Treating root-surface caries.

This article summarizes the effectiveness of restorative materials used to restore root surfaces, the mechanisms by which these materials reduce caries, and placement techniques for restoring root-surface lesions. Patients may be classified into low, medium, and high caries risk groups for root caries, and specific dental restorative material recommendations are made for each category. Effective plaque control, xylitol-containing chewing gums, antimicrobial agents, fluoride-releasing restorative materials, topically applied fluoride, and fluoride-containing toothpastes provide maximum protecti

Sealer distribution in coronal dentin.

A major cause of tooth discoloration is sealer remnants in the pulp chamber after root canal treatment. The purpose of this study was to assess coronal distribution and color changes of four commonly used sealers placed in the pulp chamber after 2 yr. Fifty extracted premolars were cross-sectioned in the coronal third of the root. The chamber contents were removed, and instrumentation was via the canal; then freshly mixed sealer was placed in each chamber. Sealers evaluated were: AH 26, Kerr Pulp Canal Sealer, Roth 801, and Sealapex. The apical access was sealed with white sticky wax, and the

Restorative materials for direct coronal restoration of permanent posterior teeth: an overview of systematic reviews.

BACKGROUND: Direct-placement dental restorative materials are required to replace the loss of tooth substance and restore the functional structural integrity of damaged posterior teeth. Whilst dental amalgam was traditionally used for these restorations, there are concerns about toxicity to human health and the environment. The Minamata Convention on Mercury recommends a phase-down of amalgam use in dentistry. Alternative mercury-free direct-placement restorative materials are available and have been evaluated in systematic reviews. OBJECTIVES: To summarise the evidence from Cochrane and othe

Shear bond strength of a self-etching primer after 10,000 and 20,000 thermal cycles.

PURPOSE: To determine the effects of thermocycling on shear bond strengths (SBSs) of a self-etching primer (SEP)after 10,000 and 20,000 thermal cycles. The tested hypothesis was that 10,000 and 20,000 thermal cycles would affect the bond strength of metal brackets bonded to enamel with the self-etching primer. MATERIALS AND METHODS: Brackets were bonded to bovine incisors with two etching protocols. In group CM (conventional method), teeth were etched with 37% phosphoric acid for 30 s. In group SEP, a self-etching primer (Transbond Plus, 3M Unitek) was applied. Brackets were bonded with light

Changes in stiffness of resin-infiltrated demineralized dentin after remineralization by a bottom-up biomimetic approach.

This study examined changes in elastic modulus, mineral density and ultrastructure of resin-infiltrated dentin after biomimetic remineralization. Sixty demineralized dentin beams were infiltrated with Clearfil Tri-S Bond, One-Step or Prime&Bond NT. They were immersed in simulated body fluid (SBF) for 1 week to maximize water sorption before determining the baseline elastic moduli. For each adhesive (N = 20) half of the beams remained immersed in SBF (control). The rest were immersed in a biomimetic remineralization medium. The elastic moduli were measured weekly for 15 additional weeks. Repres

Tooth fragment reattachment following crown root fracture: a case report.

A 32-year-old man presented with a complicated crown-root fracture of a maxillary lateral incisor. The fracture extended subgingivally, and apical to the alveolar bone crest, invading the biologic width. Flap surgery to expose the fractured root face was performed and the coronal tooth fragment reattached with a dual-cure resin adhesive. Examination six months after treatment revealed periodontal health, good aesthetics and normal function.

Two-year evaluation of a new nano-ceramic restorative material.

The purpose of this prospective study was to evaluate the clinical performance of a new restorative material (Ceram.X) in combination with a new primer-adhesive (K-0127). One operator placed two Class I or II restorations in molars of 43 patients. One molar was restored with Ceram.X/K-0127, the other one with Tetric Ceram/Syntac Classic. At baseline, after 1 and 2 years, the restorations were evaluated by one evaluator using modified Ryge's criteria. After 2 years, 31 patients were examined. One Ceram.X-restoration had to be removed for root canal treatment due to pulpitis. Thus, failure rate

[Free temporoparietal osteomuscular flap for cranioplasty].

PURPOSE: Advances in composite materials for craniofacial reconstruction surgery has tended to limit indications for osteomuscular free flaps which nevertheless must be used for certain septic patients. The purpose of this report was to illustrate the usefulness of the free temoroparietal osteomuscular flap in this particular situation. CASE REPORT: A 47-year-old man underwent surgical repair of an aneurysm of the anterior communicating artery complicated by acute hydrocephaly treated by external then ventriculoperitoneal bypass. The fronto-pteryonal approach was used. The early postoperative

Effect of fiber post and cusp coverage on fracture resistance of endodontically treated maxillary premolars directly restored with composite resin.

INTRODUCTION: There are different reinforcement methods in restoring root-filled teeth. The aim of this in vitro study was to evaluate the effect of fiber post and cusp coverage on fracture resistance of endodontically treated maxillary premolars directly restored with composite resin. METHODS: Seventy-five maxillary premolars were divided into 5 groups (n = 15). Except for the control group (intact teeth), in other groups mesio-occlusodistal (MOD) cavities were prepared after endodontic treatment. In the groups with cusp coverage, both buccal and lingual cusps were reduced up to 2 mm. Then s

Effect of implant connection and restoration design (screwed vs. cemented) in reliability and failure modes of anterior crowns.

The mechanical performance of cemented or screw-retained implant-supported crowns with an internal or external configuration is yet to be understood. This in vitro study evaluated the effect of screw-retained and cement-retained prostheses on internal and external implant-abutment connections. Thereby, the reliability and failure modes of crowns were investigated. Eighty-four implants (Emfils; Colosso Evolution system) were divided into four groups (n=21 each): screw-retained and internal connection (Si), screw-retained and external connection (Se), cement-retained and internal connection (Ci)

Comparison of conventional vs self-etching adhesive bonds to caries-affected dentin.

The mechanism of resin adhesion to caries-affected dentin is still unclear. This study evaluated the interfacial morphology of two bonding systems to caries-affected dentin, coupled with the measurement of microtensile bond strengths (mu TBS). Carious human molars were prepared as previously described in Nakajima and others (1995) and were bonded with Single Bond (SB) or FluoroBond (FB) according to the manufacturer's instructions, followed by creation of AP-X composite buildups. After one day of storage in 37 degrees C water, the teeth were serially sectioned vertically into 0.8 mm slabs, tri

Effect of immediate and delayed post preparation on apical dye leakage using two different sealers.

Eighty freshly extracted teeth were endodontically prepared and filled with gutta-percha and either a zinc oxide-eugenol-based sealer or AH26 to determine what effect these sealers, with widely differing properties, would have on apical microleakage after either immediate or delayed dowel space preparation. The teeth were randomly assigned to 1 of 4 groups, and the post space was made either immediately after filling or after being stored in 100% humidity for 1 wk. They were then immersed in a 2% methylene blue solution under vacuum, washed, and split longitudinally. The extent of dye penetrat
