Sleep complaints in periodic paralyses: a web survey.

Neuronal potassium conductance has been shown to influence the sleep-wake cycle and REM sleep homeostasis. The periodic paralyses (PP) are characterized by episodes of muscular weakness associated with changes in serum potassium levels and, therefore, with possible alterations in extracellular neuronal potassium conductance. We submitted a sleep questionnaire to the members of Periodic Paralysis International Listserv, an on-line support and information group for subjects with PP. Three control groups were made up of patients with untreated depression, patients with depression under treatment

Hypokalaemic periodic paralysis associated with controlled thyrotoxicosis.

Familial hypokalaemic periodic paralysis is an autosomal dominant muscle disease which has been linked to point mutations in the skeletal muscle L-type calcium channel alpha 1 subunit (alpha 1 s). It consists of muscular weakness episodes due to hypokalaemia caused by intracellular shifting of potassium. We describe the case of a young man of Kurdish origin, with a history of Graves' disease, who was admitted to the emergency room with hypotonic tetraplegia associated with severe hypokalaemia.

[Periodic fever due to hyper-IgD syndrome].

In a 45-year-old man who from early childhood had been suffering of periodic fever, which did not respond to any therapy attempted, the ultimate diagnosis was hyperimmunoglobulinaemia D syndrome (HIDS). HIDS attacks typically occur every 4-6 weeks and last 3-7 days. The most frequent symptoms are fever, diarrhoea, arthralgias, cold shivers, abdominal pain, vomiting and headache. Physical examination often reveals lymphadenopathy, skin lesions, arthritides, splenomegaly and serositis. Laboratory investigation includes an acute-phase response with granulocytosis and enhanced erythrocyte sediment

Postpartum, also a risk period for domestic violence.

OBJECTIVE: In the first European study of its kind the prevalence of physical and sexual violence postpartum was estimated in a random sample of 207 Swedish born women attending antenatal clinics. STUDY DESIGN: The Severity of Violence Against Women scale (SVAW) was used to measure the frequency of threats and severity of physical and sexual abuse by means of a postal questionnaire covering a period of 8 weeks postpartum. In the sample of women the same research tool had been employed during the preceding pregnancy in the form of a personal interview. RESULTS: The non-response rate was 75/20

The role of the central chemoreceptor in causing periodic breathing.

In a previous publication (Fowler et al., 1993), we reduced the classical cardiorespiratory control model of (Grodins et al., 1967) to a much simpler form, which we then used to study the phenomenon of periodic breathing. In particular, cardiac output was assumed constant, and a single (constant) delay representing arterial blood transport time between lung and brain was included in the model. In this paper we extend this earlier work, both by allowing for the variability in transport delays, due to the dependence of cardiac output on blood gas concentrations, and also by including further del

Etiology of interepidemic periods of mosquito-borne disease.

Dengue viruses and malaria protozoa are of increasing global concern in public health. The diseases caused by these pathogens often show regular seasonal patterns in incidence because of the sensitivity of their mosquito vectors to climate. Between years in endemic areas, however, there can be further significant variation in case numbers for which public health systems are generally unprepared. There is an acute need for reliable predictions of within-year and between-year epidemic events. The prerequisite for developing any system of early warning is a detailed understanding of the factors i

[Hippiatric formularies established during the period of the Revolution].

Two hippiatric formularies established during the period of the Revolution are presented. The first one published in 1793, is a printed pamphlet of 8 pages, containing 68 drugs. The second one dated of january 1794 includes 26 items only, and consists in a unique chart entitled: Nomenclature des Drogues nécessaires au traitement des maladies internes et externes des chevaux malades qui sont aux armées et dans les différents dépôts de la République. This is of particular and new interest when it tentatively synthesizes and standardizes the main pharmaceutical products able of fighting farcy and

Spectral analysis of periodic fluctuations in electrocardiographic repolarization.

