[Urodigestive fistulae: the diagnosis and treatment of 76 cases].

OBJECTIVES: Urodigestive fistulas are secondary complications of inflammatory, traumatic or neoplastic disease and can occasionally be difficult to diagnose, although the typical symptoms of pneumaturia and fecaluria should make the urologist and surgeon suspect the presence of a fistula, determine its etiology and location. The present study reviews our experience in the diagnosis and treatment of this pathology. METHODS: We reviewed the records of 76 patients with urodigestive fistula that had been diagnosed and treated over a period of 23 years. The form of presentation, usefulness of the

Demonstration of rectourinary fistulas by pressure gastrografin enema.

Rectourethral and vesical fistulas in patients with high anorectal atresia can be demonstrated by using a pressure Gastrografin enema through the distal colostomy opening. A technique is described where contrast medium is forced by gentle pressure, filling the fistula and the urethra. This picture is of great value during the repair of these anomalies.

Clinical outcomes of colo-anal pull-through procedure for complex rectal conditions.

AIM: Pull-through with colo-anal sleeve anastomosis may be used as a last resort to avoid permanent diversion in patients with complex rectal conditions. This procedure allows the preservation of intestinal continuity by minimizing the hazards of deep dissection in an inflamed and fibrosed pelvis. METHOD: A retrospective study was performed of colo-anal pull-through procedures carried out between January 1998 and December 2012 at St Mark's Hospital by one surgeon. Patients were identified from operative logbooks and their case notes were reviewed. Thirty-four patients (21 women) with a median

Operative injuries during vaginal hysterectomy.

OBJECTIVE: To evaluate incidence, characteristics and consequences of urinary and intestinal tract injuries during vaginal hysterectomy for benign conditions. STUDY DESIGN: From January 1970 to December 1996, 3076 vaginal hysterectomies with or without additional procedures, were performed for benign conditions in our department. We retrospectively analyzed operative injury cases. RESULTS: Incidence of urinary and intestinal tract injuries were 1.7 and 0.5%, respectively. Concerning urinary tract injuries, we observed only one ureteral lesion, all others being bladder lacerations (54 cases).

Management of perianal Crohn's disease. Results of a multicenter study in Italy.

PURPOSE: Management of perianal Crohn's disease is still controversial, and reports on large series are very few in the literature. The aim of this multicenter study was to investigate the outcome of both medical and surgical treatment in 225 patients. METHODS: Patients cared for at different institutions were followed up for a median of six years. Most of them had either anal fistula or an abscess (86 percent and 43 percent, respectively), but fissures were also present in 26 percent of the cases. Diarrhea and anal pain were the most common symptoms. Anal lesions preceded the onset of intest
