ECMO in Pregnancy: Analysis of Indications, Management and Outcomes.

Extracorporeal membrane oxygenation (ECMO) has become a critical rescue therapy for pregnant and peripartum patients experiencing life-threatening cardiac or respiratory failure that is refractory to standard treatments. Pregnancy induces unique physiologic changes that influence ECMO initiation, cannulation, and management, necessitating specialized approaches to optimize outcomes for both mother and fetus. Pharmacologic challenges include altered drug kinetics and the risks associated with placental drug transfer, while anticoagulation strategies require a careful balance between thrombosis

Trauma in pregnancy.

The anatomic and physiologic changes make treatment of the pregnant trauma patient complex. The fetus is the challenge, because, in pregnancy, trauma has little effect on maternal morbidity and mortality. Aggressive resuscitation of the mother, in general, is the best management for the fetus, because fetal outcome is directly related to maternal outcome. Recent literature has attempted, with little success, to identify factors that may predict poor fetal outcomes. Cardiotocographic monitoring should be initiated as soon as possible in the emergency department to evaluate fetal well-being. Oth

Clinically recognized limb ischemia in the neonate after umbilical artery catheterization.

During a 4 year period, 1,461 critically ill neonates were admitted to the newborn intensive care unit of the Newark Beth Israel Medical Center, and 507 (35 percent) had umbilical artery catheters inserted for physiologic monitoring. In five patients (1 percent) clinically significant limb ischemia developed as a result of catheter complications. Frank gangrene was observed in three patients; two died from the primary illness soon after the onset of gangrene and the third survived after leg amputation. The other two infants had advanced ischemia that responded favorably to catheter removal and

Artificial intelligence mobile health platform for early detection of COVID-19 in quarantine subjects using a wearable biosensor: protocol for a randomised controlled trial.

INTRODUCTION: There is an outbreak of COVID-19 worldwide. As there is no effective therapy or vaccine yet, rigorous implementation of traditional public health measures such as isolation and quarantine remains the most effective tool to control the outbreak. When an asymptomatic individual with COVID-19 exposure is being quarantined, it is necessary to perform temperature and symptom surveillance. As such surveillance is intermittent in nature and highly dependent on self-discipline, it has limited effectiveness. Advances in biosensor technologies made it possible to continuously monitor physi

Diagnosis and treatment of PCOS in adolescents: a summary of guidelines and systematic reviews for practitioners.

Polycystic Ovary Syndrome (PCOS) presents unique diagnostic and management challenges in adolescents due to physiologic overlap with normal puberty. This review synthesizes current adolescent-specific guidelines to identify evidence-based diagnostic criteria for adolescent PCOS, to apply first- and second-line management strategies, and to recognize key areas of controversy and unmet need. We emphasize menstrual irregularities and hyperandrogenism as key diagnostic criteria, while noting the limited specificity of polycystic ovarian morphology and anti-Müllerian hormone (AMH) levels in this ag

From Progressive Asthma to Intensive Care Unit Respiratory Failure: Approaches Nursing Approach to Patient Care.

Asthma is a systemic inflammatory disease characterized by complex and reversible airflow obstruction resulting from various bronchial reactions triggered by a combination of genetic and environmental variables. Allergens, smoking, pollen and air pollution are all common causes, and asthma exacerbations are one of the more common reasons for intensive care unit (ICU) admission. Nurses have a significant effect on the care of patients with asthma via the use of particular teaching approaches and health promotion templates focused on the principles of the Ottawa Charter for Health Promotion. Moving toward the ideal insulin for insulin pumps.

Advances in insulin formulations have been important for diabetes management and achieving optimal glycemic control. Rapid-acting insulin analogs provide a faster time-action profile than regular insulin and are approved for use in pumps. However, the need remains for therapy to deliver a more physiologic insulin profile. New insulin formulations and delivery methods are in development, with the aim of accelerating insulin absorption to accomplish ultra-fast-acting insulin time-action profiles. Furthermore, the integration of continuous glucose monitoring with insulin pump therapy enables on-g

Anesthesia for the acute abdomen patient.

Patients with acute abdomen often have marked physiologic and pathologic changes, making anesthesia both challenging and potentially hazardous for the patient. A thorough understanding of the pathophysiologic mechanisms of cardiovascular function under anesthesia and selection of appropriate anesthetic protocols are critical to a successful anesthetic outcome. The goal is to produce anesthesia while minimizing depression of the cardiovascular system. Monitoring and management of acid-base and cardiovascular function serve to ensure appropriate oxygen delivery to the tissues during anesthesia.

Splints and scar management for acute and reconstructive burn care.

Because patients assume a position of comfort during acute burn management, affected joints or regions must be splinted in positions of function to avoid contractures. Particularly with the increasing trend towards ambulatory burn care, close monitoring by the burn team and patient education is required to achieve the best functional result. Despite adequate initial care, contractures sometimes occur, requiring a shift in splinting tactics to correct the deformity. If secondary reconstruction is required, the affected region is once again maintained in the position of function. Although the ph

The use of low-viscosity fluids for hysteroscopy.

We reviewed the literature addressing the use and complications associated with low-viscosity distention fluids in urologic and hysteroscopic procedures. The possible causes of fluid complications included elevated intrauterine pressure, myometrial invasion, prolonged operating time, and operative trauma to the cervix or uterine body. Physiologic electrolyte solutions create only fluid overload, and treatment requires cardiovascular support and diuresis. Nonelectrolytic solutions result in acute dilutional hyponatremia, which has been referred to as the transurethral resection of the prostate

Anesthetic management of the pregnant patient with Marfan syndrome.

