Image-based model of atrial anatomy and electrical activation: a computational platform for investigating atrial arrhythmia.

Computer models provide a powerful platform for investigating mechanisms that underlie atrial rhythm disturbances. We have used novel techniques to build a structurally-detailed, image-based model of 3-D atrial anatomy. A volume image of the atria from a normal sheep heart was acquired using serial surface macroscopy, then smoothed and down-sampled to 50 μm(3) resolution. Atrial surface geometry was identified and myofiber orientations were estimated throughout by eigen-analysis of the 3-D image structure tensor. Sinus node, crista terminalis, pectinate muscle, Bachman's bundle, and pulmonary

Intermittent rate-dependent retrograde conduction over a concealed atrioventricular accessory pathway: what is the mechanism?

Intermittent retrograde conduction over an accessory pathway (AP) is quite an uncommon phenomenon. We describe the case of a 60-year-old woman with recurrent episodes of palpitations. Atrioventricular re-entry tachycardia was diagnosed at electrophysiological study. Interestingly, retrograde conduction over the AP was detectable only when ventricular pacing was performed at fast rate. Supernormal retrograde conduction over the AP was considered the most likely explanation of this unusual finding.

[Dissection of the accessory conduction pathways with simultaneous mitral valve surgery].

The case of surgical treatment of a 44-years old female with a double atrio-ventricular accessory pathway and mitral stenosis was described. Fast ventricular rates due to conduction through the accessory pathways during atrial fibrillation in course of mitral stenosis caused severe heart failure, occasionally with deep hypotonia. Combined treatment with prajmaline and propafenone blocked conduction through accessory pathways. The ventricular rate slowed from approx. 200/min to 150/min. Addition of digoxin slowed the ventricular rate down to approx 80-100/min. There were no signs of preexcitati

Pathologic features of myocardial hamartomas causing childhood tachyarrhythmias.

We have observed in 11 infants, aged 2 years or less, a distinct clinicopathologic lesion responsible for tachyarrhythmias that were fatal in 96% (25/26) of previously reported cases. Nine of the 11 patients, who underwent electrophysiologic mapping and surgical excision of the lesion, have survived, with follow-up periods ranging from 1 month to 6 years. The morphologic findings in these 11 patients and in the 26 cases cited in the literature are reviewed. Pathogenic considerations have included viral-induced lesion, cardiomyopathy, neoplasm, and developmental disorder of Purkinje cells. We b

Effect of pacing duration on right ventricular effective refractory period in man.

This study was performed to assess the dependence of the effect of the duration of stimulation on the effective refractory period (ERP). ERP was determined in 12 patients after four consecutive stimuli as well as after continuous stimulation for 1 min applying a constant stimuli interval of 600 msec. After 1 min stimulation all patients showed a significant (P less than 0.01) decrease of the ERP with 236 +/- 23 SD msec in comparison to the ERP of 271 +/- 26 SD msec after four consecutive stimuli. Although it is well established that ERP depends on the previous cycle length, it is additionally

Technological and Clinical Challenges in Lead Placement for Cardiac Rhythm Management Devices.

Cardiac disease is a leading cause of death worldwide. Disturbance in the conduction system of the heart may trigger or aggravate heart dysfunction, affecting the efficiency of the heart, and lead to heart failure or cardiac arrest. Patients may require implantable cardiac rhythm management devices (ICRMDs) to maintain or restore the heart rhythm. ICRMDs have undergone important improvements, yet limitations still exist, presenting important technological challenges. Most ICRMDs consist of a subcutaneous control unit and intracardiac electrodes. The leads, which connect the electrodes to the c

Interatrial transseptal electrical conduction: comparison of patients with atrial fibrillation and normal controls.

INTRODUCTION: This study analyzed the electrophysiologic properties of interatrial transseptal electrical conduction at Bachmann's bundle and the ostium of the coronary sinus (CS os) in response to pulmonary vein (PV) stimuli, which mimicked spontaneous ectopy. METHODS AND RESULTS: Forty patients with atrial fibrillation (AF) referred for ablation (15 persistent AF and 25 paroxysmal AF) and 15 control patients were enrolled in the study. During decremental extra-stimulus pacing from the PVs, right atrial activation was analyzed using noncontact mapping and multipolar catheters. The refractory

Left Ventricular Ejection Fraction and Incident Cardiac Conduction Dysfunction: Exploring the Mediating Effects of Electrophysiological Parameters.

BACKGROUND: Population-based evidence on the predictive role of left ventricular ejection fraction (LVEF) in incident Cardiac Conduction Dysfunction (CCD) and the mediating effects of electrophysiological parameters remains understudied. This study aimed to characterize the relationship between LVEF and incident CCD and explore the potential mediating effects of electrophysiological parameters. METHODS: This prospective cohort study included 32,398 participants (96.6% White ethnicity) from the UK Biobank with analyzable LVEF and electrocardiogram data. Incident CCD was defined as the first oc

[Acute aortic syndrome mimicking anterior wall ST-elevation myocardial infarction].

