Microcystic adnexal carcinoma. Ten cases treated by Mohs micrographic surgery.

BACKGROUND: Microcystic adnexal carcinoma is a slow-growing, nondescript, locally aggressive, deeply infiltrating neoplasm histologically characterized by an infiltrative pattern of basaloid or squamous cells, a desmoplastic stromal reaction, keratin-filled cysts, and glandular structures. OBJECTIVE: Microcystic adnexal carcinoma is uncommon and may be mistaken microscopically for other benign and malignant entities. Perineural or intraneural involvement by tumor cells is characteristic and extension into underlying structures including muscle, fat, and bone are frequently encountered. Althou

Merkel cell carcinoma: squamous and atypical fibroxanthoma-like differentiation in successive local tumor recurrences.

Merkel cell carcinoma (MCC) is a rare, frequently lethal, primary neuroendocrine carcinoma of the skin. Histopathologically, it appears as a dermal nodule of small, undifferentiated malignant cells. Historically, MCC was considered to be an eccrine carcinoma. Recognition of its neuroendocrine features later led to the hypothesis that it arose from Merkel cells in the skin, although recent evidence revisits the question of an epithelial origin. We present a case of MCC arising on the temple of a 78-year-old male, in association with an actinic keratosis. Three years later, a local tumor recurre

Expression of tyrosine hydroxylase (TH) in human tumors: a tissue microarray study evaluating 18,666 tumors from 150 different tumor entities.

TH immunohistochemistry (IHC) has been proposed as a sensitive and specific tool for the distinction of pheochromocytomas or paragangliomas from epithelial neuroendocrine neoplasms and tumors. However, little is known about the prevalence of TH expression in other tumor entities. To comprehensively evaluate TH expression in normal and tumor tissues, a tissue microarray containing 18,666 samples from 150 different tumor types and subtypes and 608 samples of 76 different normal tissue types were analyzed by IHC. TH immunostaining in tumors was always cytoplasmic. It was detectable in 158 (1%) of

Ocular neoplasms: Clinical, epidemiological, and histopathological analysis of mutilating procedures in a tertiary care hospital in Mexico.

BACKGROUND AND OBJECTIVE: Although uncommon, ocular neoplasms represent a significant cause of visual morbidity and, in some cases, mortality. In advanced stages, management may require mutilating procedures such as enucleation or exenteration. Clinical, epidemiological, and histopathological characterization of these cases improves diagnostic suspicion and guides therapeutic decision-making. The objective of this study was to analyze the epidemiological, clinical, and histopathological profile of patients who underwent enucleation or evisceration due to suspected ocular neoplasia at a tertiar

Targeted Molecular Sequencing of Recurrent and Multifocal Non-HPV-associated Squamous Cell Carcinoma of the Vulva.

Recurrent vulvar squamous cell carcinomas (SCCs) are a poorly understood and aggressive group of treatment-resistant neoplasms. Currently, it remains unclear whether these are in fact recurrences of the same primary tumor, or the development of entirely new tumors. Here, to address this question, we examined the mutational profile of a series of patients with recurrent or multifocal non-human papilloma virus (HPV)-associated vulvar SCC. We performed a targeted 33-gene next-generation sequencing panel on a series of 14 patients with recurrent or multifocal non-HPV-associated vulvar SCC and prec

[Submental artery island flaps for reconstruction in the head and neck region].

BACKGROUND: Reconstruction of defects after extirpation of head and neck neoplasms is a highly challenging and complex surgical undertaking. Commonly used techniques, such as the radial forearm flap or anterior lateral thigh flap, have numerous disadvantages, e.g., donor-site morbidity, poor color match for cutaneous reconstruction, and excessive tissue bulk for intraoral reconstruction. Use of a submental artery island flap is a relatively new and increasingly performed option for reconstruction in the head and neck region. MATERIALS AND METHODS: From March 2013 to August 2015, 12 patients w

Immunogenetic contribution of the fractalkine/CX3CR1 axis in oral squamous cell carcinoma.

Oral squamous cell carcinoma (OSCC) is a malignant neoplasm with high mortality and recurrence. Its etiology is multifactorial and involves environmental factors such as smoking, alcohol, and human papilloma virus infection, as well as genetic factors such as single nucleotide polymorphisms. The fractalkine/CX3CR1 axis is key in the regulation of cell apoptosis, proliferation, migration, and invasion, which are fundamental processes in cancer development. The CX3CL1 gene, which encodes fractalkine, presents variants such as rs223815 (G > C) and rs682082 (G > A), associated with resistance to c

Clinicopathological Features of Epstein-Barr Virus-Positive Neuroendocrine Carcinoma: Analysis of Twenty-Two Cases.

