Congenital syphilis presenting with a generalized bullous and pustular eruption in a premature newborn.

Congenital syphilis occurs when Treponema pallidum crosses the placenta during pregnancy or from contact with an infectious genital lesion during delivery. Cutaneous manifestations of congenital syphilis are relatively common, occurring in approximately 30% to 70% of patients. Maculopapular lesions, vesiculobullous lesions, condylomata lata lesions, annular lesions, and erythema multiforme-like targetoid lesions have been reported. We report on a premature newborn with congenital syphilis who presented with generalized bullous and pustular eruption and desquamation at birth.

Infectious complications of tattoos.

Tattooing carries several medical risks, including the transmission of infectious diseases. We review the published literature on the transmission of hepatitis B virus, human immunodeficiency virus, Treponema pallidum, papillomavirus, Mycobacterium tuberculosis, and other organisms by tattooing. Education, through public health measures, should promote the prevention of infectious disease transmission. Particular populations who could benefit from education include prisoners, individuals involved with correctional facilities, youths, military personnel, and health care providers coming in cont…

[Not Available].

Syphilis is a sexually transmitted disease caused by the spirochaete Treponema pallidum. Patients with untreated syphilis can develop meningovascular syphilis at any stage of the disease. This is a case report of a 44-year-old man displaying two instances of acute vertigo and lateralized paraesthesia. MRI showed infarctions in the left thalamus and capsula interna. Subsequent investigations including cerebral spinal fluid analysis revealed a diagnosis of neurosyphilis. The patient was treated intravenously with benzylpenicillin and ceftriaxone with complete clinical remission.

Neurosyphilis-Induced Psychosis: A Unique Presentation of Syphilis With a Primary Psychiatric Manifestation.

Syphilis is a predominantly sexually transmitted infection caused by the spirochete Treponema pallidum. The infection presents with four different stages and although rare, can lead to behavioral symptoms if not treated in its earliest form. It can cause psychosis, mania, depression, anxiety, and personality changes. Screening and early treatment of syphilis are essential in preventing neurosyphilis and its neuropsychiatric symptoms. Neurosyphilis is rarely the initial presentation of syphilis. This is a case report of a 30-year-old female with neurosyphilis who presented with psychosis as the…

Plaques en prairie fauchée.

We report a clinical picture of a mowed meadow pattern of the tongue in a 29-year-old sexually active man who has sex with men. The painful lesions also called "Plaques en prairie fauchée", occur in the context of the discovery of a postive HIV serology. The diagnosis of secondary syphilis was confirmed by a positive syphilis serology (nontreponemal RPR test title of 64) and a positive Treponema pallidum PCR, amplifying the tpp47 gene, on a tongue swab. The lesions completely resolve with a penicillin treatment.

Syphilis Causing Lichenoid Esophagitis.

We present a 50-year-old woman with human immunodeficiency virus admitted to the hospital for melenic stools and anemia who underwent esophagogastroduodenoscopy for evaluation of upper gastrointestinal bleed. She is found to have inflammation of the esophagus with ulcerations and crater formation. A biopsy reveals severe inflammation with lymphocytic infiltrates with civatte bodies suggestive of lichenoid esophagitis with the presence of spirochetes (Treponema pallidum). The presence of syphilis in the esophagus causing lichenoid esophagitis is an extremely rare presentation that has never bee…

Challenges in the diagnosis and treatment of congenital syphilis.

Despite the advent of effective antibiotic treatment of Treponema pallidum, syphilis continues to be present in our population and recently has been increasing in frequency. The consequences can be especially dire in the setting of an untreated infection in pregnancy. This case report describes a case of congenital syphilis in a dichorionic-diamniotic twin pregnancy that resulted in a stillbirth of one twin and the characteristic findings of congenital syphilis in the surviving twin. This report demonstrates a number of challenges to the clinician in the effective and timely diagnosis and trea…

[The analysis of factors which affect qualification of pregnant women as cord blood donors].

Authors' analysis refers to reasons for which some women did not meet criteria for the programme of collection of cord blood by The Cord Blood Bank in Warsaw. The main reasons were: 1) reactivation of infection with CMV virus, 2) complications of delivery and 3) technical problems. We present also preliminary results of blood tests aimed at identification of some viral (hepatitis B and C, HIV, CMV), bacterial (Treponema pallidum) and protozoal infections (Toxoplasma) in the selected group of pregnant women.

Early oral presentation of lues maligna in a patient with HIV infection. A case report.

