Isolated avulsion fracture at the plantar lateral base of the first metatarsal: a case report.

We present a case of isolated avulsion fracture at the plantar lateral base of the first metatarsal without injuries of the tarsometatarsal joint. Few case reports can be found in the literature and it has been reported only as a part of the tarsometatarsal joint injuries. The basis for the diagnosis is discussed, as well as the nature of acute rupture of the peroneus longus tendon.

Repair of Acute Patellar Tendon Rupture Using an Internal Brace Technique.

Acute patellar tendon rupture is a serious injury, resulting in the disruption of the knee extensor mechanism. Many authors recommend augmented repairs of patellar tendon ruptures to allow early active rehabilitation. An internal brace technique, which is a ligament augmentation using high-strength suture tape and knotless anchors, has been used as augmentation for the primary tendon or ligament injury. A case of acute patellar tendon rupture in a Judo player, who was successfully treated with primary repair and augmentation using an internal brace technique, is presented. In this case, the pa

[Extensor pollicis longus rupture in a pediatric patient: case report and a literature review].

INTRODUCTION: the rupture of the extensor pollicis longus (EPL) tendon is a rare pathology and usually occurs in adult women in relation to distal radius fractures. MATERIAL AND METHODS: we present the case of an adolescent female patient who, after conservative treatment with splinting of a Peterson type I physeal fracture of the radius, suffered an acute extension deficit of the thumb at six weeks, diagnosed with clinical radiological examination as spontaneous rupture of the extensor pollicis longus (EPL). She was treated with extensor pollicis indicis propius (EIP) transfer with satisfact

Reconstruction of Chronic Triceps Brachii Tendon Rupture Using Semitendinosus Autograft Surgical Technique.

Rupture of the triceps tendon is a rare event, and the care could be often problematic for orthopedic surgeons. Cases of triceps tendon re-rupture are even rarer. The stump is often retracted, atrophic, and the tissue quality is poor. Several surgical techniques have been reported. We present our surgi- cal reconstruction using free semitendinosus (ST) autograft.

Posterior Tibial Tendon Dislocation: A Case Report.

Isolated posterior tibial tendon dislocation is a rare condition. Diagnosis can be challenging, especially in the acute clinical setting. Predisposing factors include shallow posterior tibial tendon groove and tear of the flexor retinaculum. We report the case of traumatic subluxation of the posterior tibial tendon, illustrating imaging findings and surgical technique. Posterior tibial tendon dislocation was detected using dynamic ultrasound, while magnetic resonance was essential for a comprehensive evaluation that included underlying predisposing anatomic abnormalities and associated patholo

Postoperative hand therapy management of zones V and VI extensor tendon repairs of the fingers: An international inquiry of current practice.

STUDY DESIGN: Electronic Web-based survey. INTRODUCTION: Evidence supports early motion over immobilization for postoperative extensor tendon repair management. Various early motion programs and orthoses are used, with no single approach recognized as superior. It remains unknown if and how early motion is used by hand therapists worldwide. PURPOSE OF THE STUDY: The purpose of this study was to determine if there is a preferred approach and identify practice patterns for constituents of International Federation of Societies for Hand Therapy full-member countries. METHODS: Participation in t

Successful reconstruction of distal peroneus longus tendon dislocation associated with a split lesion - a case report.

BACKGROUND: Peroneal tendon injuries are one of the differential diagnoses in lateral ankle and rearfoot pain. While partial tears are not uncommon, peroneal tendon dislocation at the peroneal tubercle is very rare. Until now, only three papers have been published, presenting five cases of peroneus longus tendon dislocation over the peroneal tubercle. This report adds a previously undescribed case of a peroneus longus tendon split tear that was partially dislocated and entrapped over the peroneal tubercle. The respective operative approach and the outcome are described. CASE PRESENTATION: A 2…

Maximizing Return to Sports After Achilles Tendon Rupture in Athletes.

Achilles tendon ruptures are devastating injuries to athletes, with return-to-sports rates around 70% and some risk for diminished performance post-injury. Surgical management in athletes is often favored for a number of reasons, although evidence guiding the optimal treatment is limited. Functional rehabilitation has been supported as a key component of operative and nonoperative treatment plans. Return-to-play protocols in the literature are sparse and varied due to often ambiguous definitions of what it means to return to sport and a lack of explicit criteria. Optimal sport-specific return-

Ultrasound visualization of central slip injuries of the finger extensor mechanism.

