Small bowel obstruction in a pediatric patient following liver transplant often results from adhesions, hernias, or post-transplant lymphoproliferative disease. Here, we present an unusual and previously unreported entity - Roux-en-Y intussusception in an eight-yr-old female several years after liver transplantation. Although a rare complication, Roux-en-Y intussusception should be considered as a potential etiology in the patient presenting with bowel obstruction, with specific attention to acute presentation accompanying jaundice.

Self-expandable metal Stents (SEMS), first introduced in the late 90s for the palliation of malignant obstructive jaundice and unresectable esophageal malignancy, are now being variably placed for high risk surgical patients in the setting of malignant gastric outlet and colon obstruction. This review attempts to place SEMS into perspective with respect to surgery and other palliative modalities when treating a variety of GI malignancies and attempts to define the role, if any, of SEMS placement in patients with refractory benign GI stenoses.

Two cases of Mirizzi's syndrome are reported. The syndrome consists of: external compression of the common bile duct due to a benign lesion (Type I)--presence of a cholecystobiliary fistula with impacted stones and partial or total obstruction of the hepatic duct (Type II). Clinical signs are non-specific and suggest at first sight an obstructive jaundice. Ultrasonography shows dilatation of the upper biliary tract with narrowing of the biliary tract below the dilatation. ERCP often proves mandatory for diagnosis. Therapeutical procedures are reported.

Major biliary tract obstruction caused by tumour invasion is a rare manifestation of hepatocellular carcinoma. The authors had the opportunity to diagnose and treat five such cases, three of whom had features of acute cholangitis. The prevalence of both hepatocellular carcinoma and recurrent pyogenic cholangitis is high in patients from the Far East. The former may first present under the guise of the latter. Gastroenterologists and surgeons should be aware of hepatocellular carcinoma when managing these patients who present with obstructive jaundice, gross hepatomegaly and cholangitis.

Adenocarcinoma of the duodenum is an uncommon malignancy that usually presents itself with either obstructive symptoms or jaundice, depending on its location. The diagnosis should be suspected in any patient with a high small bowel obstruction, weight loss, or chronic abdominal pain. Endoscopy and barium contrast x-ray films are the preferred initial studies; enteroclysis may provide additional information. Surgical resection offers the only hope for long-term survival, which should approach 50% with resectable tumors.

The authors report the first known case of annular pancreas with coexistent ampullary carcinoma. Obstructive jaundice occurring in an 80-year-old woman was caused by the neoplasm rather than the congenital anomaly. Although the annular pancreas did not obstruct the duodenum, it did account for an unusual appearance of the neoplasm on air contrast roentgenography of the upper gastrointestinal tract. The two lesions were successfully treated by radical pancreaticoduodenectomy. The woman has been disease-free for more than 24 months.

Characterization of pediatric annular pancreas is provided by this analysis of 24 cases (22 neonates, 1 infant, 1 child). Salient observations include: (1) Presentation is affected by the degree of duodenal obstruction at birth and by coexistent anomalies. (2) Polyhydramnios usually accompanies complete high intestinal obstruction by annular pancreas. (3) Primary biliary interruption was not encountered and jaundice was not unusually prevalent. (4) There is a high incidence of associated anomalies. (5) Duodenal bypass by duodenoenterostomy was employed with excellent results.

Esophageal adenocarcinoma (EAC) is an aggressive malignancy with a poor prognosis, especially in advanced stages. Common metastatic sites include the liver, lungs, adrenal glands, lymph nodes, and bones; however, metastasis to the biliary tree is exceedingly rare. We report the case of a 50-year-old male with a history of testicular cancer in remission and metastatic EAC involving the liver, lungs, adrenal glands, peritoneum, bones, and lymph nodes. The patient presented with acute onset jaundice but no abdominal pain, fever, or gastrointestinal symptoms. Laboratory findings revealed significa

A 47-year-old man with a history of ESMC resection of the left chest wall seven years ago was admitted to our hospital due to mid-upper abdominal pain and jaundice for more than 10 days. Laboratory tests showed elevated direct bilirubin, alanine aminotransferase, gamma-glutamyltranspeptidase, and alkaline phosphatase. Computed tomography (CT) of the abdomen revealed soft tissue mass in the head and body of the pancreas with irregularly shaped calcifications, and an enhanced scan showed heterogeneous enhancement. Combined with the patient's past medical history, the possibility of pancreatic me