Epstein-Barr Virus (EBV)-positive neuroendocrine carcinoma (NEC) is a rare neoplasm with limited histopathological and therapeutic data. This report presents 22 cases of EBV-positive NEC, analyzing age distribution, morphology, and immunophenotype. The median patient age was 47 years (range: 27-67 years), with a male-to-female ratio of 17:5. Most cases (86%, 19/22) were localized to the nasal cavity or nasopharynx, while the remaining three (14%, 3/22) involved the lung, eyelid, and chest wall. Tumors were identified as small cell neuroendocrine carcinoma (SCNEC) or large cell neuroendocrine c

Consistent LEF-1 and MYB Immunohistochemical Expression in Human Papillomavirus-Related Multiphenotypic Sinonasal Carcinoma: A Potential Diagnostic Pitfall.

Human papillomavirus (HPV)-related multiphenotypic sinonasal carcinoma (HMSC) is a distinct, newly-described sinonasal tract neoplasm characterized by a salivary gland tumor-like appearance with myoepithelial and ductal cells, frequent surface squamous dysplasia, and relatively indolent behavior. When considering a diagnosis of HMSC, aggressive high-grade salivary gland carcinomas, particularly those with a basaloid morphology such as basal cell adenocarcinoma and adenoid cystic carcinoma, enter the differential diagnosis. The full morphologic and immunophenotypic profile of HMSC continues to

[A bilateral epithelial myoepithelial carcinoma of the parotid gland].

We report the case of a 52-year-old man, who was admitted in the department of otorhinolaryngology for a mass of the right parotid gland. The radiological and clinical hypothesis was a squamous cell carcinoma. Histopathological examination revealed a biphasic proliferation composed of epithelial cells arranged in a tubular pattern stained with cytokeratins 5-6 and 7 and EMA surrounded by clear myoepithelial cells stained with smooth muscle actin and p63. Ki-67 labeling index was low. The diagnosis of epithelial myoepithelial carcinoma was proposed. One year after, the patient noticed a centime

'Drawing' a Molecular Portrait of CIN and Cervical Cancer: a Review of Genome-Wide Molecular Profiling Data.

In this review we summarize the results of studies employing high-throughput methods of profiling of HPV-associated cervical intraepithelial neoplasia (CIN) and squamous cell cervical cancers at key intracellular regulatory levels to demonstrate the unique identity of the landscape of molecular changes underlying this oncopathology, and to show how these changes are related to the 'natural history' of cervical cancer progression and the formation of clinically significant properties of tumors. A step-wise character of cervical cancer progression is a morphologically well-described fact and, as

Plasmacytoid dendritic cells in hypertrophic discoid lupus erythematosus: an objective evaluation of their diagnostic value.

BACKGROUND: Hypertrophic discoid lupus erythematosus (HDLE), a rare variant of lupus skin disease, is difficult to distinguish from squamous neoplasms and certain dermatoses microscopically. Recently, recognition of the pathogenetic significance of plasmacytoid dendritic cells (PDCS) in cutaneous lupus erythematosus (LE) and of their patterns of distribution in different manifestations of the disease prompted us to study their diagnostic value in the context of HDLE. METHODS: Using immunohistochemistry (CD123) to label the cells, we examined their quantities and patterns of distribution in 27

Non-small cell lung cancer initially presenting with intracardiac metastasis.

Intracardiac metastasis as the initial presentation of malignant neoplasm is very rare. We report here on a 64-year-old man with non-small cell lung cancer (NSCLC) initially presenting with intracardiac metastasis which was identified with 18-F fluorodeoxyglucose positron emission tomography (FDG PET). The patient was admitted with complaints of exertional dyspnea and vague chest discomfort that had developed a few weeks ago. Two-dimensional echocardiography revealed a heart mass attached to its akinetic wall in the right ventricular chamber. CT and MRI demonstrated a large tumor involving the

A Rare Synchronous Existence of Warthin's Tumour and Oral Cancer: A systematic review.

Oral squamous cell carcinoma (OSCC) is a common malignancy, while Warthin's tumour (WT) is a benign salivary gland neoplasm, typically found in the parotid gland but rarely involving extra-parotid tissues. The synchronous occurrence of OSCC and extra-parotid WT is particularly unusual. Accurate identification of these pathologies is crucial, as their rare presentation can resemble metastasis in lymph nodes. This review systematically searched for case reports and series of synchronous WT and OSCC, including 13 papers having 17 cases. The quality of these cases was assessed using CARE guideline

Benign endoscopic biopsies may be a red herring.

A 64-year-old man presented with haematemesis and melena. Repeated endoscopies showed extensive candidiasis with an exophytic mass like a shelf of tumour. Biopsies showed chronic inflammatory changes with candidiasis without evidence of malignancy. His only complaint was feeling tired and loss of energy. There was no dysphagia but slight retrostenal discomfort on swallowing. Computed tomography scan reported an opacification in the right upper lobe adjacent to the mediastinum. This contained air bronchograms and several irregular air filled cavities. There was significant mediastinal adenopath
