Triplication of the gallbladder is a rare congenital anomaly of the biliary tract; there are only nine reported cases to date. We report a case in which laparoscopic cholecystectomy was performed in a patient with biliary colic and choledocholithiasis. Preoperative assessment with ultrasound and endoscopic retrograde cholangiopancreatography (ERCP) failed to reveal the eventual findings of a triple gallbladder. Successful excision of the triple gallbladder was carried out laparoscopically, and the final diagnosis was confirmed by the pathologist. The patient made an uneventful postoperative re

78 patients with chronic anal fissures have been mainly operated on by lateral internal sphincterotomy (LATS). Continence have been evaluated by questionnaire at least 9 months postoperatively. Patient without any additional proctological operation had minor disturbances of continence in 17%. Patient with additional operations had disturbances of continence in 30%. Especially the subgroup of patients with LATS and haemorrhoidectomy had bad results. In this group only 45% were fully continent.

Five patients with choledocholithiasis refractory to standard extraction methods after endoscopic sphincterotomy underwent peroral cholangioscopy with the "mother-daughter" endoscopy system. electrohydraulic lithotripsy of nine stones was performed successfully and without complication, other than minor transient bleeding in one patient. The outer sheath of the "daughter scope" sustained a linear burn in one instance. This method of stone fragmentation is a useful adjunct to achieve ductal clearance in difficult cases.

We performed a laparoscopic cholecystectomy in 300 patients with gallstone disease complicated or not. The rate success was 97%, however involved the pathology might be. Indeed, 15% of the patients had an acute cholecystitis (10% catarrhalis, 5% empyema). Twenty-four patients with common bile duct stones had a complete endoscopic management of the biliary disease: endoscopic sphincterotomy and laparoscopic cholecystectomy. Operative mortality was absent and overall morbidity was 3.7%, exclusively minor.

Cholangitis and pancreatitis have resulted from migration of Ascaris lumbricoides up the biliary tree. We report our experience with the endoscopic management of 11 patients who presented with cholangitis and pancreatitis. Successful endoscopic worm extraction with or without sphincterotomy was achieved for worms located in the biliary tree but endoscopic worm extraction from the pancreas was technically difficult. Anthelminthic therapy is indicated to eradicate the gut infestation and prevent recurrent disease.

We herein report two cases of hemorrhagic ampullary lesions in which endoscopic papillotomy was performed to control bleeding and resulted in successful treatment. Both patients were pathologically diagnosed with an underlying pathology characterized by inflammatory cell infiltration and capillary proliferation. They also had disposing factors for bleeding, such as antithrombotic therapy and idiopathic thrombocytopenic purpura. Endoscopic treatment was selected because the risk of surgical resection was high due to the patients' hemorrhagic condition. Both patients were successfully treated wi

PURPOSE OF REVIEW: To summarize recent data on techniques, efficacy and complications of endoscopic management of chronic pancreatitis.
RECENT FINDINGS: Extracorporeal shock wave lithotripsy with or without endoscopic retrograde cholangiopancreatography is the first-line treatment for large painful obstructive pancreatic duct calculi. Use of preextracorporeal shock wave lithotripsy secretin could result in better stone clearance. The first-line treatment for dominant pancreatic duct strictures is placement of a single 10-Fr polyethylene stent with planned exchanges every 3 months until 1 year

Up to 20 percent of the U.S. population may have cholelithiasis, but most of these persons remain asymptomatic. If symptoms or complications develop, several potential management strategies may be used. Open and laparoscopic cholecystectomy are definitive surgical treatments. Nondefinitive methods, such as lithotripsy, indirect dissolution, direct dissolution, sphincterotomy and drainage, have lower success rates but may be indicated in specific cases. Although laparoscopic cholecystectomy is a relatively new technique, its morbidity and mortality rates are comparable to those of open cholecys…

Sphincter of Oddi activity partly regulates bile flow into the small intestine. This regulation is mainly controlled by phasic contractions and basal tone of the sphincter, together with gallbladder contraction. Manometric studies of the sphincter have permitted a better understanding of its physiological role and implication in biliary dyskinesia symptoms. Motility abnormalities of Oddi's sphincter present classically as bouts of recurrent pain and/or idiopathic pancreatitis, that can be successfully cured by endoscopic sphincterotomy.

