Limb Salvage Does Not Predict Functional Limb Outcome after Revascularization for Traumatic Acute Limb Ischemia.

BACKGROUND: Traumatic vascular injury leading to acute limb ischemia (ALI) is an uncommon problem with a potential for high morbidity. We describe a contemporary series of patients with traumatic ALI managed primarily by vascular surgeons at a tertiary referral center and review factors associated with limb salvage and functional limb outcomes. METHODS: We conducted a single institution, retrospective review of all patients requiring revascularization for upper extremity (UE) and lower extremity (LE) ALI secondary to trauma from 2013 to 2016. Demographic data, transfer timing, injury severity

Comparative efficacy and acceptability of psychotherapies for post-traumatic stress disorder in children and adolescents: study protocol for a systematic review and network meta-analysis.

INTRODUCTION: Post-traumatic stress disorder (PTSD) is common among children and adolescents who are exposed to trauma, and it is often associated with significant negative impacts on their psychosocial functioning and quality of life. Many types of psychotherapies have been found to be effective for PTSD in children and adolescents. However, due to the lack of direct comparisons between different psychotherapies, the hierarchy of treatment efficacy is still unclear. Therefore, we plan to conduct a systematic review and network meta-analysis to evaluate the efficacy and acceptability of variou

A systematic review of the relationship between severe maternal morbidity and post-traumatic stress disorder.

BACKGROUND: The incidence of severe maternal morbidity is increasing in high-income countries as a consequence, in part, of increased obstetric intervention and increasingly complex medical needs of women who become pregnant. Access to emergency obstetric care means that for the majority of women in these countries, an experience of severe maternal morbidity is unlikely to result in loss of life. However, little is known about the subsequent impact on postnatal psychological health resulting in an evidence gap to support provision of appropriate care for these women. There has recently been in

Intensive care of patients with HIV infection: utilization, critical illnesses, and outcomes. Pulmonary Complications of HIV Infection Study Group.

To examine intensive care unit (ICU) admission rates and diagnoses of patients with HIV infection, and to determine the outcomes of different critical illnesses, we analyzed data derived from the 63 patients who were admitted to an ICU from among the 1,130 adults with HIV infection who did not have AIDS at the time of enrollment in a multicenter prospective study. Patients were admitted and treated according to the judgment of their physicians. During 4,298 patient-years of follow-up for the entire cohort, there were 1,320 hospital admissions, of which 68 (5%) included admission to an ICU. Twe

Association of insulin-like growth factor 1 serum levels with early recovery and in-hospital complications after spinal fusion.

OBJECTIVE: Sterile surgical trauma triggers a hormonal stress response that promotes a catabolic state, leading to fatigue, muscle loss, and impaired recovery. This postoperative catabolism is mediated, in part, by a decrease in insulin-like growth factor 1 (IGF-1), a key anabolic hormone regulated by human growth hormone (HGH). Given the established role of IGF-1 in muscle maintenance, tissue regeneration, and as a sensitive marker of nutritional status, the authors aimed to determine whether baseline IGF-1 levels, assessed using age- and sex-adjusted z-scores, predict short-term postoperativ

Disparities in casemix, acute interventions, discharge destinations and mortality of patients with traumatic brain injury between Europe and India.

BACKGROUND: Traumatic brain injury (TBI) is a major global health problem that disproportionally affects low- and middle-income countries. The needs for patients with TBI therefore may differ between levels of national development. We aimed to describe differences in epidemiology and acute care provision of TBI between India and Europe. METHODS: We used data from two prospective observational registry studies - the Collaborative Indian NeuroTrauma Effectiveness Research in TBI (CINTER-TBI) and the Collaborative European NeuroTrauma Effectiveness Research in TBI (CENTER-TBI), which included TB

Needs of parents of the child hospitalised with acquired brain damage.

This paper describes the findings of a descriptive study into needs of parents of children with Acquired Brain Damage during hospitalisation. Thirty four parents of 28 children treated at a tertiary referral pediatric hospital were interviewed. Parents described their experiences during their child's hospitalisation and identified needs which, when met, enabled them to care for their children and cope with the sudden illness and disabilities. Three themes were identified: services that optimised the child's recovery, services which supported parents cope with child's illness and services assis

Early tracheostomy in closed head injuries: experience at a tertiary center in a developing country--a prospective study.

