Conservative management following patellar dislocation: a level I systematic review.

BACKGROUND: Patellar instability is a common and disabling clinical condition. Treatment of acute primary patellar dislocation aims to reduce the risk of recurrence or painful subluxation and improve function. However, the actual clinical efficacy of any management modality following an acute dislocation has never been demonstrated in prospective or retrospective studies, and the optimal way in which the various management modalities should be used is at best unclear. METHODS: A search was conducted in PubMed, Bireme and Embase databases. Inclusion criteria followed the acronym PICOS, (P) sub

Traumatic Obturator Dislocation of the Hip Joint Associated with Greater Trochanter Fracture: A Case Report.

BACKGROUND: Traumatic obturator dislocation of the hip joint associated with greater trochanter fracture is a rare injury. We used the lateral approach through the rectus abdominis to remove the femoral head dislocated into the obturator, and the posterolateral approach was used for reduction and internal fixation of the femoral greater trochanteric fracture and total hip replacement (THR). Good follow-up results were achieved. To the best of our knowledge, this is the first report on this particular type of injury and on this approach to treating this type of injury. CASE REPORT: The patient

Early functional rehabilitation after patellar dislocation-What procedures are daily routine in orthopedic surgery?

INTRODUCTION: Patellar dislocation and rupture of the medial patellofemoral ligament (MPFL) are frequently seen in daily orthopedic practice. Besides initial non-surgical treatment, surgery and subsequent rehabilitation are crucial for restoring stability in the femoropatellar joint. This study investigated current rehabilitation strategies after patellar dislocation because knowledge on this topic has been severely limited so far. MATERIALS AND METHODS: The current rehabilitation protocols of 42 orthopedic and trauma surgical institutions were analyzed regarding their recommendations on weig

Significance of Diffusion Tensor Imaging of Vastus Medialis Oblique in Recurrent Patellar Dislocation.

BACKGROUND: Numerous studies have investigated the influence of osseous factors on patellofemoral joint instability, but research on the influence of dynamic muscle factors in vivo is still in the exploratory stage. This study aimed to use magnetic resonance imaging (MRI) and diffusion tensor imaging (DTI) to evaluate vastus medialis oblique (VMO) fiber bundles in patients with recurrent patellar dislocation to explore the changes in muscle morphology and function. METHODS: This prospective study involved 30 patients (7 males and 23 females; average age, 21.4 ± 3.8 years) clinically diagnosed

Obturator dislocation of the femoral head combined with ipsilateral femoral neck and pubic fractures: A rare case report.

RATIONALE: Obturator dislocation of the femoral head combined with ipsilateral femoral neck and pubic fracture is a rare injury. We used cannulated screws combined with a femoral neck medial plate for the first time to treat this type of injury and achieved good follow-up results. PATIENT CONCERNS: The patient was hospitalized due to an accident resulting in swelling and deformity accompanied by limited mobility of the right hip and left knee. DIAGNOSES: X-ray examination and computed tomography confirmed that the patient suffered from right hip obturator dislocation, femoral head and neck f

Three-dimensional motion analysis of clinical stress tests for anterior knee subluxations.

The three rotations and three translations that comprise total knee motion were simultaneously measured in cadaveric knees during the commonly employed clinical tests for anterior cruciate injury. A second study determined the three-dimensional motions that occurred when known forces and moments were applied. A total of eight whole lower limbs were studied. A 6 degree-of-freedom instrumented linkage (3-D electrogoniometer), rigidly mounted to the tibia and femur, was used. The ligaments sectioned included the lateral extraarticular restraints (iliotibial band, lateral capsule) and the anterior

Degenerative joint disease in the mouse knee; histological observations.

The knee joints from males of two strains (CBA/ORT and STR/ORT) were studied histologically. The incidence of degenerative joint disease was very high in the STR/ORT strain. Degeneration of the cartilage invariably occurred first at the interface of the cruciate ligament and articular cartilage of the tibia. Lesions were only seen on the medial tibial and later the medial femoral condyles. Blocks of fibrillated, uncalcified cartilage were gradually lost across the condyle, leaving the tidemark as a secondary articulating surface. Meanwhile the subchondral bone thickened and erosion continued t

Total femoral arthroplasty for salvage of end-stage prosthetic disease.

The complications and outcomes in patients who had total femoral arthroplasty for salvage of a severely compromised femur were studied. The clinical scenarios included numerous revision total hip or knee arthroplasties, failed periprosthetic fractures, or recurrent infection treated with multiple radical debridement surgeries. Fifty-nine patients (average age, 73.7 years) were identified. At an average 4.8 years followup, adequate pain relief was achieved and Harris hip pain scores averaged 33.8 of 44 points, and knee pain scores averaged 42.8 of 50 points. Good function was achieved with 98%

Separating Larsen syndrome from the "arthrogryposis basket".

