In 20 patients with hemorrhagic hereditary teleangiectasias (HHT) only 15 were treatable with argon laser. In the others septal perforation or continued bleeding made initial treatment inadequate. The laser treatment had to be repeated in all patients, due to multiplicity of lesions. Nine patients regarded the outcome very favourably with minimal bleeding episodes. In four of them the effect lasted more than one year.

The 1.06 micron Nd:YAG laser and a new fiberoptic delivery system, the Interstitial Thermo-Therapy (ITT) laser fibre, allows stereotactic interstitial irradiation of cerebral tumours. In experimental rat brain studies we found typical laser-tissue effects with a central necrosis and a sharply demarcated oedema towards the normal brain. The carbon dioxide laser, when used surgically, is a possible source of complications. Knowledge of laser physics, tissue effects, and target tissue anatomy are of the utmost importance for minimizing complications. This article seeks to elucidate the factors involved in laser beam physics and the effect of these factors on target tissue, based on specific tissue anatomy. Common tissue complications and specific procedure-oriented complications that may arise as a result of laser surgery will be discussed and reviewed.

With the purpose of improving the results of treatment of chronic prostatitis the authors has reflexotherapy (corporal and auricular). It is shown that the used method has advantages over the existing traditional methods. A multiple-criteria mathematical analysis was carried out of the efficacy of laser reflexotherapy in chronic prostatitis and is was found to equal 89%.

From analysis of the epulis patient population for 40 years, it could be established that of 1019 lesions, 12.8% relapsed, in 40% of surgical interventions, with the extraction of the tooth involved. In the palliative treatment of malignant esophageal stenosis endoscopic laser therapy and intracavitary radiation currently represent the best alternative. The results achieved with elimination of dysphagia, duration of the complaint-free interval, survival period, and complications are presented from a group of 167 patients. Referring to the quality of life, an evolving tendency towards the use of laser therapy instead of endoscopic intubation is reported.

A new way to treat glaucoma by laser coagulation of the ciliary body was developed. The laser beam is channeled to the eye through the fiberoptic system. In a three-year study, the scalpel Nd:YAG laser was compared with conventional surgery in 18 patients who underwent oculoplastic surgery. The postoperative follow-up was at least eight months. A case of localized amyloidosis of the subglottis is described. Urgent endotracheal intubation and later tracheostomy were required for upper airway obstruction. The amyloid was completely resected endoscopically using the CO2 laser. There is no evidence of recurrence two-and-a-half years after surgery.

Sixteen pregnant women with genital condylomata acuminata were treated with laser vaporization to assess effectiveness, complication rate, and impact on recurrence. Patients were treated at a gestational age of 13-35 weeks. No recurrences occurred before delivery. Five patients experienced uterine contractions before 37 weeks. Only one delivery before 37 weeks may have been attributed to the laser therapy. Four recurrences were detected during postpartum follow-up. Thirteen patients (9 women, 4 men) with anemia from acute and chronic gastrointestinal bleeding were found to have antral vascular disease consistent with watermelon stomach. The median age was 73 yr, with a range of 54-88 yr. Eight of the patients were transfusion-dependent, requiring a median of 5.5 U within the 12 mo before treatment. Thirty-eight patients had laparoscopic treatment of endometriosis by the use of the Nd:YAG laser, combined with a fiber-optic cable equipped with sapphire contact tips. In this preliminary investigative study patients in all four revised American Fertility Society classifications were treated for both gynecologic disability and infertility. The authors describe 21 cases with resection of the retinal rupture valve with Nd:YAG laser impulse, performed to reduce the vitreoretinal tractions. Resection of the 'dump' rupture valve has been carried out in 10 cases, in the rest 11 the operation has been performed in the presence of a detachment of the retina. Anatomic success (detachment of the valve from the retina) has been achieved in 16 cases. The technique of the surgery is described, as are indications and contraindications to the operation.
