Design, synthesis and structure-activity relationships of novel 15-membered macrolides: Quinolone/quinoline-containing sidechains tethered to the C-6 position of azithromycin acylides.

In the search for novel hybrid molecules by fusing two biologically active scaffolds into one heteromeric chemotype, we found that hybrids of azithromycin and ciprofloxacin/gatifloxacin 26j and 26l can inhibit the supercoiling activity of E. coli gyrase by poisoning it in a way similar to fluoroquinolones. This may modestly contribute to their potencies, which are equal to ciprofloxacin against constitutively resistant Staphylococcus aureus, whose growth is not inhibited by the presence of macrolides. In contrast, introduction of quinolines (the 3-quinoline 26b and the 6-quinoline 26o) with an…

Effect of Sub-MICs of Macrolides on the Sensitivity of Pseudomonas aeruginosa to Nitrosative Stress: Effectiveness against P. aeruginosa with and without Multidrug Resistance.

Sub-MICs of the 14-membered macrolides erythromycin (EM) and clarithromycin (CAM) decreased the growth of Pseudomonas aeruginosa PAO1 and increased its sensitivity to endogenous and exogenous nitrosative stress. However, a 16-membered macrolide, josamycin (JM), was not or less effective. In 9 of 13 non-multidrug-resistant P. aeruginosa (non-MDRP) and 9 of 27 MDRP ST235 strains, the sub-MIC of EM induced significant reductions in bacterial numbers following treatment with a nitric oxide donor.

Direct anti-biofilm effects of macrolides on Acinetobacter baumannii: comprehensive and comparative demonstration by a simple assay using microtiter plate combined with peg-lid.

Recently, opportunistic nosocomial infections caused by Acinetobacter baumannii have become increasingly prevalent worldwide. The pathogen often establishes biofilms that adhere to medical devices, causing chronic infections refractory to antimicrobial therapy. Clinical reports have indicated that some macrolide antibiotics are effective against chronic biofilm-related infections. In this study, we examined the direct anti-biofilm effects of seven macrolides (azithromycin, clarithromycin, erythromycin, josamycin, spiramycin, fidaxomicin, and ivermectin) on A. baumannii using a simple and newly…

Effects of quorum sensing-interfering agents, including macrolides and furanone C-30, and an efflux pump inhibitor on nitrosative stress sensitivity in Pseudomonas aeruginosa.

Long-term administration of certain macrolides is efficacious in patients with persistent pulmonary Pseudomonas aeruginosa infection, despite how limited the clinically achievable concentrations are, being far below their MICs. An increase in the sub-MIC of macrolide exposure-dependent sensitivity to nitrosative stress is a typical characteristic of P. aeruginosa. However, a few P. aeruginosa clinical isolates do not respond to sub-MIC of macrolide treatment. Therefore, we examined the effects of sub-MIC of erythromycin (EM) on the sensitivity to nitrosative stress together with an efflux pump…

Structure elucidation and biosynthetic gene cluster analysis of caniferolides A-D, new bioactive 36-membered macrolides from the marine-derived Streptomyces caniferus CA-271066.

Bioassay-guided isolation based on the antifungal activity of a culture broth of the marine-derived actinomycete Streptomyces caniferus CA-271066 led to the discovery of new 36-membered polyol macrolides, caniferolides A-D (1-4). Their connectivity was determined by spectroscopic methods including ESITOF-MS and 1D/2D NMR. The relative stereochemistry of each stereocluster in these compounds was established using NOE analysis, the universal database method and J-based configuration analysis, further assisted by comparisons with NMR data of structurally related macrolides. Genome sequencing foll…

Rapid Recognition of Matrine Enantiomers Using Ion Mobility-Mass Spectrometry and Theoretical Modeling with Macrolide as Chiral Selectors.

The precise analysis of chirality is crucial for elucidating their stereoselective effects. Ion mobility-mass spectrometry (IM-MS) is a promising tool for chirality analysis, yet it is constrained by limited resolving power and the correlation between collision cross-section (CCS) values and accurate structures. In this work, a rapid method for recognizing matrine (MAT) and its enantiomer sophoridine (SPD) was established using IM-MS and theoretical modeling. With Natamycin (Nat) and Amphotericin B (Amp) as chiral selectors, binary noncovalent complexes were formed with target analytes to enha

Assessment of macrolide resistance in the respiratory tract of preterm-born infants during treatment with azithromycin in the AZTEC clinical trial.

