[A conus medullaris tuberculoma: a case report and literature review].

BACKGROUND: Tuberculosis affecting the central nervous system is well recognized, but only rarely localizes to the medullary conus. OBSERVATION: We report the case of a 69 year old man who was admitted to our unit with cauda equina syndrome. The MRI demonstrated ring-enhanced necrotizing lesions involving the medullary conus, the cervical cord and the brain. His chest CT scan showed a miliary infiltrate. The clinical presentation was associated with an inappropriate secretion of antidiuretic hormone. Quadruple antituberculous therapy was initiated, with corticosteroids in the initial phase of

[Renal tuberculosis disease indicative of Pott's disease].

Tuberculosisis is a serious desease, causing high morbidity and mortality. It includes frequent extra-pulmonary forms, polymorphic in their clinico-radiological presentation, resultsing in a delayed diagnosis. We report the case of a rare association of renal tuberculosis and Pott's disease. It is the case of a 19-year-old patient. He has two brothers on chronic hemodialysis. He is hospitalized for exploration of a lumbar mass and a cachectic state. Radiological imaging (MRI, scanner) suggests osteosarcoma. The renal biopsy, performed for the nephrotic syndrome, reveals the presence of a granu

[CLINICAL INVESTIGATION OF 6 CASES OF TUBERCULOUS SPONDYLITIS].

[Objective] A delay in the diagnosis of tubercu- lous spondylitis can result in worsening of the condition. We investigated previously reported cases of tuberculous spondylitis, as well as cases experienced in our hospital, to identify factors that are useful in the diagnosis. [Materials and Methods] We retrospectively evaluated six cases of tuberculous spondylitis diagnosed in our hospital between October 2007 and September 2012, and an additional 23 cases that had been reported in Japan between 1994 and 2014. [Results] The median age of our six patients was 78.5 years and five were women. In

The classic. Streptomycin in tuberculous bone and joint lesions with mixed infection and sinuses.

At the present time, streptomycin represents a tremendous advance in the treatment of patients with these lesions. Streptomycin undoubtedly will be improved upon and superseded by some other agent in the future, giving us better control of this disease and possibly enabling us to eradicate it. In closed lesions streptomycin, in dosages of 90 grams in a period of 90 days, has failed to arrest the tuberculous process permanently. Failure of treatment with streptomycin suggests strongly the presence of a sequestrum, a thick-walled abscess, or inadequate surgery. Healing seems to be related to the

[Non-traumatic arachnoiditis and syringomyelia. A series of 32 cases].

We conducted a retrospective study of 32 patients treated for syringomyelia associated with non-traumatic arachnoid scarring. We selected the cases with documented history of arachnoiditis with pre and post-operative clinical evaluation of the neurological status and anatomical study on MRI with a minimal follow-up of one year. Extensive arachnoid scarring (Group I) was noted in 18 cases, after spinal meningitis in 15 cases (tuberculosis in 9 cases, listeria in 3 cases, pyogenic meningitis in 3 cases), subarachnoid hemorrhage in 3 cases. Focal arachnoid scarring (Group II) occurred in 10 cases

Medical-surgical treatment of progressive tuberculous (Pott's) paraplegia in Gabon.

The present study deals with the results of the medical-surgical treatment of 22 patients with Pott's tetraplegia or paraplegia. Seventeen had progressive tetraplegia-paraplegia which failed to respond solely to medical treatment. On admission, four patients exhibited an acute onset tetraplegia-paraplegia, and one had a 'spinal tumour syndrome'. In addition to antituberculous therapy, seven patients had anterior spinal surgery, consisting of four corporectomies, two anterior debridments and grafting, and one debridment alone. Moreover, one patient had a posterior interbody fusion, four had lam

Challenges in Diagnosis and Management of Atlantoaxial Tuberculosis: A Case Report.