BACKGROUND: An important factor contributing to the high mortality in patients with severe head trauma is cerebral hypoxia. The mechanical ventilation helps both by reduction in the intracranial pressure and hypoxia. Ventilatory support is also required in these patients because of patient's inability to protect the airway, persistence of excessive secretions, and inadequacy of spontaneous ventilation. Prolonged endotracheal intubation is however associated with trauma to the larynx, trachea, and patient discomfort in addition to requirement of sedatives. Tracheostomy has been found to play an

Psychological outcomes, coping responses, and posttraumatic growth among health care workers during the COVID-19 outbreak in Southern Turkey: A cross-sectional study from the initial phase of the pandemic.

OBJECTIVE: The early phase of the COVID-19 pandemic posed severe psychological challenges to health care workers (HCWs). This study examines the acute psychological symptoms, coping responses, and posttraumatic growth among HCWs in Southern Turkey during the first wave of the outbreak. It aims to establish a baseline understanding of the initial psychological impact that preceded the development of chronic conditions documented in later pandemic phases. METHOD: A cross-sectional, web-based survey was conducted between April 30, 2020 and May 15, 2020, among 639 HCWs from two COVID-19 referral

BLS competency of doctors and nurses serving during the war in Gaza: insights from a cross-sectional study.

BACKGROUND: Basic Life Support (BLS) is a standard of care and a crucial component of emergency medicine. In conflict-affected regions like Gaza, the importance of timely and effective BLS is ever more critical. However, the level of BLS knowledge among Gazan healthcare workers remains underexplored. Understanding current competency levels is vital for improving patient outcomes, guiding training programs, and enhancing emergency preparedness in this high-risk environment. METHODS: A cross-sectional survey conducted in November 2024 recruited 402 doctors and nurses from six main governmental

Enhancing nursing and other healthcare professionals' knowledge of childhood sexual abuse through self-assessment: A realist review.

AIM: To explore how child sexual abuse/exploitation (CSA/E) self-assessment tools are being used to enhance healthcare professionals' knowledge and confidence. BACKGROUND: Child sexual abuse/exploitation is common and associated with lifelong health impacts. In particular, nurses are well-placed to facilitate disclosures by adult survivors of child sexual abuse/exploitation and promote timely access to support. However, research shows that many are reluctant to enquire about abuse and feel underprepared for disclosures. Self-assessment provides a participatory method for evaluating competenci

Types of dairy foods and risk of fragility fracture in the prospective Nurses' Health Study cohort.

BACKGROUND: Fragility fractures present enormous health challenges for women. Dairy products provide many bone-beneficial nutrients, such as calcium and vitamin D. Individual dairy foods may exert different effects on bone health. OBJECTIVES: The aim of this study was to investigate the associations between total dairy, yogurt, milk, and cheese and fragility fracture risk among females in the prospective Nurses' Health Study (NHS) conducted in the United States. METHODS: In the current analysis, 103,003 females with mean age of 48 y were followed from 1980-2004. Proportional hazards models w

Resident performance in complex simulated urinary catheter scenarios.

BACKGROUND: Urinary catheter insertion is a common procedure performed in hospitals. Improper catheterization can lead to unnecessary catheter-associated urinary tract infections and urethral trauma, increasing patient morbidity. To prevent such complications, guidelines were created on how to insert and troubleshoot urinary catheters. As nurses have an increasing responsibility for catheter placement, resident responsibility has shifted to more complex scenarios. This study examines the clinical decision-making skills of surgical residents during simulated urinary catheter scenarios. We hypot

Management of temporary urinary retention after arthroscopic knee surgery in low-dose spinal anesthesia: development of a simple algorithm.

INTRODUCTION: In practice, trauma and orthopedic surgery during spinal anesthesia are often performed with routine urethral catheterization of the bladder to prevent an overdistention of the bladder. However, use of a catheter has inherent risks. Ultrasound examination of the bladder (Bladderscan) can precisely determine the bladder volume. Thus, the aim of this study was to identify parameters indicative of urinary retention after low-dose spinal anesthesia and to develop a simple algorithm for patient care. MATERIALS AND METHODS: This prospective pilot study approved by the Ethics Committee
