The authors present the clinico-pathological correlation in a women who developed at the age of 43 years progressive pain in the left hemiface with motor and sensitive disorders of the left trigeminal nerve. Progressive involvement of the left IIIrd, IVth and Vth cranial nerves appeared with persistence of intractable facial pain which was not responsive to treatment by Tegretol, nor by thermocoagulation of the left Gasserian ganglion. Partial resection of a tumor was performed. The biopsy revealed its cylindromatous nature. The patient died after eight years of evolution. Autopsy confirmed an

OBJECTIVE: The aim of this study was to develop and validate an anatomy-driven model that predicts gross-total resection (GTR) of skull base chondrosarcomas (SBCs).
METHODS: The authors conducted a retrospective multi-institutional analysis of consecutive SBCs resected at 4 academic centers in 2000-2024. Thirteen preoperative variables of interest included tumor location (petroclival, peri-lacerum, anterior, or lateral skull base), maximal tumor diameter, internal carotid artery (ICA) encasement, compartmental extension, cranial nerve (CN) involvement, prior radiotherapy and/or surgery, synch…

BACKGROUND: Endoscopic endonasal approaches (EEA) to skull base tumors are a technically challenging procedure for both surgeons and trainees. Artificial intelligence and computer vision (CV) may offer an objective analysis that provides surgical assessment on technique and enhanced educational feedback for surgeons of all levels.
METHODS: Patients undergoing EEA from a tertiary care center between January 2022 and January 2023 were retrospectively analyzed. A large, publicly available CV model fine-tuned on endoscopic surgical video in collaboration with Surgical Data Science Collective (SDS…

BACKGROUND AND OBJECTIVES: The far lateral craniotomy is a foundational approach for ventrolateral skull base lesions. We present our experience with a minimally invasive curvilinear retroauricular incision as a variation to the traditional midline "hockey-stick" incision for far lateral craniotomies.
METHODS: In this retrospective cohort study, we identified patients who underwent a far lateral craniotomy through a curvilinear retroauricular approach at our quaternary referral hospital between January 2012 and January 2025. The primary outcome measures were complications, postoperative defic…

The endoscopic endonasal approach (EEA) to the skull base has revolutionized the management of ventral skull base lesions, offering superior visualization and reduced morbidity. Yet, iatrogenic cranial nerve (CN) injuries, causing temporary or permanent deficits, remain an underexplored complication that significantly affects quality of life. This systematic review describes the number of reported cases of CN injuries in EEA and the evidence on prevention and management strategies. Following PRISMA-SR guidelines, we searched Medline, Embase, Web of Science, and the Cochrane Library, and screen…

INTRODUCTION: Hepatocellular carcinoma is the most common neoplasm of the liver. It metastasizes mainly to local lymph nodes, lungs, adrenal glands, vertebrae, pelvis, ribs and long bones. Metastasis to the clivus is extremely rare. Only a few cases have been described in the literature. Therefore, there is currently no clear consensus on the optimal treatment: biopsy only or maximum safe removal.
CASE REPORT: We present a 68-year-old lady who presented with dysfunction of the 6th and 12th cranial nerves. Imaging showed tumor formation in the clivus, most likely a metastasis. Further diagnost

INTRODUCTION: The tympanic bone (TB) is 1 of the 4 essential parts of the temporal bone, located on its lateral-inferior aspect. It plays a pivotal role in forming the bony part of the external auditory canal, facilitating the transmission of sound to the middle ear.
OBJECTIVE: The current study aims to underline the surgical importance of the TB and its role as a key structure in lateral skull-base surgery, particularly the infratemporal fossa approach type A and removal of the TB during the surgical management of tympanojugular paraganglioma (TJP).
MATERIALS AND METHODS: In this study, we

BACKGROUND: The pedicled nasoseptal flap (Hadad-Bassagasteguy flap, HBF) has been considered as the workhorse for reconstruction of high-flow cerebrospinal fluid (CSF) leak following endoscopic endonasal skull base surgery. However, if the septal mucosa is affected by a tumor or the vascular pedicle is injured due to prior surgery, the use of HBF is precluded.
METHOD: We illustrate the harvest techniques of the lateral nasal wall flap (LNWF) based on the posterior lateral nasal artery (PLNA).
CONCLUSION: The LNWF is a robust intranasal regional flap with a consistent blood supply and compara…

Primary angiosarcoma of bone (PAB) is an exceedingly rare entity characterized by aggressive clinical behavior. Following surgical removal, systemic relapse is common, and the overall prognosis is poor. Several systemic therapy agents and combinations have been used in the treatment of PAB. However, the evidence for their efficacy is limited to retrospective case series and case reports, making it difficult to assess their relative effectiveness. We report a case of systemically relapsed PAB along with a literature review aimed at increasing the available information on PAB responses to differ

PURPOSE: The escalating number of endoscopic skull base procedures necessitates exploring additional materials to reduce postoperative cerebrospinal fluid (CSF) leaks in revision or staged surgeries. This study evaluates the effectiveness of reused nasoseptal flaps (NSFs) in such clinical scenarios.
METHODS: A retrospective review was conducted on patients who previously underwent surgery involving NSFs and later had revision or secondary skull base surgeries via endoscopic endonasal approaches (EEAs) at a tertiary medical center. Patients were divided into two groups: one with reused NSFs an…

Aspergillus mastoiditis with skull base osteomyelitis is an exceptionally rare but life-threatening infection in patients undergoing peritoneal dialysis (PD). We report a 60-year-old man with diabetic kidney failure on PD who developed invasive Aspergillus flavus mastoiditis progressing to skull base osteomyelitis. Despite early mastoidectomy, PD catheter removal, and voriconazole therapy achieving therapeutic levels, the infection advanced to involve the carotid sheath and parapharyngeal space, resulting in severe neurologic and systemic complications. Molecular sequencing confirmed A. flavus…

We report a rare case of thyroid cancer with tumor embolism of the internal jugular vein (IJV) that progressed cranially into the skull base and caused multiple cranial neuropathies. An 80-year-old woman presented with a progressively enlarging right cervical mass, which had been neglected for 10 years. Ultrasonography and magnetic resonance imaging revealed a cystic cervical lesion with a tumor embolism within the IJV extending to the jugular foramen and hypoglossal and facial canals. Fine-needle aspiration cytology yielded inconclusive results on two occasions, likely because of cystic degen…

BACKGROUND: Watertight duraplasty is essential for surgical management of traumatic anterior skull base (ASB) dural defect but challenging in the deep and narrow operative corridor. Here, the authors report a trans-defect underlay watertight duraplasty (TDUWD) technique for traumatic ASB dural defect.
METHODS: TDUWD was performed by inserting a free pericranium graft under the dural defect. The diameter of the pericranium graft was larger than the dural defect. The pericranium graft was sutured to the dural defect watertightly in an "inside-to-outside" direction, with the needle not penetrati…