Background and Objectives: Atlantoaxial tuberculosis (TB) is rare, and its diagnosis is difficult. Herein, we present a rare case with a challenging diagnostic journey of atlantoaxial TB spanning over two years. Materials and Methods: A 70-year-old immunocompetent female patient presented with a four-week history of nuchal pain, stiffness, and headache. She did not have any TB-associated constitutional symptoms. The result of the initial biopsy indicated only a nonfermenting Gram-negative bacillus and the histopathological report revealed concurrent acute and chronic inflammation. Posterior fu

[Comparison of short-term effectiveness of structural and non-structural bone graft fusion in treatment of single segment thoracic tuberculosis].

OBJECTIVE: To compare the short-term effectiveness of one-stage posterior debridement with non-structural bone graft and structural bone graft in the treatment of single segment thoracic tuberculosis. METHODS: The data of 61 patients with single segment thoracic tuberculosis, who were treated by one-stage posterior debridement, bone graft fusion, and internal fixation between June 2011 and August 2015, was retrospectively analyzed. All of them, 26 cases were treated with structural bone graft (group A) and 35 cases with non-structural bone graft (group B). No significant difference was found

Tuberculosis of spine with neurological deficit in advanced pregnancy: a report of three cases.

BACKGROUND CONTEXT: Early decompression in spinal tuberculosis (TB) with complete paraplegia has a better prognosis in relation to the neurological recovery and deformity progression. Advanced pregnancy can complicate this picture in view of the various fetomaternal factors. The prevalent literature is inconclusive regarding the time and extent of surgical intervention. Delay in the surgical intervention may adversely affect the prognosis. PURPOSE: To emphasize the importance of early decompression in spinal TB complicated by neurological deficit in advanced pregnancy. STUDY DESIGN: A case r

Unusual locations of osteoarticular tuberculosis in children: a report of 12 cases.

INTRODUCTION: Unusual locations of osteoarticular tuberculosis (OA-TB) raise diagnostic issues due to their untypical and non-suggestive clinical and radiological presentation. OBJECTIVES: The present retrospective study analyzed the various clinical, radiological and therapeutic aspects. PATIENTS AND METHODS: A retrospective series included 12 children (mean age, 7 years 4 months; sex-ratio, 0.7), treated in our department between 1980 and 2010. Knee, hip and spine locations were excluded. RESULTS: Mean time to diagnosis was 32 months. Active TB infection was identified in 42% of cases. Pa

Cervical spine tuberculosis in children.

The presentation and outcome of pediatric cervical spine tuberculosis are different from those of adult cervical spine tuberculosis. We retrospectively reviewed the clinical and radiographic outcome of 58 children with cervical spine tuberculosis treated nonoperatively and operatively between 1996 and 2004. The mean age was 3.7 years (range, 1.9-14 years). The cervicodorsal junction was affected in 27 children, the atlantoaxial complex in 19 children, and the mid-cervical spine in 12 children. Multifocal noncontiguous spinal lesions were observed in 21 patients. Surgery was performed in 25 chi

Tuberculosis from head to toe.

Tuberculosis can affect virtually any organ system in the body and can be devastating if left untreated. The increasing prevalence of tuberculosis in both immunocompetent and immunocompromised individuals in recent years makes this disease a topic of universal concern. Because tuberculosis demonstrates a variety of clinical and radiologic findings and has a known propensity for dissemination from its primary site, it can mimic numerous other disease entities. Primary pulmonary tuberculosis typically manifests radiologically as parenchymal disease, lymphadenopathy, pleural effusion, miliary dis

Tongue Tuberculosis as a Complication of Pott's Disease in a Patient on Systemic Steroid Therapy without Pulmonary Tuberculosis.

A 78-year-old man with a previous diagnosis of rheumatoid arthritis on prolonged treatment with corticosteroids presented with intense and progressive pain at the cervical level that prevented him from resting his head and walking, in addition to an ulcerative lesion covering 80% of the lingual area that was previously treated as oral candidiasis without improvement. On arrival, with no clinical or serological data of rheumatoid arthritis, immunosuppressive treatment was suspended, and a biopsy of the oral cavity was requested, confirming the diagnosis of lingual tuberculosis, an extremely rar
