Percutaneous sacroplasty using CT guidance for pain palliation in sacral insufficiency fractures.

BACKGROUND AND OBJECTIVE: Sacroplasty has emerged as a treatment option for patients with painful osteoporotic sacral insufficiency fractures. We report short-term outcomes in a consecutive cohort of patients treated with sacroplasty. METHODS: We retrospectively reviewed 57 patients treated with sacroplasty for painful osteoporotic sacral fractures at our institution between 2004 and 2011. An 11-point numerical rating scale pain score was recorded at rest and at activity pre- and post-procedure. Opioids prescribed to the patient both pre- and post-procedure were recorded. RESULTS: Mean durat

The Effectiveness of Virtual Reality in Managing Acute Pain and Anxiety for Medical Inpatients: Systematic Review.

BACKGROUND: Virtual reality is increasingly being utilized by clinicians to facilitate analgesia and anxiolysis within an inpatient setting. There is however, a lack of a clinically relevant review to guide its use for this purpose. OBJECTIVE: To systematically review the current evidence for the efficacy of virtual reality as an analgesic in the management of acute pain and anxiolysis in an inpatient setting. METHODS: A comprehensive search was conducted up to and including January 2019 on PubMed, Ovid Medline, EMBASE, and Cochrane Database of Systematic reviews according to PRISMA (Preferr

Effectiveness of cold application and lavender oil on pain during drain removal: A randomized clinical trial.

BACKGROUND AND AIM: Analgesics are frequently used to prevent acute pain while removing the drain. Additional non-pharmacological methods have come to the agenda as a result of the fact that the pain cannot be fully controlled, and the pharmacological treatment response is variable. Our research was intended to determine the effectiveness of lavender aromatherapy and cold application in controlling pain during drain removal procedure. MATERIALS AND METHODS: The sample of the prospective randomized controlled study consisted of 121 patients. Patient data were collected using the introductory i

Comparison of the Effect of Balloon Catheter vs Nucleoplasty vs Balloon Catheter and Nucleoplasty in Patients With Lumbar Spinal Stenosis.

BACKGROUND: Nucleoplasty and neuroplasty are often performed in patients with refractory lower back pain when conservative treatment is ineffective. Lumbar spinal stenosis (LSS) is caused by multiple factors; in some cases, a single procedure of nucleoplasty or neuroplasty alone does not provide sufficient treatment effect. OBJECTIVES: This study aimed to investigate and compare the pain relief and pain-free interval among patients with LSS who underwent nucleoplasty, neuroplasty, and combined balloon neuroplasty and nucleoplasty. STUDY DESIGN: Retrospective study. SETTING: In-ha University

Objective and continuous measurement of peripheral motor indicators of pain in hospitalized infants: a feasibility study.

Measurement of pain in pre-verbal infants is complex. Until now, pain behavior has mainly been assessed intermittently using observational tools. Therefore, we determined the feasibility of long-term, objective and continuous measurement of peripheral motor parameters through body-fixed sensors to discriminate between pain and no pain in hospitalized pre-verbal infants. Two pain modes were studied: for procedural pain 10 measurements were performed before, during and after routine heel lances in 9 infants (age range infants: 5-175 days), and for post-operative pain 14 infants (age range 45-400…

Predictive factors of efficacy of periradicular corticosteroid injections for lumbar radiculopathy.

BACKGROUND AND PURPOSE: Steroid periradicular infiltration is a common nonsurgical sciatic pain treatment of inconsistent efficacy. The purpose of this study was to identify factors for predicting the efficacy or failure of this procedure. METHODS: Two hundred twenty-nine patients with lumbar radiculopathy were prospectively followed up at 2 weeks and 1 year after percutaneous periradicular steroid infiltration. The intensity of radicular pain was scored on the visual analog scale (VAS). Pain relief was classified as "excellent" when the pain was completely resolved or had diminished by 75% o

Best Practices for Postoperative Management of Posterior Sacroiliac Joint Fusion.

Sacroiliac joint (SIJ) pain is a common cause of low back pain. Traditionally, treatment for SIJ joint pain and dysfunction has consisted of physical therapy, medication management, SIJ injections, and SIJ ablations. Improved recognition of the SIJ as an etiology for back pain has led to advances in treatment options. Radiofrequency of the lateral sacral branches has been shown to be effective, though evidence is fraught with inconsistent patient selection, study design and procedural technique. It also does not directly address the mechanical dysfunction of the SIJ. In order to create a more

Nonpharmacological Relaxation Technology for Office-Based Rhinologic Procedures.

BACKGROUND: Office-based rhinologic procedures are increasingly performed to control costs and enhance patient convenience. Adequate management of pain and anxiety is essential for the technical performance of these procedures, in addition to ensuring patient comfort. Pharmacologic agents are often used to manage anxiety and pain. Nonpharmacological adjuncts may be useful for achieving these effects without oral opioids and benzodiazepines. METHODS: Charts of patients who underwent office-based rhinologic procedures with the NuCalm system (Solace Lifesciences, Inc., Wilmonton, DE, USA) in com

Percutaneous transpedicular vertebroplasty with calcium phosphate cement in the treatment of osteoporotic vertebral compression and burst fractures.

