BACKGROUND: A patient with intraocular silicone oildeposits underwent paracentral corneal incision but the corneal sutures were never removed and 7 years later a perforating corneal transplantation was performed due to corneal opacification.
METHODS: The corneal button was fixed in 10% buffered paraformaldehyde, embedded in celloidin and sections 200 microns thick were cut and stained with van Gieson. Some of these sections were embedded in paraffin and 5 microns thick sections were cut.

OBJECTIVE AND IMPORTANCE: Placement of a ventriculoperitoneal (VP) shunt is the most common form of treatment for hydrocephalus. Thoracic complications with VP shunts are rare, but we present the second documented case of the distal migration of the distal catheter of a VP shunt into the heart.
CLINICAL PRESENTATION: A 14-year-old boy, who underwent placement of a right occipital VP shunt at another institution after closed-head injury, presented with hypertension. Plain chest x-rays and computed tomography revealed the distal catheter to be in the right ventricle of the heart.

Two cases of uterine perforation are described, occurring 11 days and 4 months, respectively, after the insertion of GyneFix, a frameless intra-uterine contraceptive device (IUD). In both the cases initial ultrasound scan showed the intra-uterine position of the device. Removal of the IUD, either by laparoscopy or laparotomy, had to be carried out. Awareness of this complication, insertion of GyneFix by a trained operator, appropriateness of ultrasound scan monitoring and possible underreporting of this complication are discussed.

A new technique for retrieval or repositioning of migrated or malpositioned silicone punctal plugs is described. Malposition is known to occur secondary to either spontaneous migration or forceful insertion. This technique offers considerable advantages over current treatment options including simplicity and minimal trauma to both the plug and the nasolacrimal system.

We report the case of a 20-month-old girl who underwent Gianturco coil embolization to a patent ductus arteriosus in May 1997. The coil migrated to the pulmonary artery. After unsuccessful attempts to retrieve it with snares and forceps, we engaged the coil with an end-hole balloon catheter and pulled it down to the right ventricle. A premature child with congenital complete heart block had an epicardial single-chamber pacemaker implanted at 2 days of age. At 21 months of age, while sitting or standing, the patient's right anterior thigh muscles contracted at her pulse rate. Surgical exploration revealed a free-floating pacemaker in her peritoneum. A new dual-chamber pacemaker was implanted into the abdominal wall with resolution of the child's symptoms.

Skin staples are ideal for pediatric scalp laceration closure because of their rapidity of placement and economy and ease of use. We report two cases of rotatory staple migration necessitating improvised removal techniques. Clinicians should be alert for this complication, which may result from a combination of staple design, local anatomic factors, superficial placement, and prolonged delay prior to removal.

OBJECTIVES: Expandable metallic prostheses have been used widely for malignant biliary stenoses and are being used increasingly for malignant dysphagia and esophago-airway fistulas. Stenting is a modern method making possible to open malignant oesophageal stenosis. The technique is associated with various complications. The authors describe the management of a rare stent migration to the aboral part of the GI tract, causing an acute abdomen. Based on their own experience with dislocated stents they recommend, contrary to the literature, an active approach when migration to the aboral of the GI tract occurs. Early stent extraction even when the course is uneventful is advantageous.

We report a patient who developed oesophageal stricture after accidental ingestion of acid. During one of the oesophageal dilation sessions, a Savary-Gillard guide-wire got entrapped in the stomach and had to be removed surgically. A Foley catheter, placed for feeding purposes, migrated into the proximal small intestine causing acute intestinal obstruction. The balloon of the Foley catheter had to be punctured using a sclerotherapy needle and the catheter withdrawn.

Premature dislocation, a potential disadvantage of conventional, unilateral silicone intubation for lacrimal disease, can be alleviated with a firmly-tied, bilateral, superior canaliculus intubation. Since the looped silicone stent can be removed easily at home by parents, this technique is particularly beneficial for remote patients for whom postoperative, clinic removal is a financial burden.

A 38-year-old woman was treated for malignant stricture by the endoscopic placement of a biliary endoprosthesis. She developed a large retroperitoneal biloma secondary to acute duodenal perforation by the stent. We describe the successful percutaneous retrieval of the biliary stent through a retroperitoneal approach which also permitted drainage of the biloma.
