OBJECTIVE: To investigate a potential association between cervical spine injury and first- and second-rib fractures.
METHODS: Retrospective review of the radiologic and medical records of 28 consecutive patients admitted to an acute spinal injury unit.
RESULTS: A total of 10 (36%) of the patients with cervical spine trauma also had fractures of either the first or second ribs. Eight of these 10 patients had a fracture or fracture-subluxation of the seventh cervical vertebra or the C6/7 segment.

Fractures of the first rib are extremely rare and are more commonly associated with either multiple rib fractures or life-threatening injuries. First rib fractures are commonly caused by direct trauma, violent muscular contraction or by chronic stress. We present a case of isolated bilateral rib fracture in an elderly patient, in the absence of multiple trauma, where the mechanism of injury appears to be a whiplash injury.

The potential cause of stress fractures of the rib in elite rowers was examined by a retrospective review of 14 fractures in 10 patients. We present premature female twin fetuses with concordant extremely shortened ribs, short limbs, macrocephaly, median cleft upper lip and facial dysmorphism. Based on radiological criteria and the pattern of associated abnormalities, a lethal short rib-polydactyly syndrome (Beemer-Langer type) was diagnosed. The differential diagnosis of this entity is discussed.

Reconstruction of humeral defects with a pedicled compound rib-latissimus dorsi osteomusculocutaneous flap is described. This flap consists of a latissimus dorsi musculocutaneous flap and segments taken from two ribs. In addition to the advantages of the conventional latissimus dorsi musculocutaneous flap, it enables multiple ribs to be transferred to the recipient site. This compound flap transfer can be used as a choice for a reconstructive procedure of the upper arm combined with humeral defects.

The results of transaxillary excision of the first rib for thoracic outlet syndrome are reported. During a 3-year period, 40 transaxillary rib resections were performed on 32 patients. The symptoms in 33 limbs were completely relieved and in a further four symptoms were improved. These results confirm that transaxillary excision of the first rib is the operation of choice in the management of thoracic outlet syndrome.

We assessed rib cage (RC) to abdominal (AB) asynchrony in 110 children, aged 1 to 50 months who underwent diagnostic daytime polygraphic sleep studies (PSG), and correlated the results. RC-AB asynchrony was calculated by the phase angle (PA) technique and compared to normal values (0-24 degrees) derived from a separate group of 45 control children, matched for age and weight.
