[Collaboration in the first-line setting in medically unexplained symptoms: from powerlessness to coordination].

Two women aged 19 and 37 consulted their general practitioners (GPs) for medically unexplained symptoms (MUS). Patient A suffered sudden pain in her legs. She needed crutches, but no somatic explanation could be found. After systematically exploring the history (somatic, emotional and social aspects, thoughts, conduct), the GP initiated a proactive, multidisciplinary treatment regimen. He worked with a mental health nurse practitioner and focused the treatment regimen on the effects of the symptoms. The patient's pain gradually decreased. Patient B had multiple functional complaints. She had l

Effectiveness and safety of milnacipran treatment for a patient with delusional disorder, somatic type taking multiple medications for concomitant physical diseases.

A 73-year-old male patient with delusional disorder, somatic type (DDST) who had a delusion of skin infestation with insects was presented. He was taking multiple medications to treat concomitant physical diseases. Milnacipran was selected as the treatment drug because of its low drug-interaction profile. Milnacipran treatment at the doses of 25 to 50 mg/d decreased the infestation delusion significantly, without an adverse effect or event suggestive of a drug interaction. The effectiveness of the drug was confirmed coincidentally by repeated recurrences and improvements after his deliberate d

Somatization disorder, a cause of difficult pain: a case report.

Presented is a case report of a 55-year-old man with a suspected diagnosis of pancreatic malignancy who was referred for pain management to the pain and palliative care clinic. A diagnosis of pain originating from a malignancy was later ruled out. The patient was referred for psychiatric evaluation, where his pain symptoms were confirmed to be somatoform in origin. This case highlights the importance of ruling out somatization as the cause of symptoms in difficult pain patients.

The relationship between anxiety sensitivity and fear of pain in healthy adolescents.

The present study investigated the relationship between anxiety sensitivity and fear of pain in a large group of healthy adolescents (N=200). Participants completed the childhood anxiety sensitivity index for children-revised, a questionnaire measuring four specific domains of anxiety sensitivity: fear of cardiovascular symptoms, fear of respiratory symptoms, fear of cognitive dyscontrol, and fear of publicly observable anxiety symptoms, and a simplified version of the pain anxiety symptoms scale, a self-report instrument assessing pain-related anxiety and avoidance (i.e. fear of pain). In lin

Measurement properties of the Center for Epidemiologic Studies-Depression Scale in a homeless population.

The measurement properties of the Center for Epidemiologic Studies--Depression Scale (CES-D; L. S. Radloff, 1977) were evaluated in a probability sample of homeless adults residing in a large and demographically diverse community. The findings from this investigation suggest that the CES-D is a reliable measure of depressive symptoms among homeless adults and that the factor structure of the scale replicates the factor structure found in the general population. Change in the CES-D scores was associated with change in residential status, with participants who had made a transition from homeless

Depression, hassles and somatic symptoms in mandibular dysfunction syndrome patients.

Thirty-seven patients with mandibular dysfunction syndrome (MDS) and 30 age-, sex- and education-matched dental outpatient controls were assessed using the Beck Depression Inventory, the Hassles Scale and a symptom checklist derived from the DSM-III criteria for somatization disorder. MDS patients had higher levels of depressive symptoms than control patients (P less than 0.05), though only five out of 37 (13.5 per cent) of the MDS patients had depression intensity levels suggesting clinically significant psychopathology. No significant differences were found between MDS patients and controls

An office mental status examination for complex chronic dissociative symptoms and multiple personality disorder.

Chronic complex dissociative symptoms can be readily inquired about in the diagnostic interview leading to a clinical diagnosis of MPD in many cases. It is most useful to begin with inquiry about amnesia, autohypnotic, posttraumatic, pseudopsychotic, and passive-influence symptoms, and childhood abuse or traumatization. As this proceeds, overt dissociation is commonly noted including spontaneous trances, age-regression, blending or overlap of states, or frank switching. When this occurs, the interviewer can pursue more detailed information about the patient's experiences. Tracking these proces

Psychiatric illness and irritable bowel syndrome: a comparison with inflammatory bowel disease.

Structured psychiatric interviews and psychological self-report measures were administered to 28 patients with irritable bowel syndrome and 19 patients with inflammatory bowel disease. Significantly more of the patients with irritable bowel syndrome had lifetime diagnoses of major depression, somatization disorder, generalized anxiety disorder, panic disorder, and phobic disorder. They had significantly more medically unexplained somatic symptoms, and most had suffered from psychiatric disorders, particularly anxiety disorders, before the onset of their irritable bowel symptoms.

