Sleep apnea syndrome (SAS) results from modification in the control of respiration and of upper airway caliber during sleep. Although there is some overlap between central (CSAS) and obstructive (OSAS) sleep apnea syndromes, each syndrome has specific pathological associations. The first part of this review concerns the pathophysiology of OSAS, including periodic breathing and upper airway collapse. In the second part, each specific etiology is examined, and the respective contribution of anatomic narrowing and neuromuscular dysfunction of the upper airway is mentioned. Our experience with abo…

On three different occasions, six healthy young adult subjects ahd their body temperatures raised by an average of 2.0 degrees C for 30 min while sitting in baths of warm water. This was done once at 1700 h and on two occasions at 2100 h, once after the subjects had taken aspirin and once after a placebo. Nighttime sleep was recorded after each experimental condition and for baseline nights following nil heating. Records were scored both visually and by an automated sleep stager. Electroencephalographic (EEG) power was computed over the night. Results from the automated scoring were very simil…

The relative sleep time response of long-sleep (LS) and short-sleep (SS) mice was determined after the i.p. injection of varying doses of several anesthetic agents that vary in lipid solubility. The LS and SS mice were bred selectively for differences in duration of ethanol-induced sleep time. The drugs tested included: several halogenated ethanol derivatives (trifluoro-, trichloro-, tribromo- and dichloroethanol), chloral hydrate, paraldehyde, urethane and the water soluble barbiturates, barbital and phenobarbital. The LS mice slept for a longer time after the i.p. injection of the majority o…

Parasympathetic cardiac vagal neurons (CVNs) in the brainstem dominate the control of heart rate. Previous work has determined that these neurons are inherently silent, and their activity is largely determined by synaptic inputs to CVNs that include four major types of synapses that release glutamate, GABA, glycine, or serotonin. Whereas prior reviews have focused on glutamatergic, GABAergic and glycinergic pathways, and the receptors in CVNs activated by these neurotransmitters, this review focuses on the alterations in CVN activity with hypoxia-, sleep-, and sleep-related cardiovascular dise…

Cyclic alternating pattern (CAP) of microarousals reported in EEG studies is now regarded as an integrating mechanism for the different parts of the central nervous system including the autonomic system. A recently developed continuous index of cardiac parasympathetic activity (CIPA) can be time locked to the EEG allowing the relationship between EEG and autonomic changes to be studied in sleep. Eleven normal subjects were studied for evidence of periodicity in CIPA in non-REM sleep, five of whom had been sleep deprived. Fast Fourier's transformation of the CIPA data confirmed its periodicity …

Background: The increasing popularity of mountain climbing will result in greater numbers of the general population being at risk for the disturbances known to occur with altitude exposure. Methods: Observations of sleep and breathing were made in 6 healthy travellers (5 males and 1 female, 38 to 62 years of age) before, during, and after a recreational climb. We modified a portable seven channel polygraph to record sleep state, oxygen saturation, respiratory movements, body position, and oronasal airflow 4 weeks prior to the expedition at home (500m), at base camp (4200m) and in 3 climbers at…

This article investigates the progression of sleep disturbances in Alzheimer's disease (AD) and the mechanisms by which these disturbances may accelerate neurodegeneration. Key electrophysiological patterns identified through polysomnography in patients with AD and sleep disorders are analyzed. Therapeutic interventions are reviewed, with non-pharmacological and pharmacological approaches considered separately. The roles of cognitive behavioural therapy, relaxation techniques, and optimization of sleep conditions are emphasized. Current pharmacological treatment options are discussed, highligh…

Sleep disturbances are increasingly recognized as a modifiable risk factor for female infertility. This review summarizes epidemiologic evidence, clinical outcomes, and the mechanistic insights linking sleep-disordered breathing, poor sleep quality, and short sleep duration with impaired reproductive function and reduced success rate of in vitro fertilization. These findings highlight the importance of integrating thorough sleep evaluation and coordinated multidisciplinary care into standard infertility treatment protocols to optimize reproductive outcomes.

Sleep disturbances and cognitive decline are intricately connected, and both are prevalent in aging populations and individuals with neurodegenerative disorders such as Alzheimer's disease (AD) and other dementias. Sleep is vital for cognitive functions including memory consolidation, executive function, and attention. Disruption in these processes is associated with cognitive decline, although causal evidence is mixed. This review delves into the bidirectional relationship between alterations in sleep and cognitive impairment, exploring key mechanisms such as amyloid-β accumulation, tau patho…

BACKGROUND: Sleep and circadian rhythms are affected by aging. Although the relationship between sleep disorders and cellular aging has been demonstrated in some studies, it still remains unclear. Telomere length has been considered one of the sensitive biomarkers for aging in recent years. Studies investigating the relationship between sleep disorders and telomere length are limited and their results are inconsistent. This study aims to demonstrate telomere length in sleep disorders.
METHODS: The study sample consisted of 116 participants, 94 patients and 22 healthy participants. Patients gr…

Sleep disordered breathing (SDB), periodic limb movements of sleep, insomnia, sleep curtailment, and sleep architectural changes are associated with cardiac arrhythmias in community-based and clinical cohort studies. Findings appear to be strongest in magnitude and most consistent for SDB and atrial fibrillation (AF). The mechanisms most consistently identified in animal experimental models include autonomic nervous system fluctuations and intrathoracic pressure alterations, the latter resulting in direct mechanical effects on the thin-walled atria. Other studies point toward intermittent hypo…

As one of the most prevalent malignant tumors, hepatocellular carcinoma (HCC) represents a major global public health burden. Traditionally, HCC pathogenesis has been attributed to chronic liver diseases (viral hepatitis, cirrhosis) and aflatoxin exposure. However, with evolving lifestyles and environmental changes, sleep disorders have become increasingly prevalent. Emerging evidence suggest that sleep disorders may contribute to hepatocarcinogenesis through multiple mechanisms, including immunity environment disorder, oxidative stress, metabolic dysregulation, disruption of gut microbiota, a…

Sleep disorders are commonly found as comorbid problems in patients with migraine. Indeed, there are likely to be numerous levels of interaction between migraine and sleep, including physiological, pathological, and pharmacological. Of note, the presence of sleep disorders may be a modifiable factor in the trajectory of migraine, and therefore active enquiry to elicit their presence, and manage them appropriately, could be an important component in the holistic care of patients with migraine. This review attempts to provide an outline of what is known about these relationships and highlight wh…

Obstructive sleep apnea (OSA) is a common condition linked with multiple comorbidities. Continuous positive airway pressure (CPAP) devices, though the gold standard for OSA treatment, often do not address the entirety of the disorder. Various treatment strategies exist to address OSA beyond CPAP, each targeting different OSA aspects. These include managing comorbid sleep disorders, anatomic considerations, endotype-directed therapy, and lifestyle interventions. These comprehensive approaches aim to improve the benefits of treatment, and reduce the complications associated with OSA.
