Aortic valve stenosis is one of the most frequent valvular heart diseases requiring treatment in industrialized countries. The symptom onset is associated with a significantly increased mortality, so that there is a clear indication for treatment in patients with severe, symptomatic aortic valve stenosis; however, data on the optimal treatment of patients with asymptomatic aortic valve stenosis are scarce. Smaller studies in the field of cardiac surgery suggest that early surgical valve replacement is superior to a conservative approach. For this reason, the results of additional adequately po

Valvular procedures have become increasingly safe and efficacious. The number of procedures and prostheses that are available increases each year. However, a precise comparison of alternative treatments for patients with valvular heart disease requires an accurate and unbiased recording of the results. The American Association for Thoracic Surgery and the Society of Thoracic Surgeons have developed guidelines that are intended to permit a comparison of alternative procedures and prostheses employed in the treatment of patients with valvular heart disease. These guidelines have been followed by

The management of concomitant aortic and aortic valve disease with left ventricular assist device (LVAD) implantation for patients with severe cardiomyopathy is challenging, and has not been established given the complexity of LVAD surgery with concomitant aortic interventions. A 45-year-old patient presented to our institution with end-stage heart failure symptoms and non-ischemic cardiomyopathy. The patient was found to have a bicuspid aortic valve, severe native aortic regurgitation, a significant ascending aortic aneurysm, and severely depressed left ventricular (LV) function requiring two

Prosthesis-patient mismatch (PPM) is present when the effective orifice area of the inserted prosthetic valve is too small in relation to body size. Its main haemodynamic consequence is to generate higher than expected gradients through normally functioning prosthetic valves. This review updates the present knowledge about the impact of PPM on clinical outcomes. PPM is common (20-70% of aortic valve replacements) and has been shown to be associated with worse haemodynamic function, less regression of left ventricular hypertrophy, more cardiac events, and lower survival. Moreover, as opposed to

BACKGROUND: Rapid deployment aortic valves, such as the Edwards Intuity valve, shorten implantation times and facilitate less-invasive approaches. The features that enable the valve's rapid deployment pose a technical challenge if explant is required, as in the instance of endocarditis or structural valve deterioration (SVD), because the prosthesis often becomes densely invested in surrounding cardiac structures. Although international reports describe Intuity and Perceval explants, none have been reported from the United States with annular enlargement.

Prosthetic ring annuloplasty is considered the gold standard technique for mitral valve repair, but it has been associated with some drawbacks. Suture annuloplasty is less expensive and may have some physiopathologic advantages. We reviewed the literature to assess clinical results of mitral suture annuloplasty. Thirteen series, each reporting more than 50 patients and published in the last 10 years, were included in the analysis. They comprised 1,648 patients with cumulative follow-up of 5,607 patient-years. Our review suggests that suture annuloplasty is a safe procedure, but a trend toward

The metallic clicking sound created by mechanical heart valve prostheses frequently bothers patients. To test whether sounds generated by di … laints caused by prosthetic clicking and thus sound emission should be considered when a mechanical heart valve prosthesis is selected.

Technical advances and progress in resuscitation over the last ten years have led to an appreciable improvement in the surgical results of valve replacements. However, it appears that the decision to operate is sometimes difficult in cases of advanced valvular disease. A study of the literature and our own experience lead us believe that despite a significant peri-operative mortality, surgical treatment remains the best form of treatment for these types of valvular disease, whether aortic, mitral or multivalvular. Surgery ensures a longer survival of better quality than does medical treatment.

A 76-year-old male patient was referred to our institution with moderate-to-severe aortic and mitral insufficiency. The patient underwent totally endoscopic robot-assisted aortic valve replacement and mitral valve repair. In this article, we present our lateral approach to the robotic double valve surgery.