PURPOSE: To evaluate the ability of ultrasound to detect a central tear of the finger extensor tendon mechanism. METHODS: Twelve fresh-frozen and thawed cadaver fingers were used. All fingers had a midradial incision with exposure of the extensor mechanism over the proximal interphalangeal joint. Six fingers had a sharp transection of the central slip. The other 6 fingers were used as sham dissection controls. All fingers had the exposure closed by suture and then were examined by a musculoskeletal ultrasonographer. RESULTS: All 12 fingers were identified correctly as either sham controls wi

Ultrasound-guided intralesional tendon and ligament injections in the horse.

OBJECTIVE: To describe principles of ultrasound-guided injection techniques for intralesional delivery of regenerative orthobiologics into equine tendon and ligament core lesions. ANIMALS: A university-owned horse and equine cadaver forelimbs. METHODS: A reproducible protocol for ultrasound-guided injection is demonstrated with the superficial digital flexor tendon used as an example. Following diagnosis of the lesion, the horse should be sedated and the affected limb desensitized with perineural anesthesia. The lesion is identified ultrasonographically in transverse and longitudinal planes,

Terminal bifurcation of the biceps brachii muscle and tendon: anatomic considerations and clinical implications.

OBJECTIVE: The objective of our study was to describe the anatomic variation of a bifurcated distal biceps tendon with MRI, histology, and dissection in cadavers and to report the MR appearance of superimposed lesions in a patient population with this anatomic variant. MATERIALS AND METHODS: Visual and histologic examinations of the distal biceps brachii tendon in eight sectioned fresh-frozen elbow specimens were performed. Dissection of 17 elbow specimens was performed to describe the distal biceps brachii tendon. In addition, all elbow MRI reports over a 3-year period (n = 411) were retrosp

Latissimus dorsi tendon transfer for irreparable tears of the rotator cuff: An anatomical study to assess the neurovascular hazards and ways of improving tendon excursion.

Latissimus dorsi tendon transfer (LDTT) is technically challenging. In order to clarify the local structural anatomy, we undertook a morphometric study using six complete cadavers (12 shoulders). Measurements were made from the tendon to the nearby neurovascular structures with the arm in two positions: flexed and internally rotated, and adducted in neutral rotation. The tendon was then transferred and measurements were taken from the edge of the tendon to a reference point on the humeral head in order to assess the effect of a novel two-stage release on the excursion of the tendon. With the s

Surgical anatomy for reinsertion of the distal tendon of the biceps brachii muscle.

Ruptures of the distal tendon of the biceps brachii muscle usually require surgical treat-ment to restore strong elbow flexion and forearm supination. However, the surgical procedure carries a risk of injury to neurovascular structures adjacent to the insertional tendon, and its success relies on respecting the original anatomical relations during reconstruction of the tendon. The aim of this article is to present a compendious review of relevant anatomy and practical notes which may enhance the optimal functional outcomes. Structured discussion on morphological aspects of the insertional tend

Ultrastructure of tendon rupture depends on strain rate and tendon type.

Previous research has shown that both the mechanics and elongation mechanisms of tendon and ligament vary with strain rate during tensile loading. In this study, we sought to determine if the ultrastructural damage created during tendon rupture also varies with strain rate. A bovine forelimb model was used, allowing two anatomically proximate but physiologically distinct tendons to be studies: the positional common digital extensor tendon, and the energy storing superficial digital flexor tendon. Samples from the two tendon types were ruptured at rates of either 1%/s or 10%/s. Relative to unru

Commentary on the clinical diagnosis of Achilles tendon rupture.

BACKGROUND Xanthogranulomatous cholecystitis (XGC) is a rare benign gallbladder disease with an incidence rate ranging from 3.3% to 8.8%. XGC is prone to be misdiagnosed as gallbladder cancer when inflammatory lesions spread into the gallbladder wall and adjacent tissue. This case report explores the clinical and radiological features, surgical outcomes, and treatment strategies for extensive XGC lesions. CASE REPORT A 72-year-old woman had a 20-year history of intermittent recurrent abdominal pain and fever, which had worsened over the last 3 months. Physical examination revealed a 16×10 cm p
