altitude. A 63 year old male with no previous symptoms of cardiovascular disease developed acute severe shortness of breath when walking up a flight of stairs at an altitude of 2500 m. Echocardiography showed apical displacement of the tricuspid valve. Exercise capacity at sea level was normal, as were oxygen saturation at rest and maximal exercise. We think that the patient's symptoms can be explained by increased afterload on the right ventricle during exercise at high altitude. The functional capacity of the small

OBJECTIVE: To evaluate the relationship between maximal exercise capacity and walking capacity in hemiplegic stroke patients.
DESIGN: An uncontrolled observational study was conducted in the physical medicine and rehabilitation unit in an academic hospital. Participants were 21 hemiplegic stroke patients (18 men, 3 women; age, 18-70 yrs) whose stroke occurred >3 mos before the study and who could walk independently with or without walking aids. Main outcome measures were peak oxygen uptake, workload, walking capacity, and motor impairment.
RESULTS: There was a significant correlation between

BACKGROUND AND PURPOSE: Previous investigations have demonstrated that a regimen of high-intensity inspiratory muscle training (IMT) resulted in changes in ventilatory function and exercise capacity in patients with chronic lung disease, although the effect of high-intensity IMT in subjects who are healthy is yet to be determined. The purpose of this study, therefore, was to examine whether high-intensity IMT resulted in changes in ventilatory function and exercise capacity in subjects who were healthy.
SUBJECTS: Twenty subjects were randomly assigned to 2 groups.
METHODS: The training group

OBJECTIVE: Peak cardiorespiratory, metabolic, and perceptual responses in healthy men (n=11) and women (n=11) were compared during two different upper body exercise modes: standardized arm cranking and task-specific pushing-pulling.
METHODS: Each subject completed to volitional exhaustion both an incremental arm cranking and a pushing-pulling exercise continuously, in a random order, on two separate days. Physiological responses were monitored using an automated metabolic cart and a heart rate monitor, and the rating of perceived exertion was obtained using a 15-point Borg Scale.

Exercise has emerged as an effective therapeutic strategy in cancer management. Long-term supervised exercise programs, including resistance training and aerobic training, can improve body composition and physical fitness, modulate circulating factors (e.g., hormones, cytokines), enhance treatment tolerance, and reduce side effects, potentially influencing cancer progression and reducing mortality risk. Despite advancements in exercise oncology, opportunities remain to refine exercise prescription through the integration of advanced methodologies such as periodization and autoregulatory progra

BACKGROUND/AIMS: Aortic stenosis-induced chronic pressure overload leads to cardiac dysfunction and congestive heart failure. The pathophysi … nted higher levels of TNFα and iNOS, which were attenuated by ET.
CONCLUSION: Moderate ET improves exercise tolerance, reduces heart failure features, and attenuates diastolic dysfunction. In the myocardium, ET decreases the cross-sectional area of the cardiomyocyte and preserves the β-adrenergic responsiveness, which reveals that the adjustments in β-adrenergic si …

Based on the linear relationship between cardiac output and oxygen uptake direct breath-to-breath gas exchange measurements during exercise … pendent of the implanted pacing device. The improved aerobic capacity resulted in a 17% increase in exercise tolerance and a 19% increase of exercise time. Cardiopulmonary exercise testing appears to be a useful noninvasive technique to quantify the cardiopulmonary benefit of rate response pacing.

To investigate the sympathetic nervous system (SNS) responses and circulatory responses to exercise in eight patients (five male and three female) with complete atrioventricular block (CAVB) treated with atrioventricular (AV) synchronous pacing, a symptom-limited, multistaged treadmill stress test was performed, and plasma norepinephrine (NE) and circulatory parameters were measured at rest, at peak exercise, and in the recovery period. The eight patients were tested using the fixed AV interval (150 or 156 msec). Their exercise tolerance was generally poor. In all measured points, plasma NE le…
