BACKGROUND: Recurrence rates and long-term functional outcome after surgical treatment of anal fistula in Crohn's disease were assessed.
METHODS: A consecutive series of patients was treated for Crohn's fistula in ano; those without proctitis or active sepsis underwent surgery. Sex, seton usage, infliximab, previous fistula surgery, history of segmental resection and smoking were examined as risk factors for recurrence. Continence was assessed by Vaizey scale and a colorectal Functional outcome questionnaire. Results were compared with institutional data for cryptoglandular fistulas.

BACKGROUND: Soft tissue radiation necrosis (STRN) is effectively treated with hyperbaric oxygen (HBO,), believed to result from stimulation ofangiogenesis in radiation-injured tissue. In a prospective study the results of local surgery in perianal infections in patients with and without Crohn's disease were compared. In patients with Crohn's disease (CD) the abscesses and fistulas were more extensive and complicated than in patients without Crohn's disease (NCD). The source of infection was cryptoglandular in 81% (CD) resp. 100% (NCD). The wound healing in CD-patients was four weeks longer than in NCD patients. Fifty nine stage II-distal (i.e. with involvement of medium third of parametrium and/or vagina), 220 stage III and 5 stage IV cervical carcinomas were treated with radiotherapy alone (combination of intracavitary radium therapy and megavoltage external irradiation on the pelvis) in the years 1971-79. A 10 year follow-up of 109 patients with histologic Crohn's disease and anal lesions is reported. Fourteen patients (13 percent) have died, 7 from unrelated disorders. Ten required excision of the rectum, but only 5 for perianal disease (4.5 percent). Of the remaining 85 patients, 61 have been followed up to proctoscopy and rectal biopsy.
