Hybrid approach to transcatheter pulmonary valve replacement in patients with excessively large anatomy.

After right ventricular outflow tract obstruction reconstruction for CHD, surgical pulmonary valve replacement, or transcatheter pulmonary valve replacement may be performed if complicated by moderately severe or severe right ventricular outflow tract dysfunction. However, for patients whose anatomy is not suitable for transcatheter pulmonary valve replacement, surgical pulmonary valve replacement is the only option, but it has a higher rate of perioperative complications and longer hospitalisation for patients. In two cases of severe transcatheter pulmonary valve replacement in which percutan

A Rare Entity: Double Outlet of Both Ventricles.

Double outlet of both ventricles is an anomaly wherein both ventricles equally share the arterial trunks. A majority of the literature describes a variant of this disease with a muscular outlet septum which is perpendicular to the plane of the ventricular septum although a variant with a fibrous muscular septum can also be present. The condition may be associated with obstruction of either outflow tract, which may complicate repair. We report a child with double outlet of both ventricles and a fibrous outlet septum with unobstructed outflows. The child underwent a successful surgical repair us

Long-term follow-up of patients with obstructive hypertrophic cardiomyopathy treated with dual-chamber pacing.

In this study, patients with obstructive hypertrophic cardiomyopathy (HC) were treated with dual-chamber pacemaker therapy. Long-term follow-up analysis showed that dual-chamber pacemaker therapy in selected patients resulted in a significant reduction in symptoms and in the left ventricular outflow tract gradient, which was maintained up to 10 years after implantation. Dual-chamber pacing is of potential long-term benefit in selected groups of patients with obstructive HC.

Ruptured infective sinus of valsalva aneurysm complicated by peripheral mycotic pulmonary artery aneurysm: a case report.

BACKGROUND: Ruptured sinus of Valsalva aneurysm associated with infective endocarditis is a rare but life-threatening condition. Peripheral mycotic pulmonary artery aneurysm is even rarer and carries a high risk of rupture. The coexistence of infective ruptured sinus of Valsalva aneurysm and a peripheral mycotic pulmonary artery aneurysm has not been previously reported. CASE PRESENTATION: A 58-year-old woman developed pneumonia and infective endocarditis with mobile vegetation in the right ventricular outflow tract, resulting in right ventricular outflow tract stenosis and septic pulmonary e

"Diverticula" of anterior mitral valve leaflet as a cause of subvalvular aortic stenosis.

A 77-year-old male patient presented with symptoms of shortness of breath, fatigue, chest pain on exertion and dizziness. Transthoracic echocardiography suggested the presence of a diaphragmatic type of obstruction in the subaortic area of the left ventricular outflow tract. The systolic peak gradient at rest was 34 mmHg with a mean of 23 mmHg. Cardiac catheterization demonstrated rounded radiolucencies in the left ventricular outflow tract in the form of two "pouches" that moved back and forth causing subaortic stenosis. There was also a 70% stenosis of the left anterior descending coronary a

[An accurate formula for assessing the volume of the right ventricular outflow tract].

Practical application of formulae for separate assessment of the volumes of inlet and outlet sections of the right ventricle derived previously by the authors has shown that the formula for right ventricular outlet section determining it as a curved truncated cone with nonparallel bases does not take into account a narrowing in the middle third of the outlet portion during the right ventricular systole and/or supraventricular crest hypertrophy. In the specified formula right ventricular outlet portion volume is determined as a sum of the volumes of two truncated cones with a borderline in the

Outflow Graft Narrowing of the HeartMate 3 Left Ventricular Assist Device.

BACKGROUND: In patients with the HeartMate 3 (HM3, Abbott) left ventricular (LV) assist device (LVAD), outflow graft narrowing has been reported as a result of accumulation of biodebris either internal or external to the graft. This study describes the prevalence, imaging findings, and clinical outcomes associated with HM3 LVAD outflow graft narrowing. METHODS: A single-center retrospective cohort study was performed in patients who received an HM3 LVAD between November 2014 and July 2019. All patients with a computed tomographic (CT) angiogram or a CT scan with intravenous contrast sufficien

Intraoperative Two- and Three-Dimensional Transesophageal Echocardiography in Combined Myectomy-Mitral Operations for Hypertrophic Cardiomyopathy.

Transesophageal echocardiography is essential in guiding the surgical approach for patients with obstructive hypertrophic cardiomyopathy. Septal hypertrophy, elongated mitral valve leaflets, and abnormalities of the subvalvular apparatus are prominent features, all of which may contribute to left ventricular outflow tract obstruction. Surgery aims to alleviate the obstruction via an extended myectomy, often with an intervention on the mitral valve and subvalvular apparatus. The goal of intraoperative echocardiography is to assess the anatomic pathology and pathophysiology in order to achieve a

Superior vena cava reconstruction using bovine jugular vein conduit.

The glutaraldehyde-treated bovine jugular vein conduit (BJVC) is a xenograft conduit initially used for right ventricular outflow tract reconstruction and has never been used for reconstruction of superior vena cava (SVC). In September 2003, a patient with SVC obstruction underwent SVC reconstruction using BJVC. He has been alive for 42 months and free from signs and symptoms of SVC obstruction except that metastasis was found in the vertebrae. The radionuclide venography showed the graft tube was patent and only slight stenosis was found in the proximal anastomosis. The initial result support

Surgical Repair of Secondary Subaortic Stenosis in Congenital Heart Disease Without Initial Subaortic Obstruction.

