Free-hand electrode placement for intraoperative monitoring of extraocular cranial nerves in skull base surgery: preliminary experience and feasibility assessment.

Postoperative dysfunction of the oculomotor (CN III) and abducens (CN VI) nerves remains a major determinant of disability after skull base surgery for tumors. This study assessed the feasibility, safety, and diagnostic performance of a surgeon-controlled, free-hand extraocular muscle electrode placement for corticobulbar motor evoked potentials (cb-MEPs) and direct nerve stimulation (DNS). This monocentric, observational, retrospective study enrolled 40 patients scheduled for skull base tumor surgery, with planned intraoperative monitoring of CN III and/or VI. Curved needle electrodes were pl

Endoscopic Endonasal Reconstruction of Intraoperative Cerebrospinal Fluid Leak in Different Skull Base Regions: Outcomes, Meningitis, and Risk Factors.

OBJECTIVES: Various nonvascularized or vascularized techniques have been adopted in endoscopic endonasal surgery (EES) for repairing intraoperative cerebrospinal fluid (CSF) leaks after tumor resection. Vascularized nasoseptal flaps, free nasoseptal grafts, free turbinate grafts, and fascia lata and mashed muscle are frequently used. Outcomes of those grafts applied in the defects of different regions need to be clarified. METHODS: The data from a series of 162 patients with skull base tumor who underwent EES that had intraoperative CSF leak between Jan 2012 and Jan 2021 were retrospectively

Visual recovery following optic nerve decompression for chronic compressive neuropathy.

PURPOSE: Visual failure due to optic nerve compression is a common indication for decompressive surgery. Most data only refer to the odds of improvement, deterioration or remaining the same. However, patients frequently wish to know more detail about the outcomes of surgery. Our aim was to assess the visual outcome from optic nerve decompression for visual failure in detail in order to help counsel patients pre-operatively. METHODS: Sixty-eight patients undergoing 71 operations to decompress 87 optic nerves between 1991 and 2007 were identified. Thirty-four decompressions were performed via a

Imaging features of posterior fossa neoplasms in children and adults.

Masses in the posterior fossa may be divided into extra-axial and intra-axial lesions. Because of its multiplanar capabilities, improved soft-tissue resolution and contrast, and the absence of scanning artifacts related to the osseous skull base, which are frequently present on CT, MRI is the imaging modality of choice in evaluating lesions arising in the posterior fossa. It is the best means of critically localizing lesions and determining the extent of disease. Analysis of the signal characteristics of a lesion on multiple different imaging sequences (T1-weighted, T2-weighted, and enhanced i

Juvenile nasopharyngeal angiofibroma: an update of therapeutic management.

BACKGROUND: Juvenile nasopharyngeal angiofibroma (JNA) is a benign tumor of the nasopharynx composed of fibrous connective tissue and an abundance of endothelium-lined vascular spaces. The name of the disease is derived from the fact that it occurs during adolescence, almost exclusively in boys. METHODS: This study examines the natural characteristics of JNA, the imaging techniques used to diagnose and stage the neoplasm, and the treatment approaches used to manage the disease. RESULTS: Forty-three cases of JNA diagnosed and treated at the University of Texas M. D. Anderson Cancer Center ove

Operative management of skull base malignant tumors arising from the nasal cavity and paranasal sinus: recent strategies used in 25 cases.

Cancers of the paranasal sinuses and nasal cavity are the most common malignant tumors of the anterior and anterolateral skull base. The treatment of these tumors affecting the skull base is complex due to the significant anatomical features. We examined 25 patients, 17 males and 8 females with mean age 61 +/- 2 years. En bloc resections using anterior skull base resection, orbital resection, middle fossa resection, and combined procedures of these three resections were performed. Using a combination of adjuvant radiation and chemotherapy, we have achieved a 2-year disease-free survival rate o

Craniofacial approaches and reconstruction in skull base surgery: techniques for the oral and maxillofacial surgeon.

Skull base surgery (SBS) is considered the standard of care in treating benign and malignant lesions of the cranial base. SBS is a multidisciplinary team approach used to treat these complex lesions that may have intracranial extension. SBS can be broken down into 3 steps. Transfacial access is performed, followed by resection with sound oncologic principles, and then reconstruction of the cranial base and facial structures. Functional and esthetic concerns must be addressed by the surgeons. Oral and maxillofacial surgeons frequently perform elective facial osteotomies and treat victims of cra

[Solitary Fibrous Tumors of the Nasal and Paranasal Sinuses].

Solitary fibrous tumors (SFT) are uncommon neoplasm that arises in most cases from the pleura. SFT has been rarely observed in the head and neck, but SFT of the paranasal sinuses is especially rare, with 39 previously reported cases to date including those reported in this abstract. Herein we describe three cases of SFT in the paranasal sinuses that were successfully treated endoscopically. Two of the three cases involved patients with no previous history of SFT. The lesion of one of the patients was pathologically diagnosed as SFT preoperatively, but the other was diagnosed as an angiogenic t

Resectability issues with head and neck cancer.

