The spectrum of nail involvement in palmoplantar pustulosis.

Abnormalities of the nail are not a well recognized feature of palmoplantar pustulosis (PPP). However, in a group of 50 patients with PPP, we found nail dystrophy in 15 (30%). The most frequent pattern was subungual pustulation, present in 10 patients, and progressing to nail destruction in two. Onycholysis and pitting of the nail were present in a few patients. In contrast to psoriasis vulgaris, the rate of linear nail growth in PPP was no greater than in matched normal controls. This is another clinical feature of PPP that separates it from psoriasis vulgaris.

Piroxicam photosensitivity associated with vesicular hand dermatitis.

A case of piroxicam photosensitivity in a 42-year-old man is reported. While he was taking piroxicam, eczematous skin lesions on sun-exposed skin and vesicular hand dermatitis on the finger webs and palms developed. He showed a lowered minimal erythema dose (MED) to UVA when a rechallenge with piroxicam was attempted. Photopatch testing with piroxicam was positive.

Irritant cutaneous reactions to N-methyl-2-pyrrolidone (NMP).

Several workers in a small electrotechnical company in Norway experienced irritant reactions of the skin after a few days of working with the solvent N-methyl-2-pyrrolidone (NMP). Due to concern about the health risk of commonly-used organic solvents, the company had chosen to use NMP when one of its products had to be treated with a solvent. After 2 days of work with NMP, 10 of the 12 involved workers displayed acute irritant contact dermatitis of the hands. According to published reports, NMP is not considered to be particularly irritant to the skin. The Safety Data Sheet of a Norwegian sale

Pyogenic granuloma following treatment of verruca vulgaris with cryotherapy and Duoplant.

We report an unusual case of pyogenic granuloma that appeared after the application of liquid nitrogen and 27 percent salicylic acid (Duoplant, Stiefel Inc.) to a verruca vulgaris. Only two cases of pyogenic granuloma following cryotherapy appear in the literature and pyogenic granuloma as a complication of salicylic acid application has not been documented.

Sporotrichosis with bilateral lesions. A case report.

We report on a case of sporotrichosis in which lesions occurred bilaterally on upper limbs at different periods. The patient was an 84-year-old farmer living in Tsukuba City, Japan. The first lesion appeared on the left upper arm and remained untreated. The second lesion appeared on the back of the right hand 5 years later. Findings suggesting internal metastasis could not be confirmed in various examinations. We judged that the lesion of the left upper arm was primary and the lesion at the back of the right hand was caused by autoinoculation.

Aging hands: a technique of hand rejuvenation by laser resurfacing and autologous fat transfer.

Hands are a primary target for actinic damage and premature aging. Until recently, very little could be done to successfully erase the various signs of photoaging. This article presents a simple technique, combining fat transfer and laser resurfacing, to safely and effectively rejuvenate hands and erase the signs of aging.

Specific cutaneous infiltrates in chronic myelogenous leukemia.

Specific leukemic infiltration of the skin is possible in all types of leukemia, and occurs most commonly in monocytic leukemias. We report the case of a 76-year-old woman with chronic myelogenous leukemia who showed specific cutaneous lesions one month before she died. In the absence of other signs of accelerated disease or blast crisis, cutaneous lesions should be considered an ominous sign in chronic myelogenous leukemia.

Hypersensitivity to natural latex.

Rubber hypersensitivity is well described but usually as a contact dermatitis caused by chemicals added during the process of making natural latex or synthetic rubber. IgE-mediated reactions, mainly contact urticaria, have rarely been reported in Europe. We report a case of immediate hypersensitivity to latex. A 34-year-old female operating room nurse developed hand eczema to natural latex. On two occasions, while she was gloving for surgery, she had the following reactions: flushing, tachycardia, urticaria, angioedema, wheezing, and light-headedness. Prick and patch testing to thiuram mix, me

Atopic dermatitis--ichthyosis vulgaris--hyperlinear palms--an ultrastructural study.

Some 30-50% of cases of atopic dermatitis (AD) are believed to be associated with autosomal dominant ichthyosis vulgaris (ADI). The diagnosis of ADI can be proved by the ultrastructural demonstration of fewer and abnormal keratohyalin (KH) granules in all ADI patients, even in clinically unaffected skin. To prove the suggested frequent association of ADI with AD, an ultrastructural investigation of dry skin of 49 AD patients was performed. Only in 2 (4%) patients ADI could be confirmed by electron microscopy. In 17 patients, including the 2 patients with abnormal KH, hyperlinear palms were cli

Pyoderma gangrenosum of the head and neck.

Pyoderma gangrenosum is an ulcerative skin disorder usually associated with an underlying systemic disease. Head and neck involvement is rare, but possibly more common than once thought. The etiology of this disease is unclear, but may be related to an abnormal immunologic response. There are no pathognomonic histologic or laboratory findings; the diagnosis is made by the clinical appearance of the lesions and disease course. Treatment consists of immunosuppression and local wound care in addition to a search for and treatment of an underlying primary systemic disorder. A case report and revie

Rheumatoid nodulosis of the hand.

Rheumatoid nodulosis is an uncommon variant of rheumatoid disease with distinctive clinical and radiographic characteristics and a favorable prognosis. This is in contrast to the poor prognosis for the severe form of rheumatoid disease with which rheumatoid nodules are often associated. A case of rheumatoid nodulosis of the hand treated with complete resection of the nodules is presented with a 2.5-year follow-up. Fourteen cases of documented rheumatoid nodulosis are reviewed and surgical treatment is recommended for both functional and cosmetic reasons.
