Effects of a cannabinoid receptor agonist on colonic motor and sensory functions in humans: a randomized, placebo-controlled study.

Cannabinoid receptors (CBR) are located on cholinergic neurons in the brain stem, stomach, and colon. CBR stimulation inhibits motility in rodents. Effects in humans are unclear. Dronabinol (DRO), a nonselective CBR agonist, inhibits colonic motility and sensation. The aim of this study was to compare effects of DRO and placebo (PLA) on colonic motility and sensation in healthy volunteers. Fifty-two volunteers were randomly assigned (double-blind) to a single dose of 7.5 mg DRO or PLA postoperative with concealed allocation. A balloon-manometric assembly placed into the descending colon allowe

Treatment of diarrhea in patients with inflammatory bowel disease: concepts and cautions.

Diarrhea continues to be a prevalent symptom in patients with inflammatory bowel disease (IBD), requiring a wide differential diagnosis to define the pathophysiologic mechanisms in individual patients. It is essential that physicians properly evaluate complaints of diarrhea by assessing both patient symptoms and potential physiologic impacts on fluid and electrolyte status. Underlying mechanisms of diarrhea with IBD are the location, extent, and severity of inflammation; malabsorption; altered motility; and iatrogenic causes such as medications, diet, and antibiotic-associated colitis (eg, Clo

Segmental and total oesophageal transit time in patients with corrosive-induced oesophageal stricture.

INTRODUCTION: The mainstay of treatment of corrosive oesophageal strictures is endoscopic dilatation. However, even after adequate dilatation, some patients may continue to have dysphagia. An associated oesophageal motor dysfunction in such patients may contribute to the persistence of dysphagia. AIM: To assess oesophageal motor dysfunction in patients with corrosive strictures using segmental and total oesophageal transit time by radionuclide scintigraphy. METHODS: Thirty-five patients with corrosive-induced oesophageal strictures were evaluated after having achieved a dilatation of 15 mm.

A comparison of methods of assessment of scintigraphic colon transit.

UNLABELLED: There is no standard method of analysis of scintigraphic colonic transit investigation. This study was designed to compare 4 techniques. METHODS: Sixteen subjects (median age, 37.5 y; range, 21-61 y), who had sustained a spinal cord injury more than a year before the study, were given a pancake labeled with 10-18 MBq of (111)In bound to resin beads to eat. Anterior and posterior images were acquired with a gamma-camera 3 h after the meal and then 3 times a day for the next 4 d. Seven regions of interest, outlining the ascending colon, hepatic flexure, transverse colon, splenic fle

The effect of intestinal plication on intestinal transit time in rats.

Short bowel syndrome (SBS) can occur after extensive intestinal resections. In SBS, the aim of surgical techniques is to prolong intestinal transit time (ITT) and to increase the absorptive surface. This experimental study was conducted to research the effect of extramucosal intestinal plication on ITT in rats. Thirty Sprague-Dawley rats were divided into three groups. In the control group, barium solution was administrated by gavage. Forty-five minutes later, the rats were sacrificed, and the total length of their small bowels (SBs) and the distance of the barium in the SBs were measured. The

Abnormal colonic propagated activity in patients with slow transit constipation and constipation-predominant irritable bowel syndrome.

BACKGROUND: The pathophysiological basis of constipation is still unclear, and the role of colonic dysfunction is debated, especially in irritable bowel syndrome. Objective data are quite scarce, especially concerning colonic propulsive activity. AIMS: To evaluate high- and low-amplitude colonic propulsive activity in constipated patients (slow-transit type and irritable bowel syndrome) in comparison with normal controls. PATIENTS AND METHODS: Forty-five constipated patients (35 with slow-transit constipation and 10 with constipation-predominant irritable bowel syndrome) were recruited, and

Buprenorphine: new pharmacological aspects.

Buprenorphine is an opioid analgesic, derived from thebaine. Buprenorphine was initially classified as a "mixed agonist-antagonist analgesic" or a narcotic antagonist analgesic. The work of Martin et al (1976) on the animal model of the chronic spinal dog substantiated the substance's action as partial agonist at the mu-opioid receptor. These findings were underscored by the substance's general pharmacological profile. Further, buprenorphine was one of the first narcotic analgesics to be assessed for its abuse liability in humans. The lower abuse liability of the drug in humans soon turned it

Experimental study of faecal continence and colostomy irrigation.

BACKGROUND: Colostomy irrigation is a useful method of achieving faecal continence in selected conditions, but remains largely underutilized because it is time consuming. This study investigated the effect of modifying irrigation technique (route, infusion regimen and pharmacological manipulation) on colonic emptying time in a porcine model. METHODS: An end-colostomy and caecostomy were fashioned in six pigs. Twenty markers were introduced into the caecum immediately before colonic irrigation. Irrigation route (antegrade or retrograde), infusion regimen (tap water, polyethylene glycol (PEG),

Consequences of reduced fibre intake on digestion, rate of passage and caecal microbial activity in the young rabbit.

