In patients with tetralogy of Fallot (TOF) repair and a borderline pulmonary valve annulus (PVA) size, surgical repair often necessitates a transannular incision and subsequent placement of a patch with or without a monocusp or, alternatively, a right ventricle-to-pulmonary artery conduit. We discuss here a technique in which the pulmonary valve annulus can be safely preserved, with infrequent postoperative issues as well as the potential for less incidence of right ventricular outflow intervention in the long term.

BACKGROUND: The aim of this study was to identify patient-related and/or disease-related factors that influence outcomes in patients with vascular prosthetic graft infections.

Our report encompasses two cases of Dacron patch infection following carotid endarterectomy and a systematic review of the literature. Particular attention is placed on the incidence, aetiology, investigation, presentation and management dilemmas of this rare complication of carotid surgery. A simple technique utilized when replacement of the ascending aorta has to be associated with replacement of the aortic valve and the sole noncoronary sinus of Valsalva (vs. total root replacement) is presented. It offers the advantage to secure stable hemostasis in an area where surgical exposure may be difficult to achieve off-pump. For its easy reproducibility, this procedure might contribute to a more radical resection of diseased tissue thus limiting risk of further dilatation, rupture, and need for reoperation.

The authors developed a new prosthesis for patent ductus arteriosus (PDA) closure, using a delivery device inserted through the main pulmonary artery (MPA) avoiding ductal dissection and use of cardiopulmonary bypass. The life expectancy of patients with the Marfan syndrome is reduced by complications caused by dilatation of the ascending aorta. Because surgical therapy with a composite graft may alter this natural history, we analyzed the preoperative and long-term postoperative status of 50 consecutive patients who received such a graft. Since 1970, 7 patients varying in age between 1 month and 3 years have had successful correction of an aortopulmonary septal defect. Three of these patients had associated cardiac lesions which were repaired at the same time. Extracorporeal circulation was used in 3 patients and profound hypothermia with limited extracorporeal circulation in 4. In 5 patients the defect was closed with a Dacron patch through a transaortic approach. The advantages of this method are discussed.