Recent advances in the diagnosis and surgical management of cases involving the extrahepatic bile ducts are illustrated. Particular stress is laid on the universally recognised importance of preoperative cholangiography, the usefulness of the flexible choledochoscope, and the employment of cold light to visualise calculi undetected by cholangiography. The scope of and indications for direct surgery on the papilla, derivative operations, and reconstruction of the duct are discussed. Lastly, reference is made to the use of endoscopic sphincterotomy, and the suitability of continuous perfusion of…

A 67-year-old man presented with signs and symptoms of acute cholangitis following recent endoscopic evaluation at an outside facility for upper gastrointestinal bleed requiring hemostatic clip placement in the duodenum. Endoscopic retrograde cholangiopancreatography showed a hemostatic clip placed at the major papillary orifice, resulting in mechanical obstruction. The hemostatic clip was removed, and a biliary sphincterotomy was performed with subsequent biliary stent placement with successful alleviation of the patient's biliary obstruction.

Pancreas divisum is the most common congenital malformation of the pancreas. Sometimes it is considered an etiology when patients present with recurrent episodes of pancreatitis. Endoscopic retrograde pancreatography (ERP) with selective cannulation of the minor papilla with sphincterotomy and stent placement are performed in these patients. Proximal migration of pancreatic stents in pancreas divisum is rare and challenging to manage. We describe a case of proximal migration of a pancreatic stent in a patient with pancreas divisum and perform a review of literature.

The choledochocele is a cystic dilatation of the intraduodenal portion of the bile duct and corresponds to the type III biliary cysts in the Todani ´s classification. Eventhough the majority of patients remain asymptomatic they can be an atypical cause of abdominal pain or relapsing acute pancreatitis events. The risk of malignancy is lower than other choledochal cyst (<2,5%). The treatment is based on surgical or new endoscopic techniques of resection. In some of the cases an endoscopic sphincterotomy is the first approach.

Pancreas divisum is the most frequent congenital disease of the pancreas. It results from the failure of the dorsal and ventral pancreatic ducts to fuse. It is responsible primarily for recurrent acute pancreatitis, chronic abdominal pain, and chronic pancreatitis. It is diagnosed today by MRI. Treatment is recommended only for symptomatic patients. Endoscopic sphincterotomy is the main treatment, and its results are good for most patients with recurrent acute pancreatitis. Surgical sphincteroplasty is indicated if endoscopic treatment fails.

Pancreas divisum, partial or non fusion of the dorsal and ventral pancreatic ductal system, affects up to 10 percent of the population. Approximately 25 percent of patients with pancreas divisum will develop complications such as recurrent pancreatitis. Surgical and endoscopic therapy usually include minor papilla sphincterotomy or sphincteroplasty to facilitate drainage of the dorsal ductal system. The following case represents an unusual complication of the pancreas divisum with primarily ventral duct disease and pancreaticolithiasis.

According to our experience, fissurectomy is the method of choice in treating chronic anal fissure. The secondary lesions and the cicatricial distortions are removed. Sphincterotomy is not carried out. We treated 534 patients. Of these, 470 patients underwent surgery. In 3.1% of those patients in whom the surgical scars had healed, fecal spotting occurred postoperatively. There were no further continence disorders. The overall result must be regarded as good compared to reports in the literature.

Chronic anal fissure is a common condition usually treated by maximal anal dilatation or lateral subcutaneous sphincterotomy. The following method is based on a common surgical principal; to widen a stenosed tube a longitudinal incision is made across the stenosed area and this is sutured transversely as in pyloroplasty. This method of anoplasty is simple and appears to have no resultant complications. Further studies are necessary before its value can be established compared to the presently accepted methods of treatment.
