Chronic pain is associated with sleep apnea severity but attenuated by intermittent hypoxemia in people using opioids.

Patients using long-term opioids for pain often have uncontrolled symptoms which might drive ongoing opioid use and contribute to the opioid public health crisis. Opioids also cause sleep disordered breathing (SDB), which might impact pain via arousals and hypoxemia, with potential for a vicious cycle. Adults with noncancer pain using opioids daily for >3 months were recruited. Subjects underwent pain and sleep questionnaires, pain tolerance via 10°C cold pressor testing, and polysomnography. Hypoxic burden was determined from polysomnographic signal analysis. Participants (n = 143) had a medi

Subclinical Scores in Self-Report Based Screening Tools for Attention Deficits Correlate With Cognitive Traits in Typical Evening-Type Adults Tested in the Morning.

Previous studies suggest that in adolescents and young adults, evening chronotype is a subclinical factor in physical, cognitive, and psychiatric fitness; poor sleep habits and larger misalignment with the social schedule constraints may exacerbate symptoms of inattention, impulsivity and the risks for detrimental behaviors. The influence of chronotype on neurocognitive performance during morning hours and scores in self-reports about attention deficit symptoms (ADS) and executive functioning, was explored in 42 healthy young university students (29 women), divided to evening type (ET) and com

Men and women with fibromyalgia: Relation between attentional function and clinical symptoms.

OBJECTIVES: This study was the first to explore whether individuals with fibromyalgia (FM) have different cognitive alterations (i.e., in alertness, orienting, and executive control) depending on their sex. We also analysed possible gender differences in the relationships between cognitive functioning and some of the main symptoms of FM. DESIGN: Cross-sectional study. METHODS: Two clinical groups with FM (n = 58 women and n = 20 men) and two healthy control groups (n = 21 women and n = 27 men) aged between 30 and 60 years old participated in this study. Pain intensity, sleep disturbance, dep

Caffeine effects on risky decision making after 75 hours of sleep deprivation.

INTRODUCTION: Recent research indicates that sleep deprivation impairs decision making. However, it is unknown to what extent such deficits are exacerbated in a dose-response manner by increasing levels of sleepiness, and the extent to which such sleep-loss-induced deficits can be reversed by caffeine. METHODS: At three time points, 26 healthy subjects completed alternate forms of the Iowa Gambling Task (IGT): rested baseline, 51 h awake, and 75 h awake. Every 2 h each night, 12 volunteers also received 4 200-mg doses of caffeine, with the last dose occurring 3 h prior to the IGT. RESULTS: A

Exploring the Effects of Z-Score Neurofeedback Training in PTSD: A Preliminary Investigation.

Neurofeedback, a form of biofeedback using electroencephalography, enables individuals to self-regulate brain activity through operant conditioning. This technique shows promise as a non-invasive intervention for neuropsychiatric disorders like post-traumatic stress disorder (PTSD) and may improve cognitive functions such as attention and working memory. However, limited research, particularly using Z-Score neurofeedback, exists on its effects on PTSD-related symptoms, cognitive function, and identifying treatment-specific EEG markers. In this study, twenty-one individuals diagnosed with PTSD

A mixed-method process evaluation of guided online cognitive behavioral therapy for insomnia in patients with borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is often associated with insomnia, which may contribute to a vicious cycle of reciprocal exacerbation of sleep disturbance and BPD symptoms. Online cognitive behavioral therapy for insomnia (iCBT-I) has shown promise for treating insomnia in BPD patients. This study evaluated the key determinants, underlying processes, and change mechanisms of successful implementation of guided iCBT-I in BPD patients. METHODS: In the context of our clinical trial, we conducted this mixed-method process evaluation in 30 patients with BPD and comorbid insomnia

Quantification of ventilatory control in sleep apnea: from physiological insight to computable loop gain.

Sleep-disordered breathing reflects the interplay of upper-airway collapsibility, sleep depth/arousability, and instability in ventilatory control. Ventilatory loop gain (LG) quantifies the latter as the ratio of ventilatory response to disturbance: values >1 indicate self-sustaining oscillations, whereas lower values denote stable control. Despite its clinical relevance, LG measurement remains largely confined to research laboratories because conventional protocols (e.g. controlled gas challenges, stepwise reductions in positive airway pressure) are invasive and technically demanding. Recent

Gender differences in obstructive sleep apnea and treatment implications.

