Is excessive daytime sleepiness with periodic leg movements during sleep a specific diagnostic category?

Thirty-four patients who presented with excessive daytime sleepiness (EDS) and who showed an elevated number of periodic leg movements during sleep (PLMS) were studied. None of these patients reported other symptoms or presented sleep laboratory manifestations of narcolepsy or of breathing disorders during sleep. A diagnosis of restless leg syndrome, head trauma or a past history of psychopathology or infectious diseases known to cause EDS were also ruled out. In addition, none of the patients reported a history of drug or alcohol abuse, chronic sleep deprivation or irregular sleep-wake schedu

Comorbidities in children with elevated periodic limb movement index during sleep.

STUDY OBJECTIVES: Little is known about comorbidities in children who have elevated periodic limb movement index (PLMI) during overnight pol … demographics, clinical presentation, medications, medical history, family history specifically for restless legs syndrome (RLS), ferritin levels, and PSG metrics. Characteristics between those with (cases) elevated PLMI (AASM criteria) and without (controls) were summarized. RESULTS: We identified 148 subjects with elevated PLMI (67% male, mean age 7.95 years, range 1 …

Diagnosis and treatment of insomnia.

Insomnia is a disorder of initiation and maintenance of sleep that results in daytime somnolence. The differential diagnosis of the various forms of insomnia is based primarily on the history, including information from the sleeping partner. The possibility of underlying depression or sleep apnea must be given consideration in every patient with insomnia, because inappropriate therapy may be dangerous in these instances. In general, the benzodiazepines have supplanted the traditional hypnotics in the treatment of insomnia.

[Neurology and sleep medicine].

Neurology is frequently confronted with sleep disturbances and/or their effects. Sleep disturbances may be early, main- or escort symptom, in addition as well a factor of risk for a neurological illness. Goal of this contribution is to mediate a small overview of usual organic sleep disturbances and to give an insight into sleep medicine ways of thinking.

Epidemiological studies have established that sleep disorders are common and often untreated. Besides having a negative impact on overall he … re prevalent sleep disorders, including narcolepsy, insomnia, obstructive sleep apnea syndrome, and restless legs syndrome, with a summary and critique of medications used to treat these conditions. For each disorder, information is provided on recent approaches taken to develop novel therapeutics based on laboratory findings relating to the underlying biological abnorm …

The impact of change of renal replacement therapy modality on sleep quality in patients with end-stage renal disease: a systematic review and meta-analysis.

BACKGROUND: Sleep disorders are common and multi-factorial in patients with advanced chronic kidney disease and end-stage renal disease (ESR … modality switch. CONCLUSIONS AND IMPLICATIONS OF KEY FINDINGS: Sleep disturbance, sleep apnoea and restless legs syndrome all tend to improve when a switch is made to intensive dialysis or transplant. This is important information for patients struggling with disturbed sleep and marked fatigue. This hypothesis-generating review highlights the need for more high quality …

Comparison of sleep variables between chronic widespread musculoskeletal pain, insomnia, periodic leg movements syndrome and control subjects in a clinical sleep medicine practice.

BACKGROUND: Between 50% and 89% of chronic pain patients report unrefreshing sleep. The aim of this retrospective analysis was to compare the sleep of normal subjects with the sleep of a clinical population presenting musculoskeletal chronic widespread pain (CWP), psychophysiological insomnia and restless legs syndrome/periodic limb movements during sleep (RLS/PLMS) in order to identify sleep variables that may explain the poor sleep complaints of CWP patients. METHODS: Sleep data from 10 normal subjects and 37 patients (mean age 55+/-3 yo), matched for age and sex, were retrieved from our sl…

[Periodic limb movement disorder].

The periodic limb movements (PLM) are defined as stereotyped, periodic movements of the legs and/or upper limbs during sleep. The patient exhibits dorsifilexion of the ankle and extension of the big toe with occasional flexion of the knee and hip. PLM originally was described as "nocturnal myoclonus" by Symonds in 1953. Recently, the term "nocturnal myoclonus" has been replaced with PLM, because the movements are slower than true myoclonic movement. The appearance of PLM was reported in sleep apnea syndrome, delayed sleep phase syndrome, narcolepsy, spinal cord tumor, diabetes mellitus and ure

A genetic risk factor for periodic limb movements in sleep.

BACKGROUND: The restless legs syndrome (RLS) is a common neurologic disorder characterized by an irresistible urge to move the legs. It is a major cause of sleep disruption. Periodic limb movements in sleep are detectable in most patients with RLS and represent an objective physiological metric. METHODS: To search for sequence variants contributing to RLS, we performed a genomewide association study and two replication studies. To minimize phenotypic heterogeneity, we focused on patients with RLS who had objectively documented periodic limb movements in sleep. We measured serum ferritin level…

[Sleep induction].

Physiological sleep induction is a fragile phenomenon which may be altered by environmental factors and various somatic and psychiatric disorders. Therefore sleep induction disorder is a common component of different types of insomnia, including sleep-onset insomnia and to a lesser degree multiple awakenings insomnia and early morning awakening insomnia. The treatment of sleep induction disorders is difficult and requires a precise analysis of the disorder and its aetiological factors. Specific treatment of aetiological factors is more likely to be efficacious in the long term than pharmacolog

Pramipexole ameliorates cognitive deficits in aged mice after sevoflurane anesthesia by upregulating dopamine D2 receptors in parvalbumin interneuron of the hippocampus.

Pramipexole (PPX), dopamine D2/D3 receptor agonist, exhibits neuroprotective effects in patients with restless legs syndrome and Parkinson's disease. The present study aimed to investigate the cellular and molecular mechanisms underlying PPX treatment in memory and cognitive impairments induced by sevoflurane anesthesia in the aged mice. Aged mice received 2-h daily exposures to 60% oxygen, either with or without 3% sevoflurane, for three consecutive days. For interventional evaluation, PPX was administered intraperitoneally 1 h before sevoflurane exposure on three consecutive days. After the …

The Effect of Dopaminergic Medication on Impulse Control and Compulsive Behaviour: A Translational Perspective.

Dopaminergic medication used in disorders like Parkinson's disease (PD) and restless legs syndrome can cause impulsive-compulsive behaviour (ICB), often with strong negative effects on patients' quality of life. This narrative review presents translational evidence on iatrogenic ICB, taking findings from epidemiological, clinical, neuroimaging and preclinical studies into consideration. Epidemiological and clinical studies find dopamine agonists with high D2/3-selectivity to be most strongly linked to ICB. Their effect on ICB has often been shown to be dose-dependent, but the impact of combini…

Mining and disproportionality analysis of adverse events signals for naltrexone based on real-world data from the FAERS database.

BACKGROUND: This study aims to analyze adverse events (AEs) associated with naltrexone based on the FAERS database, providing a foundation f … er, tremor, delirium, and decreased libido. Additionally, our findings suggested potential risks of restless legs syndrome, eosinophilic pneumonia, and otolithiasis, which were not mentioned in the drug's label, thereby supplementing the existing safety information. CONCLUSIONS: The analysis of the FAERS database identified AEs associated with naltrexone, contributing …