BACKGROUND: Our multicentre, double-blind, randomised, placebo-controlled Azithromycin therapy for prevention of chronic lung disease of prematurity trial assessed if early 10-day azithromycin treatment improved survival without development of chronic lung disease of prematurity when compared with placebo in 796 preterm-born infants. Since antibiotic resistance remains a significant global health priority, we had preplanned macrolide-resistance assessment in recruited infants. We, therefore, identified baseline macrolide-resistant genes in respiratory samples and determined if this was altered

Doxycycline for Macrolide-Resistant Mycoplasma pneumoniae Pneumonia in Children: Clinical Updates and Therapeutic Insights Post-COVID-19.

Mycoplasma pneumoniae (MP) is a leading cause of community-acquired pneumonia (CAP) in children. Macrolides have long been first-line therapy due to favorable safety profiles and low minimum inhibitory concentrations (MICs) in pediatric populations. However, the global surge in macrolide-resistant MP (MRMP) has compromised conventional treatments, creating an urgent need for alternative agents. Mounting evidence supports doxycycline, a second-generation tetracycline, as an effective therapy for pediatric MRMP, particularly post-COVID-19. Compared to azithromycin, doxycycline shortens disease d…

Genomic and molecular dynamics analysis of mefA-encoded macrolide efflux protein from Tn2009 and Tn2010 in Streptococcus pneumoniae.

Antibiotic resistance in Streptococcus pneumoniae remains a major clinical challenge, particularly for macrolides such as erythromycin. This resistance is commonly associated with transposons Tn2009 and Tn2010 carrying the mefA gene, which encodes a macrolide efflux protein. This study investigated the genetic characteristics of mefA from Indonesian clinical isolates and elucidated its efflux mechanism using integrated genomic analysis and molecular dynamics simulations. Whole-genome sequencing confirmed the presence of Tn2009 and Tn2010 in clinical isolates. Multiple sequence alignment showed…

Multiplex amplicon sequencing reveals polyclonality and low macrolide resistance in the French Mycoplasma pneumoniae outbreak, 2023-2024.

OBJECTIVES: Unprecedented global increases in Mycoplasma pneumoniae respiratory infections began in late summer 2023. Although a predominance of p1 subtype 1 associated with alarmingly high rates of macrolide resistance was reported in Southeast Asia, data from Europe remain limited. We aimed to characterise the circulating M. pneumoniae strains in French hospitalised patients, using a high-throughput amplicon sequencing approach. METHODS: Phylogenetic analysis was performed using p1 adhesin gene subtyping and multi-locus sequence typing. Mutations associated with antibiotic resistance were s

Genomic Surveillance Reveals Emergence and Spread of Macrolide-Resistant Mycoplasma pneumoniae in Australia During the 2023-2024 Epidemic.

BACKGROUND: The resurgence of Mycoplasma pneumoniae, first reported in China in 2023, was attributed to waning postpandemic immunity with increases in macrolide-resistant M. pneumoniae (MRMP) (> 80%). In Australia, infections peaked in early 2024, particularly among children under 15 years. While MRMP remains low in Europe, North America, and Australia (< 5%), limited routine testing restricts understanding of resistance dynamics. As macrolides are first-line therapy in many settings, MRMP surveillance is essential for guiding empirical treatment. METHODS: We applied a capture-based targeted …

[Severe pulmonary Mycobacterium genavense infection in an immunocompetent adult : Diagnostic challenges and successful macrolide-based combination therapy].

Mycobacterium genavense is a rare cause of non-tuberculous mycobacterial disease, typically occurring in immunocompromised hosts. The case of a 49-year-old immunocompetent man with respiratory symptoms is presented. Imaging revealed a hilar mass with mediastinal lymphadenopathy, initially suggesting malignancy. Molecular diagnostics identified M. genavense as the causative pathogen. A 15-month regimen of rifampicin, azithromycin, and ethambutol led to complete clinical and radiological remission.
