[Analysis of national maternal death surveillance: 1989-1991].

An analysis of maternal mortality in 247 cities, districts and countries of 30 provinces, municipalities and autonomous regionscovering a population of 100,000,000 in China in 1989-1991 was reported. The total number of live births was 4,201,457 in the pilot area, and the number of maternal deaths was 3,274 giving an average maternal mortality rate (MMR) of 80.0/100,000. After correction, the average MMR was 87.8/100,000. The MMR per annum was in decreasing trends and in different regions were different from one another. The first 6 leading causes of maternal deaths in 3,274 cases, in decreasi…

The "grand multipara"--is it a problem? A review of 5785 cases.

The Grand Multipara (GM) has almost disappeared in the Western countries due to the advancement of family planning. Having a heterogeneous population, the problem of Grand Multiparity still exists in our country. This study is based on 5785 cases of GM which were treated in our Obstetrical Department during a period of 16 years (1960-1975). We have compared this group to the general obstetrical population in terms of pregnancy and delivery complications. Face and breech presentations as well as transverse lie were twice, brow presentations were three times as frequent in the GM group. Postpart

A verbal and social autopsy study to improve estimates of mortality determinants in Pakistan: study design and methods.

BACKGROUND: Reducing newborn, child, adolescent, and maternal mortality is a global priority, yet Pakistan continues to bear a high burden of preventable deaths. Routine civil registration does not capture causes or social determinants for most of these deaths. The verbal and social autopsy (VASA) methodology fills this gap by combining symptom-based identification of the cause of death with structured assessment of social and care-seeking factors. Here we describe the methodological framework of a national VASA study conducted using the Pakistan National Nutrition Survey 2018 sampling frame, …

Why do maternal and newborn deaths continue to occur?

According to estimates in 2015, there were 303,000 maternal deaths, 2.7 million newborn deaths and 2.6 million stillbirths. A wide range of factors, from health system dynamics to social determinants of health and underlying health conditions, contribute to this outcome. The highest mortality risk for mothers and their babies is on the day of birth, and most of these deaths are preventable. The largest burden of deaths occurs in low-income countries, particularly in sub-Saharan Africa and South Asia, due to their young population and high fertility. Substantial reductions in maternal and newbo

Causes of mortality in an African city.

Registered deaths from the vital statistics registration system of Lagos City, a system that was judged to be 60% complete, were analysed for the year 1977. Nearly 40% of total registered deaths were from infections, parasitic diseases and motor vehicle accidents. Of the reported deaths, 17.3% were from ill-defined conditions. Deaths from neoplasms, diseases of the nervous and sense organs, diseases of the digestive and genitourinary systems as well as those from congenital anomalies are relatively less frequent. Maternal mortality appears to be very high. The age pattern of mortality is diffe…

Pregnant for Nine Months, Postpartum for Life: Long-term Implications of Pregnancy Related Conditions.

Maternal mortality in the state of Missouri remains among the highest in the nation. Recognizing that the burden of making corrective actions does not rest solely with obstetric providers, it is time to consider how pregnancy affects our patients before, during, and after childbirth. Reviewing pregnancy history and pregnancy intention can be a chance to address modifiable risk factors to improve patient outcomes when providing medical care.

Burden and determinants of puerperal sepsis in Ethiopia: a systematic review and meta-analysis.

BACKGROUND: Puerperal sepsis is a leading cause of maternal morbidity and is responsible for nearly one-fifth of maternal deaths worldwide. In Ethiopia, it remains a significant contributor to maternal mortality; however, a comprehensive understanding of its determinants is still lacking. Therefore, the aim of this systematic review and meta-analysis is to estimate the pooled prevalence of puerperal sepsis and to identify its determinant factors in Ethiopia, thereby providing comprehensive and up-to-date evidence to inform maternal health interventions. METHODS: The review was conducted in ac…

Infant Mortality Rates Among US-Born and Foreign-Born Latinx Women: The Effect of Black Race.

