A 54-y-o woman presented to the Emergency Department with shortness of breath and sore throat after intranasal administration of Ecbalium elaterium as a folk remedy for her sinusitis. The patient's history included nasal aspiration of the juice of the squirting cucumber (Ecbalium elaterium) for acute maxillary sinusitis. Obstructive sleep apnea results from a combination of a structurally small upper airway and loss of upper airway muscle tone. Surgical therapy is effective by enlarging the upper airway and decreasing collapsibility. Limited palatal surgery has been demonstrated successful for the treatment of snoring but less for the treatment of obstructive sleep apnea. CO2 laser pharyngotomy is a newly described technique used in treatment bronchopathy without sleep apnea syndrome. Retrospective results of 70 CO2 laser pharyngotomies and 63 surgical pharyngoplasties are studied and compared. All patients were SAS free snoring patients. The results were rated satisfactory by 54% of the CO2 Laser treated patients against 78% for classical surgery. CO2 laser pharyngotomy is an easy procedure done in the office. We have studied the effects of phonation and posture on the Mallampati classification of view of the pharyngeal structures. Auditory brain-stem responses (ABRs) of 20 patients suffering from obstructive sleep apnea syndrome (OSAS) were studied. Their average I, V latencies and III-V intervals were significantly prolonged; the I-III intervals were statistically shortened; but the III latencies and I-V intervals basically were normal. An increasing number of loud snorers seek medical attention because of the social impact of snoring as well as its association with sleep apnea. Uvulopalatopharyngoplasty is reported to reduce or eliminate snoring in the majority of patients; however, little data are available to document the procedure's success. Thirty-four patients (32 male, 2 female; mean age 53 +/- 7 years) with confirmed sleep apnea syndrome (SAS) were studied before and after uvulopalatopharyngoplasty (UPPP).