Two patients with Marfan syndrome who presented for labor and delivery are reviewed. The characteristics of Marfan syndrome and their significance in relation to the physiologic changes due to pregnancy and the implications for anesthetic management are discussed. Marfan syndrome can present with musculoskeletal, cardiovascular, ocular, and pulmonary complications believed to be secondary to an inheritable disorder of connective tissue metabolism. These patients must be carefully evaluated, monitored, and managed during the prepartum and peripartum period. The physiology of pregnancy, particul

Multimodal neonatal procedural and postoperative pain assessment dataset.

This paper presents the first multimodal neonatal pain dataset that contains visual, vocal, and physiological responses following clinically required procedural and postoperative painful procedures. It was collected from 58 neonates (27-41 gestational age) during their hospitalization in the neonatal intensive care unit. The visual and vocal data were recorded using an inexpensive RGB camera while the physiological responses (vital signs and cortical activity) were recorded using portable bedside monitors. The recorded behavioral and physiological responses were scored by expert nurses using t

Key physiological indicators and technological trends in physiology-directed cardiopulmonary resuscitation: A narrative review.

Cardiac arrest remains a critical public health challenge with consistently low survival rates worldwide, underscoring the urgent need to optimize the quality of cardiopulmonary resuscitation. Physiology-directed cardiopulmonary resuscitation, through real-time monitoring of parameters such as end-tidal carbon dioxide and diastolic blood pressure, enables clinicians to objectively assess cardiopulmonary resuscitation quality and make adjustments in real time, advancing towards personalized resuscitation therapy. This review investigates the relationships between key physiological indicators -

Symposium review: Precision technologies for dairy calves and management applications.

There is an increasing interest in using precision dairy technologies (PDT) to monitor real-time animal behavior and physiology in livestock systems around the world. Although PDT in adult cattle is extensively reviewed, PDT use for the management of preweaned dairy calves has not been reviewed. We systematically reviewed research on the use and application of precision technologies in calves. Accelerometers have the potential to be used to monitor lying behavior, step activity, and rumination, which are useful to detect changes in behavior that may be indicative of disease, responses to painf

Impella SmartAssist Technology: Practical Foundations and Clinical Management.

Temporary mechanical circulatory support device technology has undergone significant evolution with the introduction of the SmartAssist platform, enabling enhanced monitoring of patient-pump interactions. Continuous-flow pump dynamics and patient hemodynamics are closely interconnected, and pump performance depends on physiologic (eg, afterload, preload) and anatomic (eg, pump position, ventricle size) parameters. The SmartAssist platform readily provides an array of data, including measured and derived variables, to support clinical decision-making; however, these new features are often not w

Altered Mental Status and Delirium in Pediatric Patients.

Mental status is the collection of an individual's consciousness, perception, emotion, memory, and cognition at a particular point in time, which is inferred by the clinician through careful observation and interaction. The pediatric mental status assessment must be approached with an understanding of cognitive, language, and psychosocial development. Alterations must then be comprehensively and clearly described. Delirium is a phenotypic diagnosis with a specific set of criteria in the DSM and is a serious neurocognitive disorder caused by physiologic changes due to illness, injury, toxins, m

Use of Visual Electrophysiology to Monitor Retinal and Optic Nerve Toxicity.

It is important for clinicians to consider exposure to toxic substances and nutritional deficiencies when diagnosing and managing cases of vision loss. In these cases, physiologic damage can alter the function of key components of the visual pathway before morphologic changes can be detected by traditional imaging methods. Electrophysiologic tests can aid in the early detection of such functional changes to visual pathway components, including the retina or optic nerve. This review provides an overview of various electrophysiologic techniques, including multifocal electroretinogram (mfERG), fu

High-throughput phenotyping reveals expansive genetic and structural underpinnings of immune variation.

By developing a high-density murine immunophenotyping platform compatible with high-throughput genetic screening, we have established profound contributions of genetics and structure to immune variation (http://www.immunophenotype.org). Specifically, high-throughput phenotyping of 530 unique mouse gene knockouts identified 140 monogenic 'hits', of which most had no previous immunologic association. Furthermore, hits were collectively enriched in genes for which humans show poor tolerance to loss of function. The immunophenotyping platform also exposed dense correlation networks linking immune

Anesthesia for in utero repair of myelomeningocele.

Recently published results suggest that prenatal repair of fetal myelomeningocele is a potentially preferable alternative when compared to postnatal repair. In this article, the pathology of myelomeningocele, unique physiologic considerations, perioperative anesthetic management, and ethical considerations of open fetal surgery for prenatal myelomeningocele repair are discussed. Open fetal surgeries have many unique anesthetic issues such as inducing profound uterine relaxation, vigilance for maternal or fetal blood loss, fetal monitoring, and possible fetal resuscitation. Postoperative manage

Helping patients quit smoking: lessons learned in the trenches.

Cigarette smoking is a genuine and complex addiction that involves three interconnecting links: the central link, nicotine (physiologic) addiction, is joined to two additional links, psychologic (cognitive and affective) dependence and sociocultural (environmental) factors. It is precisely this dynamic combination of related factors that makes cigarette smoking so incredibly resistant to long-term extinction. Successful management of highly dependent smokers requires that clinicians use a structured, multifaceted, patient-oriented approach that includes the use of nicotine replacement and nico