OBJECTIVE: To determine the extent to which Black race is associated with the infant mortality rate (< 365 day, IMR) of births to US-born and foreign-born Latinx women. METHODS: Stratified and multivariable binominal log-linear regression analyses were performed on the 2010-2013 National Center for Health Statistics linked birth-death certificate files of singleton infants. RESULTS: The IMR of births to US-born Black Latinx women (N = 54,542) exceeded that of births to US-born White Latinx women (N = 1,320,084): 5.7/1000 vs 4.2/1000, RR = 1.4 (1.2, 1.5). In contrast, the IMR of births to for

Feasibility of implementing a mobile phone-based telemonitoring programme to support pregnant women at high-risk for pre-eclampsia in Karachi, Pakistan: protocol for a mixed-methods study.

INTRODUCTION: High maternal mortality from pre-eclampsia/eclampsia results from lack of early identification and management of pregnant women at high-risk for pre-eclampsia. A potential tool to support pregnant women at high-risk for pre-eclampsia is telemonitoring. There is limited evidence on the use and effectiveness of telemonitoring for pregnant women in low-income and middle-income countries (LMICs) which limits the understanding of the process and mechanisms through which the intervention works in LMICs. This study will explore the feasibility of implementing a mobile phone-based telemo…

Pulmonary Embolism and Amniotic Fluid Embolism.

Venous thromboembolism (VTE) as well as other embolic events including amniotic fluid embolism (AFE) remain a leading cause of maternal death in the United States and worldwide. The pregnant patient is at a higher risk of developing VTE including pulmonary embolism. In contrast, AFE is a rare, but catastrophic event that remains incompletely understood. Here the authors review the cause of VTE in pregnancy and look at contemporary and evidence-based practices for the evaluation, diagnosis, and management in pregnancy. Then the cause and diagnostic difficulty of AFE as well as what is known reg

Postpartum hemorrhage in maternal mothers at Anutapura Public Hospital in Palu, 2017.

OBJECTIVE: Postpartum hemorrhage is one of the factors causing maternal mortality in Indonesia. In Indonesia, at least 128.000 women suffer from hemorrhage, causing them to death. Data from the Anutapura Public Hospital in Palu showed that the rate of postpartum hemorrhage within 2015-2017 was fluctuated and still caused maternal mortality. This study aimed to determine the risk factors of the incidence of postpartum hemorrhage in maternal mothers at Anutapura Public Hospital, Palu. METHOD: The type of study was observational with a case-control approach. The subject of the study was mothers …

Perceptions of Postpartum Teaching and Knowledge of Warning Signs among Black Mothers.

The postpartum period is the time of highest risk for maternal death. Quality postpartum education is important to minimize risk. We assessed the postpartum education given to Black mothers on post-birth warning signs by conducting a cross-sectional survey of 80 Black women who had given birth in the previous 12 months. Participants completed an electronic questionnaire, and data were analyzed using descriptive and regression models. On average, educational needs were reported by 20 participants as being unmet, 27 as being met, and 30 as being more than met. Only 54.4% of the participants repo

Anaesthesia for ruptured ectopic pregnancy at district level.

In South Africa, deaths as a result of ectopic pregnancies are increasing despite the overall improvements in maternal mortality. These deaths occur predominantly in district hospitals, with the final cause of death being hypovolaemic shock in almost all cases. In most cases, no anaesthesia was attempted despite the district hospitals having the clinical skills, equipment and infrastructure to provide a caesarean delivery service. It appears that there is a skills gap between the provision of anaesthesia for caesarean delivery and that of ruptured ectopic pregnancy. There is a growing recognit…

Optimizing task-sharing in abortion care in Ghana: Stakeholder perspectives.

Ghana has made progress in expanding providers in abortion care but access to the service is still a challenge. We explored stakeholder pers … to practice task-sharing in abortion care because unsafe abortion was contributing significantly to maternal mortality. They noted that the Ghana Health Service utilized the high maternal mortality in the country at the time, advancements in medicine, and the lack of clarity in the definition of the term "health practitioner" to work with partner nongovernmental org …

The Epidemiology of Abortion And Its Prevention in Chile.

Mortality by abortion has continuously decreased over the past fifty years in Chile. In fact, maternal death as a result of an induced abortion has become an exceptionally rare phenomenon in epidemiological terms (a risk of 1 in 4 million pregnant women of fertile age or 0.4 per 100,000 life births for abortion of any type, excluding ectopic pregnancy). After abortion became illegal in 1989, deaths related to abortion continued to decrease from 10.8 to 0.39 per 100,000 live births. This scientific fact challenges the common notion that less permissive abortion laws lead to greater mortality as