The present work was undertaken to study in vivo fibre degradation, rate of passage and caecal fermentation activity (CFA) in the young rabbit (7 weeks old) receiving ad lib. a control (C) or low-fibre (LF) diet (400 and 220 g neutral-detergent fibre (NDF)/kg respectively). As a consequence of the 50% reduction in the dietary fibre level, the voluntary food intake of the rabbits decreased by 25%, and the daily fibre intake was reduced by 60% (26.7 and 10.8 g NDF/d for groups C and LF, respectively). In spite of a longer mean retention time of the fibre particles, the quantity of fibre digested

Effect of Phoneutria nigriventer spider venom on gastric emptying in rats.

The effect of Phoneutria nigriventer spider venom (PNV) on the gastric emptying of liquids was studied in 240 young adult Wistar rats (2-3 months of age) divided into subgroups of 8 animals each. The study was performed in 3 stages. Initially, PNV was injected into rats at doses of 0.19, 0.38 or 0.76 mg/kg and the effect on gastric emptying was assessed 30 min later. In the second stage, a time-course study was performed by injecting 0.76 mg PNV/kg and measuring the effect on gastric emptying 15, 60 and 120 min post-venom. In the last stage, in order to investigate the possible mechanisms of P

Anorectal physiological parameters in chronic constipation of unknown aetiology (primary) and of cerebrovascular accidents--a preliminary report.

Chronic constipation may be secondary to dietary, metabolic or neurological causes such as cerebrovascular accidents (CVA). In certain patients, the cause is unknown (primary idiopathic constipation [PIC]). This study compared the anorectal physiological parameters of 15 PIC patients (all females; median age 40, range 32-48 years) with 7 CVA patients (M:F = 6:1; median age 58, range 51-67 years), to observe the influence of cerebral factors on the anorectal physiology of constipation. Twenty-five normal subjects (M:F = 13:12; median age 47, range 43-50 years) acted as controls. Transit marker

Constipation: physiopathology and classification.

Constipation is commonly defined on the basis of the number of bowel movements in a week (greater than 3 week) or on the stool weight (less than 35 g/day) and on their consistency and expulsion capacity. A pathophysiological classification of constipation subdivides the patients into 2 groups. The first includes those patients in which a propulsion defect is prevalent (inertia coli) and the second one comprehends those in which a predominant expulsion defect is found. This second group consists of patients with myogenic, neurogenic or mechanical disorders. Moreover, we can consider in this gro

A device and technique for gastrointestinal lavage in the horse.

A device and technique for intra-operative gastro-intestinal lavage was developed to remove ingesta from the stomach, large intestine and caecum of horses. The Gastro-Intestinal Lavage System (GILS) is composed of a nozzle connected to both water under pressure and suction. Water jets across an intake portal in the nozzle, breaks up food and debris within the nozzle and is evacuated under negative pressure into the aspirating tube which is connected to a collection drum. The GILS nozzle was introduced at the pelvic flexure through a sterile enterotomy cuff and plastic sleeve. Water was first a

Effects of botulism on ducks drinking saline water.

Mallard (Anas platyrhynchos) ducklings (2 wk old) were given water from natural saline wetlands or fresh water as drinking water for 1 or 2 wk prior to, and after, receiving material containing Clostridium botulinum type C toxin. Water with conductivity ranging from 3,460 to 6,690 mu mhos/cm had no detectable effect on the occurrence or severity of clinical signs of botulism. Ducks drinking water with conductivity of 7,130 mu mhos/cm for 1 wk prior to receiving toxin had more severe clinical signs and greater mortality than did birds drinking fresh water. Ducks given the same water for 2 wk pr

Transitional care for patients with surgical pediatric hepatobiliary disease: Choledochal cysts and biliary atresia.

Choledochal cysts (CDCs) and biliary atresia (BA) are rare pediatric hepatobiliary anomalies that require surgical intervention due to increased risk of malignancy and liver failure, respectively. The underlying disease and operative procedures place patients at risk for long-term complications, which may continue to affect them into adulthood. Lack of a transitional care model in the health-care system potentiates the challenges they will face following aging out of their pediatric providers' care. We sought to elucidate the long-term complications and challenges patients with CDCs and BA fac

[Neuroendocrine system of the pancreas and gastrointestinal tract: origin and development].

Gastroenteropancreatic neuroendocrine tumours (GEP NETs) originate from the neuroendocrine cells through the gastrointestinal tract and endocrine pancreas. The embryologic development of the pancreas is a complex process that begins with the "stem cell" that come from the endodermus. These cells go through two phases: in the first transition the "stem cell" differentiates in exocrine and endocrine cells. This process is regulated by transcription factors such as Pdx1 ("insulin promoter factor 1"), Hlxb6 and SOX9. In the second transition the neuroendocrine cell differentiates in the 5 cell typ
