The purpose of this study was to evaluate the inflammatory response in dento-gingival units with or without attached gingiva in monkeys. Two different types of dento-gingival units with or without attached gingiva were established in premolar and first molar areas of three monkeys. In the experimental group, a part of the keratinized gingiva was removed with periodontal scissors following mucoperiosteal flap procedure, while in the control group, sham surgery was performed. After the surgery, plaque control was performed by mechanical tooth cleaning procedures three times a week for 3 months.

This case report describes a unique creeping attachment that developed mesiobucally on a deep, wide recession (3 mm) and extended along the remaining buccal recession (2 mm) of a maxillary first molar with a full-crown gold restoration subsequent to autogenous gingival grafting. Complete coverage of the root by this degree of creeping attachment on a restored multirooted tooth has not previously been reported in the dental literature.

The biologic width includes both the connective tissue attachment and the junctional epithelium and has a mean dimension of approximately 2 mm. Invading the biologic width with a restoration can result in localized crestal bone loss, gingival recession, localized gingival hyperplasia, or a combination of these three. When restoring teeth that have subgingival caries or fractures below the gingival attachment, a clinical crown-lengthening procedure is needed to establish the biologic width. This article presents three case reports that utilized crown-lengthening procedures.

A pathologic entity, the gingival fenestration, has been described. The lesion is seen infrequently in clinical practice, probably due to the short time span of its existence and the lack of acute symptoms. It is possible that it occurs with greater frequency than generally realized. A hypothesis is presented as to the method of formation of the gingival fenestration and it is suggested that the area should be treated as gingival recession extending to the apical border of the fenestration.

In cases of orthodontic traction of unerupted teeth, gingival recession and long clinical crowns are often seen. Satisfactory esthetic demands are not always met. A case of an unerupted maxillary central incisor is presented, in which traction was used, with emphasis on the surgical technique and the direction of the pull. The result was clinically favorable. This case series evaluated the use of a collagen matrix with a coronally advanced flap procedure for the treatment of multiple recession defects. Fifteen patients with a total of 80 recession defects were included. Root coverage was 85% ± 13% at 6 months and 81% ± 22% at 12 months. Six patients with buccal gingival recession (4 to 6 mm) were treated by means of the guided tissue regeneration technique utilizing a polyglycolactic membrane (Vicryl). Three months following the first surgical procedure, all sites were reentered to evaluate newly formed tissue above the exposed root. Complete root coverage was obtained in four out of six patients; 0.5 and 1.5 mm of recession were still present in the remaining two.