Repolarization alternans (RPA) indicates alternate-beat fluctuations in the temporal or spatial characteristics of the echocardiogram (ECG) STU segment which may represent dispersion in repolarization. Spectral decomposition has revealed microvolt-level RPA which has been found to correlate with ventricular tachycardia (VT) and fibrillation, and is increasingly being used for clinical risk stratification. However, while interruptions in periodicity are known to affect spectral decomposition, their quantitative impact on RPA and its clinical utility have been poorly described. We therefore stud…

Duration of periods of euthymia in patients with dysthymic disorder.

OBJECTIVE: The purpose of this study was to assess the duration of periods of euthymia in patients with dysthymia. METHOD: All patients with dysthymia who came to the Center for Anxiety and Depression over a 10-month period (N = 22) were interviewed by the author regarding the duration of their euthymic episodes. RESULTS: The 22 patients with dysthymia reported euthymic periods from 2 to 30 days (mean = 8.0 days, SD = 6.6). CONCLUSIONS: The euthymic period of up to 2 months that is specified in DSM-IV for dysthymic disorder might confound the results of clinical trials. Data from additional

[Typhoid fever in periods of wars and armed conflicts].

Study of military-epidemiological importance of typhoid-paratyphoid infections in military operations of XIX and XX centuries. Despite the research progress, and new means of special prevention, the problem has not been solved yet. In wars water becomes an important factor of typhoid spread in the troops and civil population, so water supply should be closely supervised by the civil authorities and military medics.

[Thyrotoxic hypokalemic periodic paralysis in a male Kurd].

Thyrotoxic hypokalemic periodic paralysis has been described to occur quite frequently in male Asiatic patients. The syndrome is, however, very rare in patients of Caucasian origin. To our knowledge it has never been described in Austria so far. This is the reason why we present the following case: A 22-year old male patient of Kurdish origin suffered from two periods of typical flaccid paralysis of the extremities after strenuous physical exertion, that were 4 months apart. The periods of paralysis were quickly reversed by substitution with potassium. Graves' disease was retrospectively diagn

Sudden periodic paralysis: rare manifestation of thyrotoxicosis.

Nonfamilial hypokalemic thyrotoxic periodic paralysis is rarely diagnosed among Caucasians and blacks in the western world but it is relatively common among Asiatics. Sudden paralysis occurring while at rest after a large carbohydrate meal or strenuous exercise in an undiagnosed mild thyrotoxic patient is a common presentation. A case illustrating such presentation is reported. Intracellular shifts of potassium triggered or facilitated by hyperthyroidism and hyperinsulinemia are the biochemical features. Correction of the thyrotoxic state is the definitive treatment for this disorder. Judiciou

Congenital malformations presenting during the neonatal period.

Discharge of the newborn within 24 to 48 hours after birth makes it more difficult to detect some congenital malformations and increases the need for careful examination and review of the history of the pregnancy, delivery, and nursery course. Progressive physiologic changes after birth, especially in the cardiovascular system, precede the development of signs indicative of disease for certain congenital malformations. Discharge before these changes occur may delay their detection because the newborn is not being monitored by medical or nursing caregivers. The AAP Committee on the Fetus and Ne

[Prognosis of proximal radius fractures in the growth period].

Fractures of the proximal radius in children may lead to deformities of the radial head and functional disturbance of pro- and supination. However, traumatization is not only caused by the injury itself, but may also occur secondary to surgical reduction, manipulation of fracture fragments and excessive physiotherapy. In a prospective long-term follow-study (2-20 years after trauma) of 38 children with displaced proximal radius fractures we found functional disturbances in 11% of children only. These were not caused by radial head deformities, which were present on 83% of follow-up radiographs

Silent period abnormalities in carpal tunnel syndrome.

Cutaneous silent period (CSP) was measured on stimulating digits 2 and 5 in 19 patients with carpal tunnel syndrome (CTS) and compared with 20 healthy volunteers. In 2 patients with severe CTS, CSP was absent on digit 2 but present on digit 5. In mild/moderate CTS digit 2 CSP mean duration was 50.9+/-13.9 ms, significantly longer than control subjects (36.9+/-8.7 ms) (P = 0.005). CSP was found to be abnormal in CTS, being absent in severe and prolonged in mild/moderate cases.