We present a case of a 58-year-old man presenting with chest pain irradiating to the back and left arm, history of smoking and untreated hypertension. The anamnesis, symptoms and ECG findings consisting of ST elevation in leads aVR and V1-V2 suggested ST segment elevated myocardial infarction. Performed ECHO examination revealed possible acute aortic dissection. Considering haemodynamic instability, augmentation of chest pain and passing time which was obviously worsening the prognosis patient was submitted to aortography which finally proofed acute aortic dissection. Patient was subsequently

The Emerging Role of Left Bundle Branch Area Pacing for Cardiac Resynchronisation Therapy.

Cardiac resynchronisation therapy (CRT) reduces the risk of heart failure-related hospitalisations and all-cause mortality, as well as improving quality of life and functional status in patients with persistent heart failure symptoms despite optimal medical treatment and left bundle branch block. CRT has traditionally been delivered by implanting a lead through the coronary sinus to capture the left ventricular epicardium; however, this approach is associated with significant drawbacks, including a high rate of procedural failure, phrenic nerve stimulation, high pacing thresholds and lead disl

The petrified heart in sepsis.

We report the case of a 33-year-old transsexual man who developed severe sepsis after an accidental intravenous injection of urine (3-5 ml) instead of methadone. He died unexpectedly 28 days after the onset of sepsis. On postmortem examination, the outstanding findings were restricted to the heart with an unusual macroscopic presentation. On histological examination extensive calcifications of the heart muscle, particularly of the left ventricle were found. The pattern of calcifications on the right ventricle was also striking. In contrast, the entire cardial conduction system was unaffected.

Cardiac damage in polymyositis associated with antibodies to tissue ribonucleoproteins.

Cardiac damage, consisting of mild diffuse myocarditis or severe inflammation and fibrosis of the conduction system or both, occurs in more than 70% of patients with idiopathic polymyositis. The lesions are strikingly similar to those detected in the infants of mothers with connective tissue diseases. In these infants the damage is associated with the transplacental passage of a maternal antibody to tissue ribonucleoproteins (anti-Ro). The same antibody was identified in 60% of 55 patients with polymyositis and in 69% (23/33) of those with associated cardiac damage including four with complete

[High frequency current ablation of supraventricular tachyarrhythmias in congenital heart defects].

Supraventricular tachycardia is a frequent cause of disease in patients with congenital heart defects and has a potentially high impact on quality of life, morbidity and mortality of this patient cohort. Conventional treatment often fails to avoid recurrences of tachycardia in a long-term perspective. Potential side effects of antiarrhythmic drugs include aggravation of heart disease related disturbances of impulse generation and conduction properties or negative inotropic effects on haemodynamically impaired ventricular chambers. For these reasons, interventional electrophysiology is increasi

[Intermittent bundle branch block: a clinical model for the study of electrophysiological phenomena].

Disorders of intraventricular conduction (bundle branch block and hemiblock) are usually stable and remain unchanged irrespective of heart rate. Not infrequently, however, their appearance is related to the duration of the cardiac cycle, so that they appear and disappear with changes in heart rate. This may not even represent a pathological phenomenon, since sudden and consistent changes in cardiac cycle can result, even physiologically, in aberrant conduction. However, when a bundle branch block appears intermittently for simple and progressive increments, or even deceleration, of the sinus r

A case of Kearns-Sayre sindrome with autoimmune thyroiditis and complete atrio-ventricular block.

The Kearns-Sayre syndrome, (characterized by its onset before the age of 20 years, chronic ophthalmoplegia, pigmentary retinal degeneration and at least one of the following symptoms: ataxia, heart block and high protein content in the cerebrospinal fluid) is a severe variant of chronic progressive external ophthalmoplegia with frequent rearrangements of the mitochondrial DNA (mtDNA). The aim of this paper is to report a sporadic paediatric case of Kearns-Sayre syndrome with mtDNA heteroplasmic deletion, absence of cytochrome c-oxidase in many muscle fibers, autoimmune thyroiditis, complete at

Congenitally complete heart block. Developmental aspects.

Three cases of congenitally complete heart block are described of hearts in which other minor congenital malformations were not in themselves severe enough to disrupt the atrioventricular conduction system. The cases fitted well into the categorization of complete heart block suggested by Lev. Two exhibited lack of communication between the atrial and conducting tissues, the other had discontinuity of the penetrating atrioventricular bundle. In an attempt to explain why this discontinuity between different segments of the conducting tissues occurs, we re-examined several series of graded human
