Persistent falsely positive rapid tests for infectious mononucleosis. Report of five cases with four--six-year follow-up data.

In many laboratories, rapid slide tests incorporating differential absorption have replaced heterophil antibody tube tests in the investigation of suspect infectious mononucleosis. The present report describes serologic data from five individuals whose Monospot tests remained falsely positive for four years or more without other evidence of infectious mononucleosis or underlying disease states. Three of the five had never been infected with the Epstein-Barr virus. All five had negative results when studied with the ox cell hemolysin and the horse cell differential absorption tube tests. False-

Recurrence of Wegener's granulomatosis 13 years after renal transplantation.

Wegener's granulomatosis (WG) can cause renal failure, requiring long-term renal replacement therapy. Renal transplantation in patients with WG is successful, but the risk for recurrence of the disease necessitates continued vigilance. We report a patient that originally presented with acute renal failure secondary to a pauci-immune focal necrotizing crescentic glomerulonephritis. Subsequent nasal involvement and serologic tests for antineutrophil cytoplasmic antibodies suggested a diagnosis of WG.

Brucellosis: atypical presentation with peritonitis and meningitis.

A case of brucellosis associated with peritonitis, intestinal obstruction, granulomatous hepatitis, inappropriate antidiuretic hormone (ADH) secretion and meningitis is reported. Initially, the patient was diagnosed as a case of disseminated tuberculosis and treated accordingly. However, the serologic tests for brucellosis were strongly positive and the patient was subsequently treated as a case of brucellosis and recovered fully. The gastrointestinal manifestations of brucellosis are reviewed.

Mycobacterial, fungal, actinomycotic, and nocardial infections of the pleura.

Granulomatous pleuritis is relatively common, comprising about 10 per cent of all pleural effusions. A search for the etiologic agent is important since mycobacteria, fungi, and the higher bacteria Actinomyces and Nocardia produce similar clinical, radiographic, and pleural fluid findings. The appropriate use of diagnostic tests including pleural biopsy and serologic techniques is discussed, as are current approaches to the management of these infections.

[Localized scleroderma (morphea) and septic arthritis. Clinical manifestations of Lyme borreliosis seen in El Ferrol].

Two cases of Lyme's disease seen at El Ferrol (Spain) were described. One of them developed a recurrent knee arthritis and the other had a localized sclerodermia (morphea) syndrome. Diagnosis was established by means of clinical picture and serologic tests (enzyme-linked analysis and/or indirect immunofluorescence tests). Joint involvement has often been described in patients diagnosed of having Lyme's disease in Spain, however, the relationship between morphea and borreliosis is still a matter of controversy. We believe that patients with localized sclerodermia and high significant titers of …

Recognizing a MIS-Chievous Cause of Acute Viral Gastroenteritis.

Historically, children evaluated for vomiting and diarrhea secondary to viral enteritis have symptoms lasting 2-4 days and respond to supportive care, including oral rehydration and anti-emetics if required. Recently, within a 14-day timespan, we encountered three children with severe diarrhea who rapidly became dehydrated and went into hypotensive shock. Although SARS-CoV-2 molecular tests were negative by nasopharyngeal swab, all were later found to have MIS-C. This small case series underscores features reported in previous larger studies and emphasizes the rapid clinical evolution of this

Rapid diagnosis in serology.

Serological diagnosis of acute phase infections implies the detection of IgM specific response, an effective marker of primary infection, but with less clinical significance in reactivations or reinfections. The aim of this article is to provide an updated view of the rapid diagnosis in serology by detecting the IgM isotype and reviewing its applications and limitations. Point-of-care (PoC) tests are analyzed. PoC tests are used in geographical areas where traditional tests are not available, as well as in other circumstances where their use brings the diagnosis directly to the target populati

Future application of research tests to the diagnosis of yaws and other endemic treponematoses.

Recent advances in treponemal research have increased our understanding of the pathogenesis and host response to treponemal infection was well as the antigenic structure of the pathogenic and nonpathogenic treponemes. Although these new methodologies are currently limited to research laboratories, the new findings may ultimately have practical applications to the control of the treponematoses. It is reasonable to expect that reagents for the identification of pathogenic treponemes (vs. non-pathogens) will be commercially available in the next three years. The field application of these reagent

[Effectiveness of the surface fixation test in the diagnosis of brucellosis in hares].

In the course of two years the reliability of surface fixation test was tested in the diagnostics of hare brucellosis on 961 hare blood samples taken during chases at chosen localities of the Olomouc district. Out of 33 serologically positive hares, 29 reacting ones were subjected to complex laboratory examination for brucellosis. In all cases brucellosis was confirmed patho-anatomically, by cultivation in 78.95% of bacteriologically examined hares and in the rest cases by bioassay on guinea-pigs. The results have confirmed that the surface fixation test represents a highly effective method fo