BACKGROUND: Secondary subaortic stenosis (SSS) is a rare heart disease of the left ventricular outflow tract (LVOT). It usually occurs after cardiovascular correction with or without initial left ventricular outflow tract obstruction (LVOTO). Because most patients with SSS are asymptomatic, many do not realize the need for reoperation until the obstruction worsens. Few studies suggest the characteristics and reasons of SSS without initial SAS. We conducted a retrospective study to describe the characteristics and surgical outcomes of these patients. METHODS: In this study, we examined a singl

BACKGROUND: The SESAME (SEptal Scoring Along Midline Endocardium) procedure is early in its evolution but seems to be an effective adjunct to facilitate transcatheter mitral valve replacement (TMVR) in patients with complex anatomy. CASE SUMMARY: An 80-year-old woman was admitted with pulmonary edema and diagnosed with severe mitral annular calcification (MAC) and significant mixed mitral disease prohibitive for surgery. She had a history of hypertrophic obstructive cardiomyopathy previously treated with alcohol septal ablation. We performed a SESAME followed by LAMPOON (Laceration of the Ant

Extended aortic root replacement with cryopreserved allografts: do they hold up?

The extended aortic root replacement technique is used for the surgical repair of left ventricular outflow tract obstruction complicated by concurrent hypoplastic aortic annulus, multiple levels of obstruction, or aortic insufficiency. Extended aortic root replacement incorporates the concepts of aortic root replacement and aortoventriculoplasty to create a comparatively simple procedure. Unique features of the technique include the implantation of an allograft, which makes the need for anticoagulation obsolete, and use of the donor mitral leaflet to enlarge the outflow tract. Since 1985, 32 p

Pulmonary Valve Regurgitation: Neither Interventional Nor Surgery Fits All.

Introduction: PV implantation is indicated for severe PV regurgitation after surgery for congenital heart defects, but debates accompany the following issues: timing of PV implantation; choice of the approach, percutaneous interventional vs. surgical PV implantation, and choice of the most suitable valve. Timing of pulmonary valve implantation: The presence of symptoms is class I evidence indication for PV implantation. In asymptomatic patients indication is agreed for any of the following criteria: PV regurgitation > 20%, indexed end-diastolic right ventricular volume > 120-150 ml/m2 BSA, and

Familial hypertrophic obstructive cardiomyopathy with the GLA E66Q mutation and zebra body.

BACKGROUND: Fabry disease is caused by mutations in the α-galactosidase A (GLA) gene, which is located in X-chromosome coding for the lysosomal enzyme of GLA. Among many gene mutations, E66Q mutation is under discussion for its pathogenicity because there is no clinical report showing pathological evidence of Fabry disease with E66Q mutation. CASE PRESENTATION: A 65-year-old Japanese female was referred to our hospital for chest discomfort on effort. Transthoracic echocardiography showed severe left ventricular (LV) hypertrophy with LV outflow obstruction. Maximum LV outflow pressure gradient

Does myectomy convey survival benefit in hypertrophic cardiomyopathy?

Obstructive hypertrophic cardiomyopathy (HCM) can be treated by transaortic subaortic myectomy, which has been shown to improve patients' symptoms and abolish left ventricular outflow tract obstruction. Different options, such as dual chamber pacing and septal ethanol ablation, are now available for the management of obstructive HCM, and it is important to evaluate the risks and benefits of myectomy, especially its impact on survival. In this article, the authors review the clinical course of obstructive HCM and the early and long-term results of surgery. They examine the outcomes of obstructi

Right ventricular outflow tract stenting in tetrology of fallot with restrictive ventricular septal defect.

A 6-month-old child with worsening cyanosis from birth, hypercyanotic spells and failure to thrive, was admitted with severe desaturation. Systemic saturation was < 56%, and echocardiography revealed severe infundibular pulmonary stenosis with normal pulmonary valve and a restrictive ventricular septal defect. The child underwent percutaneous stenting of the right ventricular outflow tract (RVOT), with good reduction in gradient and clinical stabilization. The child is being followed up for elective intracardiac repair.

Tissue Doppler imaging in hypertrophic cardiomyopathy: impact of intraventricular obstruction on longitudinal left ventricular function.

BACKGROUND: In non-obstructive hypertrophic cardiomyopathy, tissue Doppler imaging of the mitral annulus shows severe systolic and diastolic dysfunction, with marked heterogeneity and asynchrony. In obstructive forms, the complexity of pathophysiological interactions makes conventional echocardiographic functional assessment extremely difficult and complex. OBJECTIVE: To study longitudinal left ventricular function with tissue Doppler imaging in the obstructive forms of hypertrophic cardiomyopathy. METHODS: Twenty-six patients with hypertrophic obstructive cardiomyopathy and 23 patients with

The relationship of the outlet septum to the aortic outflow tract in hearts with interruption of the aortic arch.

We examined 13 hearts with concordant atrioventricular and ventriculoarterial connections and interruption of the aortic arch to establish and describe the morphologic features of the outflow tracts in relation to axial deviation and malalignment of the outlet septum as opposed to overriding of the arterial valvular orifices. Interruption in all cases but one was between the left common carotid and left subclavian arteries; the other arch was interrupted at the isthmus. A patent arterial duct and ventricular septal defect were universally present. When its borders were viewed from the right ve
