BACKGROUND: Hyperparathyroidism (HPT) is common in chronic kidney disease and may persist after kidney transplantation, but its prognostic significance remains uncertain because of heterogeneous definitions and variable timing of assessment.
METHODS: We performed a systematic review and meta-analysis of observational studies in adult kidney transplant recipients (KTRs) in accordance with PRISMA 2020 (PROSPERO CRD420261293712). PubMed/MEDLINE and Embase were searched from inception to 9 February 2026. Exposures included pre- and post-transplant parathyroid hormone (PTH)/Persistent HPT assessed

PURPOSE OF REVIEW: There have been no well defined guidelines to determine whether a kidney transplant should be offered to liver transplant candidates who have chronic kidney disease (CKD) or prolonged acute kidney injury while awaiting a liver transplant. This article provides a review of current literature on risk factors for CKD progression after liver transplantation alone (LTA) in patients with pretransplant renal dysfunction and the utility of cystatin C (Cyst C) to assess renal function in cirrhotic patients. Studies evaluating risk factors for transplant futility are also discussed. B

Detailed results of 12 combined pancreas and kidney transplantations (Comb) were compared with those of two matched diabetic controls per patient--one living donor kidney recipient (LD) and one cadaveric donor kidney recipient (CD--who, though eligible for pancreas transplantation also, preferred kidney transplantation only. Mean follow-up was 22, 23, and 21 months in the three groups. There was no mortality in the LD group, but two CD and one Comb patient died from cardiovascular disease. Two kidneys were lost in both the Comb and the LD group, compared to five in the CD group. Ten major vasc

Simultaneous pancreas-kidney transplantation (SPK) is the only established therapy of type 1 diabetes mellitus with diabetic nephropathy and preterminal/terminal kidney failure. Despite a high demand, with 551 patients on the waiting list in Germany in 2022, only about 42 pancreas transplants were performed in that year. As a result, with waiting times of 2-3 years, there is a significant number of deaths among patients on the waiting list. In addition to SPK, isolated pancreas transplantation (IPT) and pancreas transplantation after kidney transplantation (PAK) are only considered for specifi…

Transplantation is both a cause of, and a therapy for, diabetes. Solid organ transplantation requires immunosuppressive regimens that freque … severe hypoglycaemia unawareness. Both procedures may be done in association with other solid organ transplants, most commonly with kidney transplantation. They can induce insulin independence and improve allograft survival. Recently, stem cell-derived therapy for T1D has been shown to be possible in humans and is likely to become an important therapy for peo …

Airway stenosis and malignant tumors are the 2 main reasons to perform a surgical resection of part of the trachea. Tracheal lesions with a length shorter than 5 cm can be resected and primary reconstruction can be safely effected. Excessive anastomotic tension should be avoided because it may lead to restenosis or dehiscence at the anastomosis. Reconstruction of a longer defect may be attempted with a tracheal allotransplant or with autologous tissue.

The progressive rise in the number of kidney transplant recipients in the last 2 decades is reflective of the technological advances in the field. Nephrologists are responsible for providing long-term longitudinal care to these patients. It is pertinent that nephrologists understand the various nuances of aspects such as immunosuppression, opportunistic infections, and identification of causes associated with graft dysfunction.

Spermatogonial transplantation is the unequivocal method to detect spermatogonial stem cells (SSCs) based strictly on the functional definition of stem cells - the cells' regenerative capacity. This method further allows for SSC quantification. A weakness of spermatogonial transplantation is its time-consuming nature; it takes 2 months to confirm the production of terminally differentiated cells in spermatogenesis, spermatozoa, in mice, which gives the assay endpoint. Using the mouse as the model system, we here describe the basic techniques of spermatogonial transplantation and provide practi

PURPOSE OF REVIEW: Adequately managing a complex medical regimen is paramount to the success of organ transplants. When patients stray from their prescribed medical regimen posttransplant, graft rejection, and death can occur. Predictors of adherence have been studied for many years, and various factors have been identified as contributing to adequate or poor adherence. Both demographic and personal characteristics have been associated with adherence behavior. However, recent developments, such as the COVID-19 pandemic, increased use of mobile health interventions, and use of medical biomarker…

eHealth ("electronic" Health) is a new field in medicine that has the potential to change medical care, increase efficiency, and reduce costs. In this review, we analyzed the current status of eHealth in transplantation by performing a PubMed search over the last 5 years with a focus on clinical studies for post-transplant care. We retrieved 463 manuscripts, of which 52 clinical reports and eight randomized controlled trials were identified. Most studies were on kidney (n = 19), followed by liver (n = 10), solid organ (n = 7), bone-marrow (n = 6), and lung transplantation (n = 6). Eleven artic

Laryngeal transplantation offers the hope of replacing voice and laryngeal function in patients with debilitating laryngeal injuries or loss of the larynx from trauma or oncologic reasons. Our group at UC Davis performed a laryngotracheal transplantation, and our experience is reviewed in this chapter. The indications, challenges, and limitations of this procedure are highlighted, and the world's other published cases are reviewed.

The article reviews history, indication and follow-up after heart transplantation, including the mechanical assist devices. Various complications of posttransplant follow-up are mentioned, e.g. rejection, infection, vasculopathy, meta-bolic disorders, hypertension or malignities. Pharmacotherapy used for immunosuppression is discussed. Heart transplantation improves the prognosis of patients with previous heart failure and also their quality of life. Key words: heart transplantation - immunosuppression - mechanical assist devices - rejection - terminal heart failure.

In the renal allograft, transplant glomerulopathy represents a morphologic lesion and not a specific diagnosis. The hallmark pathologic feature is glomerular basement membrane reduplication by light microscopy or electron microscopy in the absence of immune complex deposits. Transplant glomerulopathy results from chronic, recurring endothelial cell injury that can be mediated by HLA alloantibodies (donor-specific antibodies), various autoantibodies, cell-mediated immune injury, thrombotic microangiopathy, or chronic hepatitis C. Clinically, transplant glomerulopathy may be silent, detectable o

Pancreas transplantation consistently induces insulin-independence in beta-cell-penic diabetic patients, but at the cost of major surgery and life-long immunosuppression. One year after grafting, patient survival rate now exceeds 95 % across recipient categories, while insulin independence is maintained in some 85 % of simultaneous pancreas and kidney recipients and in nearly 80 % of solitary pancreas transplant recipients. The half-life of the pancreas graft currently averages 16.7 years, being the longest among extrarenal grafts, and substantially matching the one of renal grafts from deceas

PURPOSE OF REVIEW: Twenty-one face transplants have been performed to date. This review provides an overview of the clinical outcomes and the lessons learned from these initial cases.
RECENT FINDINGS: Facial transplantation has progressed over the past 10 years from an experimental possibility to a clinical reality. The most recent transplants performed in the USA, Spain and Turkey involve the full face. Good sensory recovery of the allograft has been consistently reported, even in the absence of nerve repair. As predicted, motor recovery has been slower than re-innervation of sensation. Dram…

Liver transplantation is the only curative therapy for many liver diseases, mostly chronic liver diseases, occasionally also in acute or fulminant liver failure. Currently, about 100 liver transplantatations are performed each year in Switzerland. Clichy criteria are used for acute liver failure. Liver transplantation for liver cirrhosis improves survival if the Meld-Score is above 15. 5- and 10-year-survival after liver transplantation are 71% and 61%, respectively. With increasing survival after liver transplantation, the management of drug side effects in the late postoperative course becom