OBJECT: Osteoporotic vertebral fractures occasionally lead to late-onset collapse, kyphosis, persistent back pain, and disability. The authors describe a series of patients in whom they performed percutaneous vertebroplasty by using calcium phosphate cement (CPC) to obtain early pain relief and improve the integrity of the osteoporotic vertebral body (VB). METHODS: Between August 2000 and February 2001, they performed 17 percutaneous transpedicular CPC-assisted vertebroplasty procedures in 16 patients who harbored thoracic or lumbar osteoporotic vertebral fractures. Following repositioning an

The use of virtual reality hypnosis for prostate cancer patients during transperineal biopsy/gold seed implantation: A needs assessment study.

BACKGROUND: Patients having radiation therapy treatment to the prostate may require invasive preparation procedures under local anesthesia (LA), such as the insertion of gold seeds into the prostate or targeted biopsies. These procedures can induce pain and anxiety for some patients. Virtual Reality Hypnosis (VRH) is the combination of a 360-degree video display with audio and mental guides for relaxation and distraction during medical procedures. The objective of this research was to assess the level of patient interest in the use of VRH during gold seed insertion and biopsy, and to identify

Duodenal Biopsy Gone Bad: Hematoma, Obstruction, Hemoperitoneum, and Everything Else You Don't Want.

INTRODUCTION: We report a case of a 25-year-old man that was admitted to emergency department for mild abdominal pain and vomiting episodes. The first imaging, with abdominal ultrasound, showed a > 10 cm space-occupying lesion (SOL) in epigastrium. The CT-scan diagnosed an intramural duodenal hematoma. AREAS COVERED: Since massive intramural duodenal hematoma (m-IDH) is a rare entity due to trauma, spontaneous causes, post-procedural complications, or underlying medical conditions, therapy can vary. In our case report, conservative treatment was conducted with success. EXPERT COMMENTARY: m-ID

[Osteoarthritis Care; in transition towards a national movement and education program?].

Osteoarthritis of the knee and hip leads to pain, functional limitations, and reduced quality of life. Although hip and knee arthroplasty are effective treatments, they remain costly and carry procedural risks. Evidence from the GLA:D programme shows that structured education, exercise therapy, and lifestyle interventions can delay surgery and improve mental wellbeing. Dutch policy documents emphasise prevention and network-based care, but regional variation and fragmented financing hinder nationwide implementation. A national movement and education programme, embedded within multidisciplinary

Case Report: Occult splenic rupture during left-sided VATS decortication: diagnostic role of early perioperative FAST.

Video-Assisted Thoracic Surgery (VATS) is the preferred method for many thoracic diseases, featuring advantages of minimal trauma, reduced postoperative pain, and low complication rates. However, it carries potential risks, including splenic rupture-a rare but life-threatening complication. A 69-year-old male patient developed hemodynamic instability during VATS for left-sided empyema clearance; intraoperative exploration failed to identify an obvious bleeding source. Early post-procedural FAST bedside focused assessment with sonography for trauma (FAST) confirmed splenic rupture, which was ma

Bilateral Cluster Headache-like Attack as a Manifestation of Reversible Cerebral Vasoconstriction Syndrome Following Carotid Artery Stenting.

A 34-year-old Japanese man developed bilateral periorbital pain and cranial autonomic symptoms that mimicked cluster headache after carotid artery stenting. Imaging revealed hemorrhagic infarction and multifocal vasoconstriction, confirming reversible cerebral vasoconstriction syndrome (RCVS). He had a history of radiotherapy for nasopharyngeal carcinoma, which likely contributed to the delayed vascular injury, including carotid stenosis and vertebral artery dissection. Combined with procedural stress from stenting, these factors may have triggered the RCVS. This case highlights the importance

Bone in emergency room: Not only a skeletal problem! A rare case of accidental chicken bone ingestion in the afferent loop following Billroth II partial gastrectomy.

We report the case of a 58-year-old male with prior Billroth II gastrectomy who presented with worsening abdominal pain. Contrast-enhanced CT revealed a hyperdense, corticated, triangular-shaped foreign body (FB), consistent with a chicken bone, which had undergone retrograde migration and become impacted in the afferent duodenal loop. The FB's orientation and location were precisely characterized, with associated inflammatory changes but no perforation. Push enteroscopy confirmed and successfully removed the FB. Post-procedural imaging excluded complications. This case highlights the diagnost

Update on treatments for erosive vulvovaginal lichen planus.

Vulvovaginal lichen planus (VVLP) is a debilitating disease that causes significant pain and psychological distress. Management is made difficult by the chronic course of the disease and its resistance to treatment. While topical steroids have been accepted as the first-line treatment, they fail to achieve symptomatic control in approximately 40% of patients. Second-line therapies include other topical treatments such as calcineurin inhibitors, systemic therapies including oral steroids, methotrexate, mycophenolate mofetil, biologics, and tacrolimus, and procedural options including surgery an

Developments in Minimally Invasive Surgical Options for Vertebral Pain: Basivertebral Nerve Ablation - A Narrative Review.

Historically, intervertebral disc degeneration has been the etiological target of chronic low back pain; however, disc degeneration is not necessarily directly associated with pain, and many other anatomical structures are potential etiologies. The vertebral endplates have been postulated to be a source of vertebral pain, where these endplates become particularly susceptible to increased expression of nociceptors and inflammatory proliferation carried by the basivertebral nerve (BVN), expressed on diagnostic imaging as Modic changes. This is useful diagnostic information that can help physicia
