Reduction of missed posterior dislocation of the shoulder. Report of 2 cases, 1 of them bilateral.

We report 2 cases of missed posterior dislocation of the shoulder, 1 of them bilateral. Prominence of the acromion and coracoid, posterior bulging and complete elimination of external rotation of the humerus lead to the diagnosis which was established by an axillary radiographic view. All 3 shoulders were treated surgically according to Mc Laughlin. At 3-4 years follow-up the function was good and there were no signs of arthrosis or avascular necrosis of the head.

Independent index extension after extensor indicis proprius transfer.

The extensor indicis proprius was used to restore mobility to a variety of hand movements in 27 patients. Retained independent index finger extension was obtained in a majority of patients postoperatively. Potential postoperative complications of index extension lag and deviation may be avoided if the extensor indicis proprius is sectioned immediately proximal to the dorsal hood. A corresponding laboratory study was undertaken to better define the anatomic constraints limiting independent finger extension. Distinct differences between juncturae tendinum and extensor compartment musculature hel

Reconstruction of elbow flexion, wrist and finger extension by transposition of pedicled latissimus dorsi muscle and flexor carpi ulnaris muscle.

The technique of restoring extension of the hand and fingers is described in an inveterate injury of the brachial plexus. The insertion of m. flexor carpi ulnaris was transferred with an intact nervous and vascular supply to different tendons of the m. extensor digitorum closely above the retinaculum extensorum. The muscle strength was 4--using Janda's muscle test. Dorsal flexion of the hand was possible from the basic position, with extension of the fingers to their full extent. Anatomical investigation revealed that the mean length of the caput humerale m. flexor carpi ulnaris is 26.5 cm, th

Comparison of Staple, Anchor, and Tenodesis Screw for Posterior Tibialis Tendon Fixation: A Biomechanical Analysis.

Various posterior tibialis tendon fixation techniques are described in literature. Suture anchor, staple and tenodesis screws are widely used for posterior tibialis tendon transfer, but their stiffness and the maximal ultimate failure load were not tested before. We aimed to compare the initial ultimate failure load and stiffnesses of suture anchor, staple and tenodesis screws on bovine tendon fixation to bovine metaphyseal bone. Thirty-five fresh bovine ankle joints and hooves were obtained from a local abattoir. Metatarsals bones with long extensor tendons were harvested. Staple group had 15…

Chronic Achilles Ruptures: Reconstructive Options.

Achilles tendon ruptures, if neglected or identified late, lead to impairments in function and gait. Surgical reconstruction is typically required to restore the resting length and tension to the gastrocnemius-soleus complex. A variety of reconstructive options have been described, depending on several factors, including chronicity, residual gap size, remaining tissue quality and vascularity, location of tendon rupture or deficiency, and patient-specific factors. Despite the many surgical options described from local soft-tissue rearrangements and tendon transfers, to the use of allograft tiss…

Surgical Management of Spasticity of the Thumb and Fingers.

Spasticity of the hand profoundly limits an individual's independent ability to accomplish self-care and activities of daily living. Surgical procedures should be tailored to patients' needs and functional ability, and even patients with severe cognitive injuries and poor upper extremity function may benefit from surgery to improve appearance and hygiene. Careful preoperative examination and planning are needed, and consideration is given to the potential unintended detrimental effect of a surgical procedure on hand function.

Spontaneous Rupture of the Extensor Pollicis Longus with the Extensor Pollicis Brevis Deformity.

This paper examines three cases of spontaneous ruptures of the extensor pollicis longus (EPL) with the extensor pollicis brevis (EPB) deformity. The patients ranged from 39 to 71 years old. Non-dominant hands were involved in all three cases. There were no trauma nor pathological cause of EPL ruptures. A tendon transfer using the extensor indicis proprius was performed in each case. Intraoperative findings showed hypoplastic EPB in one case and EPB defects in two cases. A weak or defective EPB puts an excessive load on the EPL, which might be one of the causes of spontaneous EPL rupture.

Complex flexor and extensor tendon injuries.

Complex hand injuries are those that involve significant soft-tissue loss with variable exposure of bones and/or joints, lacerated tendons, and neurovascular structures. Management of these injuries is optimally accomplished through the restoration of thin, pliable, ideally sensate tissue with reliable vascularity and a gliding surface to facilitate restoration of motion after repair. Unfortunately, optimal restoration may require complex surgical reconstruction and/or staged surgical procedures. This article presents an overview of some of the more commonly used options available to the recon

Correction of Wassel type IV thumb duplication with zigzag deformity: results of a new method of flexor pollicis longus tendon relocation.

The purpose of this study is to present a treatment algorithm and a method of flexor pollicis longus tendon relocation for Wassel type IV thumb duplication with zigzag deformity. Forty-two thumbs in 42 patients were included in this study and the mean follow-up was 4 years. In addition to excision of the extra digit, tendon relocation and metacarpal and/or proximal phalangeal osteotomy were carried out, based on the degree of angulation at the metacarpophalangeal and interphalangeal joints. Tendon relocation was achieved using our pull-out suture technique. The results were assessed using an e

Reconstruction of the type IIIA hypoplastic thumb.