PURPOSE: Early diagnosis of biliary atresia (BA) is critical for best outcomes, but is challenged by overlapping clinical manifestations with other causes of obstructive jaundice in neonates. We evaluate the performance of the modified Simple BA Scoring System (SBASS) in diagnosing BA.
METHODS: We performed a prospective, cross-sectional study on infants with cholestatic jaundice (June 2021-December 2022). Modified SBASS scoring was applied and compared to the eventual diagnosis (as per intraoperative cholangiogram (IOC) and liver histopathology). The score (0-6), consists of gall bladder len

A 77-year-old man developed a fever up to 38.4 degrees C, with diarrhoea, acute renal failure (creatinine up to 8.7 mg/dl; urea up to 308 mg/dl) and marked jaundice (total bilirubin up to 24.3 mg/dl). In addition there was thrombocytopenia, conjunctivitis and epistaxis, as well as cerebral symptoms with somnolence and general slowing up. At first he was thought to have cholangitis resulting from previously diagnosed gall-stones, and he was therefore treated with ampicillin, 2 g two times daily, and metronidazole, 0.5 g two times daily. The fewer regressed, but the renal failure required haemod

Of 451 patients with cholelithiasis, 273 suffered from 396 preoperative complications; the common ones were acute cholecystitis (n = 120), jaundice (n = 135), and acute pancreatitis (n = 52). Of the 195 patients under the age of 50 years 48% had complications, compared with 70% of the 256 above the age of 50 years. For acute cholecystitis and acute pancreatitis, an interval cholecystectomy was carried out 3-6 wk after initial conservative treatment. Except in the 33 cases with obstructive jaundice of indeterminate etiology (n = 15) or jaundice associated with uncontrollable cholangitis (n = 18

We present the case of a 67-year-old female who developed simultaneous metastases to the ampulla of Vater and the breast. Her medical history is significant for a radical nephrectomy performed twenty-one years prior for renal cell carcinoma (RCC). The patient was referred for evaluation due to the development of progressive jaundice, fatigue, and weight loss. A computed tomography (CT) scan of the abdomen revealed a 40 mm nodule located at the ampulla of Vater, accompanied by dilatation of the bile duct, indicative of obstructive jaundice. Additionally, ultrasonography identified a mass approx

Primary hydatid cyst of the pancreas represents a rare clinical entity. The clinical presentation depends on the location of a hydatid cyst in the pancreas. A hydatid cyst located in the head of the pancreas usually manifests as obstructive jaundice due to the extrinsic compression of the common bile duct. A case of a hydatid cyst of the head of the pancreas in a 4-year-old female is reported. She presented with abdominal pain and jaundice. Contrast-enhanced computed tomography scan of the abdomen revealed a cystic mass at the lower end of the common bile duct, which mimicked a choledochal cys

We report a child who developed acute febrile cholestasis with jaundice and pruritus as the inaugural manifestation of Kawasaki's disease (KD). The severe obstructive icterus and hydrops of the gallbladder required cholecystectomy that was not followed by remission of the fever and cholestasis. KD was suspected after the exclusion of all infectious, metabolic and neoplastic conditions responsible for acute cholestasis. The administration of intravenous gammaglobulin (IVGG) promptly induced defervescence and improvement of the patient's general condition. Mucocutaneous alterations, peeling of t

Jaundice caused by intraluminal bile duct obstruction in infancy is rare but may occur in association with biliary sludge, inspissated bile plugs, or gall stones. Nine boys (aged 2 weeks-6 months) with obstruction caused by inspissated bile (n = 7) or gall stones (n = 2) are presented. Haemolysis was not a factor in the patients' histories but an abnormal entry of the common bile duct into the third part of the duodenum was demonstrated in two and one had an asymptomatic haemangioma. Ultrasonography was the most useful investigation. Surgical removal of the bile duct obstruction was necessary

Hereditary spherocytosis (HS), a common inherited hemolytic anemia, is characterized by red blood cell membrane protein defects leading to chronic hemolysis. This condition significantly predisposes patients to gallstone disease, including both gallbladder and bile duct stones, due to excessive bilirubin production from hemolysis. Gallstones in HS patients, primarily composed of bilirubin, can lead to complications such as cholecystitis, cholangitis, and obstructive jaundice. This review provides a comprehensive landscape of the pathophysiological mechanisms linking HS to gallstone formation,