We present a case of a patient infected with the human immunodeficiency virus who developed syphilis manifested by atypical early oral and skin ulcerations. The profound immune defects associated with human immunodeficiency virus may lead to an altered clinical presentation and a more aggressive course in patients infected with Treponema pallidum. The unusual clinical manifestations observed in this case emphasize the importance of considering secondary syphilis in the differential diagnosis of any inflammatory mucosal and skin disorder in patients with the human immunodeficiency virus.

[Liver involvement in early secondary syphilis].

Two patients with secondary new syphilis are described, abusing alcohol, developing early syphilitic hepatitis. The manifestations of this condition are similar to those of viral hepatitis. This necessitates a thorough examination of the skin integument, visible mucosae, genitals and perianal area, and of the peripheral lymph nodes in the patients with suspected viral hepatitis, in order to timely detect a possible liver involvement in Treponema infection. When such patients consult a doctor, specific serologic tests should be carried out and case history be carefully recorded.

Atypical early syphilis in an hiv-infected homosexual male.

A homosexual male, seropositive to HIV and with previously documented syphilitic infections, developed a slowly growing nodule on his left wrist (possibly a chancre) followed, after 2 months, by a few scattered, large papular lesions. Serological evidence of active syphilis was obtained and treponema were identified in the initial lesion by means of immunofluorescent staining. Treatment with both aqueous crystalline penicillin G and benzathine penicillin G healed the lesions. This represents a further case of an atypical presentation of early syphilis in an immunocompromised host.

Lyme disease-a tick-borne spirochetosis?

A treponema-like spirochete was detected in and isolated from adult Ixodes dammini, the incriminated tick vector of Lyme disease. Causally related to the spirochetes may be long-lasting cutaneous lesions that appeared on New Zealand White rabbits 10 to 12 weeks after infected ticks fed on them. Samples of serum from patients with Lyme disease were shown by indirect immunofluorescence to contain antibodies to this agent. It is suggested that the newly discovered spirochete is involved in the etiology of Lyme disease.

Comparison of the standard Q syphilis antibody rapid diagnostic test to gold standards for yaws detection in children.

INTRODUCTION: Yaws remains a public health problem in Indonesia, and it is the largest contributor to Yaws cases in Southeast Asia. Yaws is caused by bacterial infection of Treponema pallidum subspecies pertenue, mainly affecting the skin and bones. An estimated 75% of new cases were found in children under 15. The diagnosis of yaws is based on clinical findings with serological confirmation. Rapid diagnostic tests (RDT) have been developed to diagnose Yaws faster and simpler and can thus be used in areas with limited resources. METHODOLOGY: In this study, sensitivity and specificity of the s…

Lysinoalanine cross-linking is a conserved post-translational modification in the spirochete flagellar hook.

Spirochetes cause Lyme disease, leptospirosis, syphilis, and several other human illnesses. Unlike other bacteria, spirochete flagella are enclosed within the periplasmic space where the filaments distort and push the cell body by the action of the flagellar motors. We previously demonstrated that the oral pathogen Treponema denticola (Td) and Lyme disease pathogen Borreliella burgdorferi (Bb) form covalent lysinoalanine (Lal) cross-links between conserved cysteine and lysine residues of the FlgE protein that composes the flagellar hook. In Td, Lal is unnecessary for hook assembly but is requi…

Case Report: A Case Report of Neurosyphilis Mimicking Limbic Encephalitis.

Neurosyphilis (NS) is an infection of the central nervous system caused by Treponema pallidum. It mimics various neurological and psychiatric diseases. In recent years, there have been several NS cases that manifest as limbic encephalitis (LE). Therefore, the diagnosis of neurosyphilis in the early stages is difficult. Here, we present a case of an NS patient who presented with LE manifestation. The 62-year-old woman presented with acute clinical manifestations of gibberish speech, poor memory, and seizures. Brain MRI showed abnormal signals on the right medial temporal lobe. In addition, the …

Response to Penicillin for Presumed Neurosyphilis in a Patient with Human Immunodeficiency Virus (HIV), a Normal CD4 Count, and an Undetectable Viral Load: A Case Report.

BACKGROUND Neurosyphilis is a bacterial infection of the brain and the spinal cord, caused by Treponema pallidum. Its nonspecific clinical presentation includes cognitive impairment and motor and/or sensory function compromise. Neurosyphilis infections in patients with HIV have increased over the past few years and many cases of neurosyphilis manifest in patients with HIV who have low CD4 T-cell counts and high viral loads (VL). However, there is extremely limited acknowledgement in the literature about neurosyphilis presentations in patients with HIV who have normal CD4 counts. CASE REPORT We…
