Left Ventricular Apex Chimney Technique for Central Extracorporeal Life Support.

In cardiogenic shock with severely compromised left ventricle (LV) function, systemic perfusion and LV unloading are crucial to reduce pulmonary afterload and thrombus risk. Peripheral extracorporeal life support (ECLS) is commonly used, but central ECLS with LV venting is often required when the peripheral support is insufficient. We propose the "LV apex chimney graft technique," a novel and straightforward method for direct LV cannulation via median sternotomy or left anterior thoracotomy. This approach allows for secure cannula fixation and effective LV venting in the central ECLS, minimizi

Perinatal wellbeing through restoring healthy family systems in a nêhiyaw (Plains Cree) community: a mixed methods study.

INTRODUCTION: A collaborative research partnership in Alberta (Canada) was formed with the goal of supporting optimal health during the perinatal period for diverse Indigenous populations. Our approach utilized community-led strategies that promote the best possible healthy life for families and children. This partnership was guided by a Restoring and Supporting Healthy Family Systems (RSHFS) conceptual model based primarily on Indigenous ways of knowing. We sought to explore components of the RSHFS model with members of a nêhiyaw (Plains Cree) community of childbearing age, focused on refinin

Infection in childhood and neurological diseases in adult life.

Other chapters in this issue discuss the evidence that implicates infection during infancy and childhood in the etiology of respiratory disease. Here I argue that experience of infection in early life may also be involved in the aetiology of some diseases of the adult nervous system. The descriptive epidemiology of three neurological diseases is compatible with the hypothesis that they are delayed consequences of childhood infection. It is not difficult to imagine that the effects of an infection which results in loss of cells from an organ system, like the central nervous system, whose cell p

Association of myositis and myocarditis in progressive systemic sclerosis.

A retrospective review of 47 patients with progressive systemic sclerosis revealed a highly significant (P less than 0.001) association between myositis and myocarditis. Three patients developed clinically overt myocarditis as an early feature of their disease. These patients had an inflammatory myopathy indistinguishable from polymyositis. Despite an excellent clinical response to steroids, the late occurrence of life-threatening conduction system defects appeared while they were receiving therapy. Consequently, patients with progressive systemic sclerosis and myositis should be evaluated for

Upgrading removable denture design by using strategic implants: A case report.

Inserting a dental implant in a strategic position under a removable partial denture (RPD) can upgrade the RPD design by changing a mainly tissue-supported RPD to an implant-tissue-supported RPD or an implant-tooth-tissue-supported RPD with better retention, support, and stability. CASE PRESENTATION: The patient (female, 77-year-old) was unsatisfied with her maxillary and mandibular clasp-retained removable dentures (RDs). Medical, social, and systemic health evaluations revealed that the patient was healthy. Dental history, intraoral and extraoral examinations, functional screening, and esthe

Quality of end-of-life care in multiple myeloma: A 13-year analysis of a population-based cohort in Ontario, Canada.

Optimizing end-of-life (EOL) care for multiple myeloma (MM) represents an unmet need. An administrative cohort in Ontario, Canada was analysed between 2006 and 2018. Aggressive care was defined as two or more emergency-department visits in the last 30 days before death, or at least two new hospitalizations within 30 days of death, or an intensive care unit (ICU) admission within the last 30 days of life. Supportive care was defined as a physician house-call in the last two weeks before death, or a palliative nursing or personal support visit at home in the last 30 days before death. Among 5095

Effects of Prematurity and Growth Restriction on Adult Blood Pressure and Kidney Volume.

Gaining insight into the complex cycle of renal programming and its early-life clinical associations is essential to understand the origins of kidney disease. Prematurity and intrauterine growth restriction are associated with low nephron endowment. This increases the risk of developing hypertension and chronic kidney disease later in life. There is appreciable evidence to support mechanistic links between low nephron endowment secondary to intrauterine events and kidney size, kidney function, and blood pressure in postnatal life. A clear understanding of the cycle of developmental programming

Evidence-based approaches to other symptoms in advanced cancer.

Dyspnea, nausea and vomiting, anorexia, fatigue, and sleep disturbances are common and distressing in advanced cancer. We updated previous systematic reviews of how these symptoms can be alleviated with targeted literature searches. The approach to these symptoms requires comprehensive symptom assessment; treating underlying causes when benefits exceed risks; prioritizing treatment, as patients usually have many symptoms; and addressing psychosocial and spiritual distress. For dyspnea, evidence supports systemic opioids and nonpharmacological treatments such as a fan. The strongest evidence su

Nursing interventions for foot health and self-management in frail older adults with diabetes: A narrative review.

Diabetes and frailty are two interrelated health conditions that mutually exacerbate one another and have a detrimental impact on quality of life in older adults. Frailty, characterised by depleted physiological reserves, cognitive decline and impaired self-care capacity, complicates diabetes management. Consequently, this population faces an elevated risk of serious foot-related complications, including diabetic foot ulcer development and delayed wound healing. This narrative review evaluates the pathophysiology, epidemiology and nursing interventions for foot-health issues in frail older adu

Cardiac Rehabilitation in Women: A Scientific Statement From the American Heart Association.

Cardiac rehabilitation (CR) is a proven intervention to improve cardiovascular health, offering benefits such as reduced hospital readmission rates, lower mortality rates, and enhanced quality of life. Poor CR participation, which is a universal problem, is particularly pronounced in women. Considerable sex and gender disparities exist in CR access, engagement, and outcomes. Despite the effectiveness of CR, women are underrepresented in CR programs, and face barriers related to lower referral rates, medical comorbidities, societal roles, and financial constraints. These challenges are further

An Early-Stage Economic Evaluation of Superabsorbent Wound Dressings for the Management of Moderately to Highly Exuding Leg Ulcers in Slovakian Settings.

BACKGROUND: The management of chronic leg ulcers, including venous leg ulcers (VLUs), causes a considerable economic and clinical burden to healthcare systems. Factors such as nursing time, hospital care, and wound dressings account for approximately 85% of the total cost. Superabsorbent dressings (eg, superabsorbent polymers [SAPs]) are recommended as a first-line treatment for moderately to highly exuding VLUs. OBJECTIVE: To assess the cost-effectiveness of utilizing SAPs compared with the standard of care (SoC) for managing patients with moderately to highly exuding VLUs within the Slovaki

Effects of Opioids on Immune and Endocrine Function in Patients with Cancer Pain.

Opioids are an important treatment in managing cancer pain. Uncontrolled pain can be detrimental to function and quality of life. Common adverse effects of opioids such as sedation, constipation and nausea are well recognised, but opioid effects on the endocrine and immune systems are less apparent. The evidence for the immunomodulatory effects of opioids suggest that some opioids might be immunosuppressive and that their use might be associated with reduced survival and increased rates of infection in patients with cancer. However, the quality of this evidence is limited. Opioid-induced endoc

Hydrolysis of Phosphate Esters Catalyzed by Inorganic Iron Oxide Nanoparticles Acting as Biocatalysts.

Phosphorus ester hydrolysis is one of the key chemical processes in biological systems, including signaling, free-energy transaction, protein synthesis, and maintaining the integrity of genetic material. Hydrolysis of this otherwise kinetically stable phosphoester and/or phosphoanhydride bond is induced by enzymes such as purple acid phosphatase. Here, I report that, as in previously reported aged inorganic iron ion solutions, the iron oxide nanoparticles in the solution, which are trapped in a dialysis membrane tube filled with the various iron oxides, significantly promote the hydrolysis of