Head and neck surgeons often rely on imaging to determine if a neoplasm is resectable. Many of the critical issues are outlined in the American Joint Committee on Cancer Staging Manual, wherein T4a and T4b head and neck cancers are defined as resectable and unresectable, respectively. Even within the T4a advanced resectable classification, there are critical determinants that define whether the surgical option is such that major morbidity and mortality could be expected. This review article examines the imaging literature to determine the accuracy and diagnostic criteria of different modalitie

[Diagnosis and treatment of hypoglossal neurinoma].

OBJECTIVE: To investigate the symptoms, diagnosis, operative approaches and prognosis of hypoglossal neurinoma. METHODS: The clinical symptoms, radiological features, prevalence, operative approaches and prognosis of 9 cases of hypoglossal neurinoma were analyzed. RESULTS: Cases of neurinoma of the hypoglossal nerve accounted for 1.9% of the cases of skull base neoplasm in the same period. Most of the tumor was in the shape of dumbbell, involving both intracranial and extracranial segments of the nerve. In a few cases, the tumor mainly involved the intracranial segment of hypoglossal nerve.

Magnetic resonance imaging of the brain and spine.

The efficacy of magnetic resonance imaging (MRI) in the diagnosis of diseases of the central nervous system is reviewed. MRI, computed tomography (CT) and certain radionuclide studies are compared in the evaluation of intracranial tumours, cerebral vascular disease, multiple sclerosis and other white matter diseases, dementia, head injury, infection, epilepsy, spinal lesions and in paediatric central nervous system disorders. The measurement of cerebrospinal fluid volumes and dynamics by MRI is discussed. MRI most clearly has advantages where CT is degraded by bone hardening and streak artefac

Factors contributing to failure in endoscopic skull base defect repair.

BACKGROUND: Endoscopic repair of skull base (SB) defects is successful in over 90% of cases. Certain factors may contribute to failure of SB repair techniques or need for secondary repair. METHODS: Five-year retrospective review of endoscopic SB defect repairs performed by a single surgeon. RESULTS: Eighty-nine patients undergoing 110 procedures to repair 97 SB defects were evaluated. Etiology of defects included surgical/iatrogenic (64%), spontaneous (17%), traumatic (12%), congenital (6%), and idiopathic (1%). Defects occurred in the sella (41%), sphenoid sinus (18%), ethmoid roof (17%), o

Tuberculum sellae meningiomas: grading scale to assess surgical outcomes using the transcranial versus transsphenoidal approach.

OBJECTIVE Tuberculum sellae meningiomas (TSMs) are surgically challenging tumors that can severely impair vision. Debate exists regarding whether the transcranial (TC) or endoscopic transsphenoidal (TS) approach is best for resecting these tumors, and there are few large series comparing these approaches. METHODS A retrospective chart review was performed at 2 academic centers comparing TC and TS approaches with respect to vision, extent of resection, recurrence, and complications. The authors report surgical outcomes and propose a simple preoperative tumor grading scale that scores tumor size

Evidence-based practice: endoscopic skull base resection for malignancy.

Successful outcomes of endoscopic approaches to benign sinonasal tumors have launched interest in expanding its use for sinonasal malignancy. Because of the heterogeneity and rarity of sinonasal malignancy, evidence for clinical outcomes of endoscopic approaches versus traditional craniofacial resection is low. Using the Oxford Center for Evidence-based Medicine guidelines, we present the existing evidence comparing both techniques for a variety of sinonasal malignancies.

Radiation induced optic neuropathy: Does treatment modality influence the risk?

Radiation induced optic neuropathy (RION) is a rare but disastrous complication of radiation therapy in treatment of periorbital tumors. The objective of this study is to investigate the incidence of RION in series of patients treated from peri orbital tumors by recent photon and proton irradiation modalities. We searched the Pub Med database for studies in periorbital tumors including base of skull, sinonasal, pituitary, nasopharyngeal tumors and craniopharyngioma treated with Intensity modulated radiotherapy (IMRT) and with proton beam therapy (PBT) between 1992 and 2017 excluding metastatic

Paranasal sinus metastasis of breast cancer.

A 76-year-old woman presented with symptoms suggestive of acute sinusitis. Previously, her breast carcinoma was treated with right lumpectomy, adjuvant chemotherapy and breast radiotherapy. She remained free from recurrence for the following 8 years. After initial treatment with antibiotics, the local symptom worsened with exophthalmos, eye blindness and development of an ulceration of the hard palate. MRI showed irregular enhancement of the nasal cavity extended to the maxillary sinus and ethmoidal lamina and concomitant infiltration of the orbit and skull base. A biopsy of the palatal ulcer