Many patients with larsen syndrome have been misdiagnosed as having arthrogryposis. Larsen syndrome should be considered in any child with multiple joint dislocations, particularly when the knees are involved. Spinal malalignment of a serious degree was encountered in five of the six patients we report and appears to have been underemphasized previously. Two of the patients had unusual ossicles of uncertain derivation at the elbow. This has been described only five times before.

A roadmap for developing a program to self-manage post-traumatic knee osteoarthritis risk.

Youth and young adults who experience a traumatic knee injury (i.e., ligament, meniscus, or cartilage injury; intra-articular fracture or dislocation) have a 6-fold elevated risk of early-onset post-traumatic knee osteoarthritis (PTOA), contributing to joint pain and disability for most of their adult lives. This reality represents a daunting personal and societal burden. Currently, there is no proven clinical program that supports individuals at-risk of PTOA self-manage their knee health and reduce their risk, despite evidence that interventions supporting exercise, physical activity, and hea

Patellofemoral Arthroplasty for Irreducible Chronic Bilateral Patella Dislocation-A Case Report.

INTRODUCTION: Patellofemoral joint arthroplasty (PFJA) is mainly used to treat older patients suffering from osteoarthritis. However, this case report sheds light on the implications of this treatment for young patients suffering from patellar maltracking and secondary patellofemoral osteoarthritis (PFOA). CASE REPORT: A female patient in her late 3rd decade of life presented with long-standing bilateral knee pain and significant functional impairment, including difficulty walking and stair climbing. She also expressed concern as a young female about her walking esthetics. Medical records ind

Braces and Splints for Common Musculoskeletal Conditions.

Braces and splints can immobilize and protect joints, reduce pain, decrease swelling, and facilitate healing of acute injuries. They are also used for injury prevention and chronic pain reduction, and to alter the function of a joint. The medial unloading (valgus) knee brace is an option for patients with medial knee osteoarthritis, but evidence of long-term benefit is limited. The patellar stabilizing brace helps maintain proper patellar alignment but has mixed results in treating patellofemoral pain syndrome. The patellar tendon strap is effective in treating pain from patellar tendinopathy.

Osteochondritis dissecans-Does platelet rich plasma really help.

Osteochondritis dissecans is a common disorder of knee and can be treated by various methods, depending on age of patient and stability of chondral fragment. In Osteochondritis dissecans ICRS type III lesion i.e. articular cartilage discontinuity but no dislocation, variable rate of union as well as high rate of non-union was observed in previous studies when treated with arthroscopic or open reduction and fixation. In previous study it has been also shown that platelet rich plasma help in fracture healing. In this study we are trying to extend the benefit of platelet rich plasma to Osteochond

Prediction of Subsequent Contralateral Patellar Dislocation after First-Time Dislocation Based on Patellofemoral Morphologies.

The subsequent dislocation of a contralateral patellofemoral joint sometimes occurs after a first-time lateral patellar dislocation (LPD). However, the anatomic risk factors for subsequent contralateral LPD remain elusive. This study included 17 patients with contralateral LPD and 34 unilateral patellar dislocators. The anatomic parameters of the contralateral patellofemoral joints were measured using CT images and radiographs that were obtained at the time of the first dislocation. The Wilcoxon rank-sum test was performed, and a binary regression model was established to identify the risk fac

Motor Vehicle Accident during Pregnancy with Two Lifes at Risk: A Case Report.

INTRODUCTION: Subtalar dislocations are rare injuries, representing around 1% of all dislocations. Defined as talonavicular and talocalcaneal separation, occurs in medial, lateral, anterior, or posterior direction. They are more common in young men, usually as a result of high energy trauma and are associated with posttraumatic arthritis, especially when accompanied with peritalar fracture (a cuboid fracture in this case), so prompt recognition is necessary in order all these complications to be avoided. CASE PRESENTATION: This study presents a rare case of subtalar dislocation associated wit

Conservative versus tailored surgical treatment in patients with first time lateral patella dislocation: a randomized-controlled trial.

BACKGROUND: Patellar instability has a high incidence and occurs particularly in young and female patients. If the patella dislocates for the first time, treatment is usually conservative. However, this cautious approach carries the risk of recurrence and of secondary pathologies such as osteochondral fractures. Moreover, there is also risk of continuous symptoms apparent, as recurrent patella dislocation is related to patellofemoral osteoarthritis as well. An initial surgical treatment could possibly avoid these consequences of recurrent patella dislocation. METHODS: A prospective, randomize

The outcomes of the modified Fulkerson osteotomy procedure to treat habitual patellar dislocation associated with high-grade trochlear dysplasia.

BACKGROUND: Habitual patellar dislocation is not common in clinical practice, but it has a deep impact on the patient's lifestyle and movement. There has been no large case-control study on habitual patellar dislocation, and the management of it is still controversial. The aim of this study was to observe the efficacy of the modified Fulkerson procedure on patients with habitual patellar dislocation with high-grade trochlear dysplasia without trochleoplasty and to evaluate the results of this procedure. METHODS: A total of 25 patients who were admitted to our hospital from April 2007 to Octob