Body image disturbance in body dysmorphic disorder and eating disorders. Obsessions or delusions?

At this time, the question posed by this article's title--body image disturbance in body dysmorphic disorder and eating disorders: obsessions or delusions?--is probably best answered "both." Both disorders appear to be characterized by obsessional and delusional thinking. In addition, it is likely that their nondelusional and delusional variants constitute a single disorder encompassing a spectrum of insight, with the entire spectrum characterized by obsessional thinking. This view represents a considerable departure from DSM-III-R, in which the psychotic disorders were encapsulated in a separ

[Mental disorders induced by experiences of being bullied].

The author investigated 25 adolescent patients (9 males, 16 females) whose mental disorders seemed to be induced by their experiences of being bullied at school. The patients were treated by the author in the outpatient clinic or the inpatient ward of National Defense Medical College Hospital during 10 years, 1978 through 1988. These patients were classified into 4 groups from the viewpoint of their clinical symptoms and signs. The author called each group as follows; 1) paranoid-hallucinatory group, 2) withdrawal group, 3) escaping-into-fantasy group, and 4) somatoform group. The paranoid-hal

Prevalence of and Factors Associated With Post-Coronavirus Disease 2019 (COVID-19) Condition in the 12 Months After the Diagnosis of COVID-19 in Adults Followed in General Practices in Germany.

BACKGROUND: Little is known about the epidemiology of post-coronavirus disease 2019 (COVID-19) condition (PCC) in primary care practices. Th … f PCC with asthma (OR = 1.38), reaction to severe stress, and adjustment disorders (OR = 1.24), and somatoform disorders (OR = 1.23). CONCLUSIONS: Post-COVID-19 condition was found in the 12 months after the diagnosis of COVID-19 in approximately 8% of adults from general practices in Germany. More data from other settings are warranted to confirm these findings.

Section 377 and The Mental Healthcare Act, 2017: Breaking barriers.

Internationally, psychiatric work capacity assessments increasingly emphasise functioning, participation, contextual factors, and return-to-work processes rather than diagnosis alone. Nevertheless, substantial inter-rater variability persists in routine clinical practice, particularly in short-term sickness benefit evaluations, in which clinicians must balance therapeutic protection, proportionality, prevention of chronicity, and occupational reintegration. Since 2015, Swiss Federal Supreme Court case law has progressively established a structured medico-legal framework for evaluating psychiat

A comparison of schemas, schema modes and childhood traumas in obsessive-compulsive disorder, chronic pain disorder and eating disorders.

BACKGROUND: In this study, we investigated early maladaptive schemas (EMS), schema modes and childhood traumas in patients suffering from obsessive-compulsive disorder (OCD) in contrast to patients with other Axis I disorders. Based on cognitive theories on OCD, our main research question was whether schemas belonging to the domain of 'impaired autonomy and performance' are more prevalent in OCD than in both eating disorders (ED) and chronic pain disorder (CPD). SAMPLING AND METHODS: EMS, schema modes and traumatic childhood experiences were measured in 60 patients with OCD, 41 with ED, 40 wi

Phenomenology of psychogenic movement disorders in children.

Psychogenic movement disorders are heterogeneous and diagnostically challenging. Despite the growing literature on adult forms, clinical features in children have received relatively little attention. We retrospectively reviewed medical records and video of patients <18 years diagnosed with a psychogenic movement disorder at our institute between 2007 and 2010. We identified 14 patients (6 males and 8 females) with a mean onset age of 11.5 years. Levels of diagnostic confidence were documented (2 patients), clinically established (8 patients), and probable (4 patients). A single movement disor

The Epworth sleepiness scale: relationship to sleep and mental disorders in a sleep clinic.

BACKGROUND AND PURPOSE: Both sleep disorders and mental disorders are associated with excessive daytime sleepiness as measured by the Epworth sleepiness scale (ESS). We sought to assess the relationship between the ESS scores and mental disorders in a sleep clinic. METHODS: One hundred sixty-five consecutive new referrals to a sleep clinic completed the ESS prior to their initial visit. Mental disorders (major depression, minor depression, anxiety not otherwise specified, and panic disorder) were diagnosed with the primary care evaluation of mental disorders (PRIME-MD) questionnaire, a reliab