The management of the hypoplastic thumb depends on the degree of bony malformation, soft tissue anomaly, and deficiencies. Type IIIA thumbs have thumb-index web-space narrowing, hypoplasia of the thenar musculature, metacarpophalangeal joint instability, and extrinsic tendon abnormalities. They also have a hypoplastic metacarpal with a stable carpometacarpal joint, differentiating them from type IIIB thumbs which have an unstable carpometacarpal joint due to partial metacarpal aplasia. Whereas type IIIB thumbs are best treated with ablation and index pollicization, type IIIA thumbs are amenabl

Limb salvage with Chopart's amputation and tendon balancing.

For several decades, Chopart's amputation has met with some skepticism owing to reports of significant equinus deformity developing soon after the procedure is performed. However, with appropriate tendon balancing, which generally includes anterior tibial tendon transfer and tendo Achillis lengthening, this level of amputation is often more functional than slightly more distal amputations, such as Lisfranc or short transmetatarsal amputations. The authors offer a rationale for this observation, which includes a discussion of the longitudinal and transverse arch concept of the foot. This concep…

Clinical presentation of congenital convex pes valgus.

Congenital convex pes valgus, frequently termed vertical talus in past descriptions, is a malformed foot that requires early diagnosis and treatment. A brief review of clinical features and a case presentation are provided to enhance the understanding of this complex deformity. A successful treatment regimen for infants and children is described, involving manipulation and early surgical correction.

Transposition of the tendo calcaneus for post-traumatic bone defects of the tibia.

Certain post-traumatic defects of the lower extremity present difficult problems in therapy and current methods of management do not offer satisfactory solutions. One such defect is an ulcerative lesion of the lower limb eroding the tibia and forming a chronic, rigid-walled cavity. We describe an operation in which the bone cavity is filled by viable tissue which acts as a bed for a skin graft. In chronic stasis ulcers it has been established that the tendo calcaneus will accept a skin graft, provided the paratenon is left undisturbred.

Ruptured Extensor Pollicis Longus Tendon After a Nondisplaced Distal Radius Fracture in a Young Adult Soccer Player.

Forearm fractures of the distal radius are one of the most common fractures seen in the upper extremity, and they represent approximately 1/6 of fractures treated in the emergency department. Forearm fractures are associated with a rare but known complication of a delayed ruptured extensor pollicis longus (EPL) tendon. This sequela is more commonly seen in adults after a nondisplaced distal radius fracture, with much variability in the incidence ranging from 0.07% to 5%. By contrast, this complication in the pediatric population is almost exclusively seen after a displaced or unstable fracture

Transposition of the Extensor Indicis Proprius (EIP) for inveterate post-traumatic rupture of the Extensor Pollicis Longus (EPL) of the hand. 12 clinical cases.

BACKGROUND AND AIM OF WORK: Subcutaneous tendon rupture of the Extensor Pollicis Longus (EPL) is one of the most frequent injuries of the extensor tendons of the hand. In this paper, we report our experience on 12 cases of atraumatic subcutaneous EPL lesions treated in our hospital with Extensor Indicis Proprius (EIP) transposition. METHODS: This observational study was conducted between January 2015 and December 2018 in the Casa di Cura "Prof. Nobili", Castiglione dei Pepoli (Bologna). Twelve patients were included in the study, nine of whom were females and three were males, with an average

The enigmatic diagnosis of posterior tibialis tendon rupture.

Posterior tibialis tendon rupture is a diagnosis that is often missed. This is thought to be secondary to nonspecific clinical findings and the lack of any laboratory or radiographic test to reliably confirm the diagnosis. We report sixteen cases of surgically confirmed posterior tibialis tendon rupture. Based on our review of these patients, the diagnosis of posterior tibialis tendon rupture should be strongly suspected in the adult patient presenting with a history of a twisting ankle injury and generalized medial ankle pain and swelling. A flexible, asymmetric pes planus and forefoot pronat

[Extensor indicis muscle as a replacement for the abductor pollicis longus muscle. A case report].

The abductor pollicis longus muscle is very important for the function of the thumb. Its deficit hinders grasping bigger objects. Therefore, the reconstruction of the APL tendon is important in chronic injuries. The interposition of a tendon graft is considered the best method. However, the muscle loses its contractility very quickly because of its small gliding amplitude. Consequently, this procedure is only usable in the first six to eight weeks after injury. Later the function of the abductor pollicis longus muscle can only be recovered by a tendon transposition. The described tendon transp

Krogius tenoplasty for recurrent dislocation of the patella. Failure associated with joint laxity.

Thirty-three patients treated with a Krogius tenoplasty for recurrent dislocation of the patella have been followed for an average of 3 years; the knees were classified according to Rünow. In 14 of 34 operated knees dislocations had recurred. Most of the recurrences were found in patients with generalized joint laxity, whereas only one of nine knees with only a high Insall index had redislocated. Patients with generalized joint laxity should not be treated with the Krogius tenoplasty as the sole procedure.
