Management of bone loss that occurs after severe trauma of open lower extremity fractures continues to challenge reconstructive surgeons. The authors report four cases of inferior dislocation of the proximal tibiofibular joint. A literature search disclosed reports on superior, anterolateral and posterolateral dislocation, but none on inferior dislocation of the proximal tibiofibular joint. We report the results of the treatment of nine children with an aneurysmal bone cyst of the distal fibula (seven cysts were juxtaphyseal, and two metaphyseal). Interfragmentary lag screw internal fixation was combined with external fixation to treat five difficult oblique lower extremity fractures. Uncomplicated primary bone healing was achieved in all five cases in spite of crush injury, devascularization, or local infection.