Obstructive sleep apnea (OSA) is a common cause of daytime sleepiness for millions of Americans. It is also a disease associated with an increased likelihood of hypertension, cardiovascular disease, stroke, daytime sleepiness, motor vehicle accidents, and diminished quality of life. A number of population-based studies have shown that OSA is more common in men than in women and this discrepancy is often evident in the clinical setting. There are a number of pathophysiological differences to suggest why men are more prone to the disease than women. Although the exact mechanisms are unknown, dif

Propriospinal myoclonus upon relaxation and drowsiness: a cause of severe insomnia.

Three patients, aged 71, 50, and 41 years, presented with myoclonic activity arising in the relaxation period preceding sleep onset and causing severe insomnia. Polygraphic studies showed that the myoclonic activity began in spinally innervated muscles, propagating at low speed to rostral and caudal muscular segments. Myoclonus arose whenever patients relaxed mentally and showed diffuse electroencephalographic alpha activity and was independent of posture. The jerks disappeared during sleep. Clonazepam afforded symptomatic improvement. Propriospinal myoclonus arising from a spinal generator ma

[Therapy of Cheyne-Stokes respiration with nocturnal oxygen therapy].

Cheyne-Stokes respiration is common in patients with severe congestive heart failure and is associated with significant nocturnal oxygen desaturation and sleep disruption. The pathogenesis of Cheyne-Stokes respiration in patients with congestive heart failure has been well described and is related to prolonged circulation time between the lung and the carotid body mainly due to increased cardiac dimensions, reduced body stores of oxygen and carbon dioxide, disturbance of ventilation and respiratory control due to arousals and a relatively high hypercapnic ventilatory response. Oxygen is likely

Expectations about arousal and nocturnal panic.

Expectations about arousal were examined in relation to nocturnal panic (NP). Eighteen panic disorder patients suffering from NP attacks and 18 control individuals were assigned randomly to conditions in which they were informed that audio feedback signals reflected heightened arousal that was either (a) expected and harmless or (b) unexpected. Participants relaxed and slept for 45 to 60 min, followed by presentation of periodic audio feedback signals. Physiological recording was continuous while subjective measures were collected at completion of the signals phase. NP patients in the unexpect

Sleep apnea syndrome symptoms and automobile driving in a general population.

Automobile accidents are reported as being overrepresented in those suffering from the obstructive sleep apnea syndrome (SAS), evident by snoring, sleep disturbances and diurnal hypersomnia. An estimation of the prevalence of these symptoms amongst an adult population, predominantly automobile drivers, was assessed by using a one-stage questionnaire procedure. From a national random sample of 1214 persons a weighted reply rate of 76% was achieved. Snoring, breath cessations, mid-sleep awakenings, and diurnal hypersomnia were reported in 24, 3.8, 27 and 9.1%, respectively. The maximum prevalenc

Impacts of shift work on sleep and circadian rhythms.

Shift work comprises work schedules that extend beyond the typical "nine-to-five" workday, wherein schedules often comprise early work start, compressed work weeks with 12-hour shifts, and night work. According to recent American and European surveys, between 15 and 30% of adult workers are engaged in some type of shift work, with 19% of the European population reportedly working at least 2 hours between 22:00 and 05:00. The 2005 International Classification of Sleep Disorders estimates that a shift work sleep disorder can be found in 2-5% of workers. This disorder is characterized by excessiv

Measurement of P300 and sleep characteristics in patients with hypersomnia: do P300 latencies, P300 amplitudes, and multiple sleep latency and maintenance of wakefulness tests measure different factors?

To explore further the relationship between ease of falling asleep, ability to maintain wakefulness, attention and information processing in sleep apnea and other sleep disorders, we conducted a thorough analysis of the similarities, differences and correlations between auditory and visual P300 amplitudes and latencies, and tests of sleepiness. The 283 consecutive patients presenting with hypersomnia were administered nocturnal polysomnography. Next day they underwent auditory and visual P300 recordings, Multiple Sleep Latency Test (MSLT) and Maintenance of Wakefulness Test (MWT). Correlation
