Predicting the need of tracheostomy amongst patients admitted to an intensive care unit: a multivariate model.

BACKGROUND: Patients requiring prolonged invasive mechanical ventilation are prone to complications, such as infections, tracheal stenosis a … Pa/Fi ratio of less than 200 (p=0.046), and the presence of chronic pulmonary disease (p=0.035) or hypernatremia (p=0.012) on intubation day were significantly associated with the requirement of prolonged intubation. DISCUSSION: Invasive mechanical ventilation is a common reason for admittance to the ICU. The abovementioned predictors can be of assistance whe …

Blood purification in fulminant hepatic failure.

Fulminant hepatic failure (FHF) can be described as a potentially fatal condition presenting with hepatic encephalopathy (HE) and coagulopat … wing HFCHDF. We also revealed that HFCHDF was useful for preventing the side effects of PE, such as hypernatremia, metabolic alkalosis, and sharp decrease in colloid osmotic pressure. It is concluded that HFCHDF is useful in the treatment of HE and for preventing the side effects of PE. Therefore, we suggested that HFCHDF + SPE should be standardized for the tr …

Management issues in toxicology.

The lack of evidence-based information in toxicology results in debate and differing recommendations on management issues. Gastric lavage is … cts on clinical outcomes have been demonstrated and potential complications include hypokalemia and hypernatremia. Blood alkalinization for cyclic antidepressant toxicity has become standard therapy. Alkalinization is most effective in treating significant cardiac toxicity. Peritoneal dialysis as therapy for electrolyte and acid base disorders.

Peritoneal dialysis (PD) does not demand special equipment and its fluid composition can be easily changed according to the individual condi … from 0.99 to 2.6. Hyposaline and normo-osmol solution (Na+ 70 mEq/L) were used for one patient with hypernatremia and consciousness disturbance. Serum (S)-Na decreased from 170 to 138 mEq/L, and consciousness became almost clear with the use of 15 L of PD fluid. Hypersaline solution (Na+ 190 mEq/L) was used for two patients with hyponatremia (S-Na 113 and 121 m …

[Etiology of water and electrolyte metabolism imbalance following the rupture of cerebral aneurysms--with special reference to preoperative condition].

One hundred fourteen patients with ruptured cerebral aneurysms were reviewed in regard to the incidence and etiological factors of preoperat … (SAH) and the development of hyponatremia was 13.5 days (range 6 to 26 days). No patients developed hypernatremia (more than 155 mEq/l). Preoperative diabetes insipidus (DI) occurred in 7 (6.1%) of 114 patients. The mean interval between the last SAH and the onset of DI was 26.5 days (range 15 to 35 days). When compared with the onset of hyponatremia following …

Neonatal hypernatraemic dehydration: clinical spectrum, risk factors and immediate outcomes at a tertiary care center, Karachi, Pakistan.

OBJECTIVE: To determine the prevalence, clinical manifestations, risk factors and acute consequences of neonatal hypernatremic dehydration in late preterm and term newborns with or without dehydration. METHODS: The retrospective, cross-sectional study was conducted from December 2020 to May 2022 at Aga Khan University Hospital, Karachi, and comprised data of neonates aged 1-28 days with diagnosis of neonatal hypernatraemic dehydration admitted to the neonatal intensive care unit and step-down unit between June 10, 2015, and June 9, 2020. Demographic and clinical data along with risk factors,

Adverse outcomes and mortality in individuals with eating disorder-related electrolyte abnormalities in Ontario, Canada: a population-based cohort study.

BACKGROUND: Individuals with eating disorders are at a higher risk of electrolyte abnormalities than the general population. We conducted the first representative cohort study assessing whether electrolyte abnormalities in people with eating disorders were associated with mortality and physical health outcomes. METHODS: This was a retrospective population-based cohort study in Ontario including people aged 13 years or older with an eating disorder and an outpatient electrolyte measure within 1 year (between Jan 1, 2008 and June 30, 2019). An electrolyte abnormality was any of hypokalaemia, hy

The epidemiology of hypernatraemia in hospitalised children in Lothian: a 10-year study showing differences between dehydration, osmoregulatory dysfunction and salt poisoning.

INTRODUCTION: The relative frequencies of the causes of hypernatraemia in children after the neonatal period are unknown. Salt poisoning and osmoregulatory dysfunction are extremely rare and potentially fatal. In this retrospective 10-year study, the incidence, causes and differential biochemistry of hypernatraemia in children is examined. METHODS: Children with hypernatraemia (sodium ≥ 150 mmol/litre) aged >2 weeks to 17 years were identified from laboratory data of two paediatric departments serving the Lothian region of Scotland. A review of patient notes established time of onset and caus

[Neonatal hypernatraemic dehydration--why increasing incidence?].

INTRODUCTION: Neonatal dehydration with hypernatraemia is a serious condition with risk of cerebral damage and death. Recent studies have reported a rising incidence. MATERIALS AND METHODS: A retrospective study was conducted at Hvidovre Hospital over a 5-year period to identify term or near-term infants (>35 weeks of gestation) who were admitted with a weight-loss >10%. Infants admitted between the 3rd and the 14th day of life with a discharge diagnosis code indicating dehydration were also included. RESULTS: During the period a total of 89 infants were admitted and 24 had hypernatraemia. T

Late results of radical excision of craniopharyngiomas in children.

The results of radical surgical excision of craniopharyngiomas in children operated on by Dr. Donald Matson beginning in 1950 are presented. The patients are analyzed in regard to survival and quality of survival. While 22 of 34 children so treated at the initial operation are presently alive and tumor-free, high mortality and morbidity followed in cases where reoperation was performed. Properly treated endocrinological deficits need not be a serious problem, but persistent hyperosmolality carried a grave prognosis. No predictive criteria are yet available to determine which tumors are amenabl

Neurosarcoidosis-associated central diabetes insipidus masked by adrenal insufficiency.

Central diabetes insipidus (CDI) is an infrequent complication of neurosarcoidosis (NS). Its presentation may be masked by adrenal insufficiency (AI) and uncovered by subsequent steroid replacement. A 45-year-old woman with a history of NS presented 2 weeks after abrupt cessation of prednisone with nausea, vomiting, decreased oral intake and confusion. She was diagnosed with secondary AI and intravenous hydrocortisone was promptly begun. Over the next few days, however, the patient developed severe thirst and polyuria exceeding 6 L of urine per day, accompanied by hypernatraemia and hypo-osmol

Inhibition of K+ secretion in the distal nephron in nephrotic syndrome: possible role of albuminuria.

Nephrotic syndrome features massive proteinuria and retention of sodium which promotes ascite formation. In the puromycin aminonucleoside-induced rat model of nephrotic syndrome, sodium retention originates from the collecting duct where it generates a driving force for potassium secretion. However, there is no evidence for urinary potassium loss or hypokalaemia in the nephrotic syndrome. We therefore investigated the mechanism preventing urinary potassium loss in the nephrotic rats and, for comparison, in hypovolaemic rats, another model displaying increased sodium reabsorption in collecting
