Penetrating trauma is the primary cause of upper extremity vascular injury almost in 95% of cases. Blunt trauma due to traffic or industrial accidents and falls account for the remaining 5% to 10%. However axillary artery injury from blunt trauma to the shoulder is extremely rare. The location of the axillary artery, surrounded by the bones and muscles of the shoulder girdle, explains the low incidence of trauma suffered by this arterial segment. But its anatomical proximity to the humeral head makes it quite vulnerable to blunt trauma during shoulder injury. Herein we report two cases of axil…

We report the case of a patient who experienced post-traumatic static anterosuperior subluxation of the shoulder. The displacement worsened after surgical treatment associating a preglenoid bone block and pectoralis major transfer. We studied the lines of action of the conjoint tendon on an anatomic model. The biomechanical study demonstrated that alone, the coracoid bone block did not provoke the anterior glenohumeral subluxation. On the contrary, the compression components of the humeral head into the glenoid cavity increased and could not explain the worsening dislocation. Inversely, the te…

Two cases of tuberculous arthritis in a joint affected with chondrocalcinosis are reported. No similar cases have been published. Diagnosis was established by demonstration of the tubercle bacillus and calcium pyrophosphate crystals in the joint fluid. Both patients were elderly French females. One patient with involvement of a knee required amputation. The other patient had involvement of a shoulder and developed inferior dislocation of the humeral head and drooping shoulder despite antituberculous therapy. Concomitant occurrence of the two conditions was apparently coincidental but may have …

Traumatic anterior glenohumeral joint dislocation is the most common type of shoulder instability. Lesions that usually result are avulsion of the anterior capsule and glenoid labrum from the glenoid rim (Bankart lesion), compression fracture of the posterosuperior humeral head (Hill-Sachs lesion), and laxity of the joint capsule. Another common lesion is a lengthwise disruption of the rotator cuff at the interval between the subcapularis and supraspinatus tendons. The shoulder that dislocates repeatedly after trauma has an excellent success rate when treated by surgical repair. The aim of the…

BACKGROUND: Rotator cuff muscle degeneration is an important parameter to consider when planning shoulder arthroplasty.
HYPOTHESIS: We hypo … and fatty infiltration. Scapulohumeral subluxation was determined in 3D as the distance between the humeral head center and the glenoid surface center, projected on the same CT slice, and normalized by the humeral head radius. We tested all potential correlations between muscle degeneration and scapulohumeral subluxation.
RESULTS: Muscle degeneration, primari …

BACKGROUND: Treatment of humeral head fractures in the elderly remains a challenge. Within the framework of demographic change the incidence as well as the direct and indirect consequences of injuries will rise. The analysis of an optimal treatment should include functional parameters as well as global health parameters, e.g. quality of life.
OBJECTIVE: The aim of this study was the evaluation of functional outcome, institutionalization (necessity of placement in a residential care home for the elderly, even if temporary), the influence of cognitive deficits, quality of life and the pattern o…

BACKGROUND: Proximal humerus fractures are one of the most common fractures. Intramedullary locked nailing is becoming a popular alternative treatment, especially for easier fracture patterns. Although axillary nerve injury has been reported, no study has compared the safety of the proximal locking options relative to the axillary nerve and the ascending branch of the anterior circumflex artery.
METHOD: Six different commercially available proximal humeral nails were implanted in 30 shoulders of 18 cadavers. After fluoroscopically guided implantation the shoulders were carefully dissected and

There was one delayed union that healed in 4 months. One case of avascular necrosis of the humeral head was observed (a four-part fracture), but remained asymptomatic and did not require further treatment. OBJECTIVE: The authors' purpose was to report their experience with the use of locked nails in treating humeral surgical neck nonunions. Locked nailing has the advantages of minimal soft tissue trauma, automatic fracture reduction, and stable fracture fixation.
METHODS: Excluding 3 patients with severe medical illness, a total of 15 consecutive patients with 15 nonunions were treated by humeral locked nailing and bone grafting. Delay from trauma to surgery averaged 13.8 months, and 10 patients had previous operations. The average age of patients was 55.1 years; average follow-up time was 22.4

INTRODUCTION: The anterior compartment of the arm consists of three muscles: the biceps brachii (BB), brachialis, and coracobrachialis muscle. The aim of the present study was to characterize possible variations in the supernumerary heads of the biceps brachii and use these to prepare an accurate classification of the area that could be used for planning surgical procedures in the region. Material and Methods. One hundred (51 left and 49 right, 52 females and 48 males) upper limbs fixed in 10% formalin solution were examined.
RESULTS: Four types of supernumerary BB heads were identified, with

End-stage osteoarthritis (OA) alters bone density in the humeral head, complicating implant fixation during stemless shoulder arthroplasty. Current preoperative assessments fail to consider the mechanical properties of bone directly supporting the humeral component. While in-scan phantom calibration is used to determine volumetric bone mineral density (vBMD), phantoms are rarely used clinically. An internal density calibration method has been developed, but not yet applied in the proximal humerus. This study examined correlations between phantom and internal density calibration, and between vB…

OBJECTIVES: To assess fracture union rates using novel intramedullary plate technique. Post-operative humeral neck-shaft angles, patient range of motion and complication profile were secondary outcomes of interest.
METHODS: Single surgeon, retrospective case series of surgical technique at major trauma centre in Scotland. A 2.0mm mini-fragment plate was secured to the humeral head to act as an intramedullary strut during fixation in complex proximal humerus fractures. Fracture union and neck-shaft angle were assessed radiologically, whilst range of motion and complication profiles were assess…

Treatment of unstable, displaced, and comminuted fractures of the proximal humerus remains challenging, and optimal treatment continues to b … infection, failure of the plate, and persistent stiffness. One patient had screw perforation of the humeral head articular surface, and 1 had subacromial impingement. Of the 32 patients who had been in active employment before the injury, 28 returned to their previous occupation.

Background and purpose - Previous reports on elbow injuries with concomitant comminute radial head fracture are difficult to interpret, since they include an array of different soft-tissue and bony injuries around the elbow. We focused on Mason-IV fracture dislocations of the elbow and retrospectively reviewed 2 treatment options: radial head resection or replacement with a radial head arthroplasty, both in combination with lateral ligament repair. Patients and methods - In Linköping, 18 consecutive patients with Mason-IV fracture dislocation and with a median age of 56 (19-79) years were trea
