Assessment of Liver Fibrosis Stage and Cirrhosis Regression After Long-Term Follow-Up Following Sustained Virological Response.

Background/Objectives: Previous studies have demonstrated that the cessation of liver damage after HCV cure can improve liver function, histological necroinflammation, and portal hypertension. However, scarce data about fibrosis stage or cirrhosis regression have been reported during follow-up. Methods: A prospective study evaluating hepatic biopsies and liver stiffness measurement by vibration-controlled transient elastography (VCTE-LSM) after the end of treatment (EOT) in patients with compensated advanced chronic liver disease (cACLD). Fibrosis was evaluated according to two semi-quantitati

Brief communication: virological response and drug resistance patterns among men who have sex with men in Yaoundé-Cameroon.

Men having sex with men (MSM) in Cameroon are disproportionately affected by HIV. We evaluated virological response and HIV drug resistance (HIVDR) at a community-based organization between 2022 and 2023. Enhanced adherence counselling (EAC) sessions were conducted for all and cases with unsuppressed viral load (VL > 1000 copies/ml) underwent genotyping. From 502 treatment-experienced MSM enrolled, 95 (18.9%) had unsuppressed VL with 82 (86.3%) accepting to participate (ART-duration: 3 [2-5] years; 95.1% (78/82) under Tenofovir + Lamivudine + Dolutegravir). EAC led to 97.6% (80/82) suppression

Impact of sustained virological response on prognosis after hepatectomy for hepatitis C-related hepatocellular carcinoma: a retrospective cohort study.

BACKGROUND: Eradicating hepatitis C virus (HCV) with direct-acting antivirals (DAAs) reduces hepatocellular carcinoma (HCC) risk, but its impact on post-hepatectomy outcomes remains uncertain. We evaluated whether achieving sustained virological response (SVR) improves prognosis in HCV-related HCC after curative hepatectomy. METHODS: This retrospective cohort study included HCV-related HCC patients undergoing hepatectomy (2017-2024). Patients were stratified by SVR status: DAA-treated Group (achieved SVR post-DAA) and Untreated Group (no treatment). Kaplan-Meier analysis compared recurrence-f

Predictive Value of Sustained Virologic Response at Week 4 in Patients with Hepatitis C Virus Infection Treated with Sofosbuvir/Velpatasvir.

Direct-acting antiviral therapies can cure most people with hepatitis C virus (HCV) infection with little need for testing or monitoring. A major challenge to eliminating HCV is ensuring patients complete all steps of care, including confirmation of cure. We assessed the concordance of sustained virologic response (SVR) at 4 weeks (SVR4) and 12 weeks (SVR12) post-treatment to evaluate the viability of SVR4 as a predictor of cure in patients treated with sofosbuvir (SOF)/velpatasvir (VEL). We conducted a retrospective analysis of patients from the Phase 3 ASTRAL-1, -2, and -3 programs and a his

Virologic Response to Antiviral Therapy in Chronic Hepatitis B Patients With High Viral Load: A Real-World Cohort Study.

Current guidelines for chronic hepatitis B (CHB) have progressively expanded antiviral treatment indications, with age > 30 years increasingly being incorporated as an important criterion. However, real-world evidence regarding treatment outcomes in this expanded population remains limited. This study evaluated the 48-week treatment response and its independent predictors in treatment-naïve CHB patients aged > 30 years with a high baseline viral load. This retrospective study included 808 treatment-naïve CHB patients aged > 30 years with baseline HBV DNA ≥ 2000 IU/mL who received first-line nu

Integrase Strand Transfer Inhibitors for Treatment-experienced Young Adults With Perinatal HIV in the US: Immunologic, Virologic, and Anthropometric Outcomes.

BACKGROUND: The efficacy of integrase strand transfer inhibitors (INSTIs) among adults with HIV is well-established, though adverse effects, particularly weight gain, are common. Comparable data for treatment-experienced adolescents and young adults with perinatally-acquired HIV (AYAPHIV) are limited. METHODS: AYAPHIV in the US-based Pediatric HIV/AIDS Cohort Study who switched to bictegravir (BIC), dolutegravir (DTG), elvitegravir (EVG), or raltegravir (RAL) from any prior regimen were eligible. Using mixed-effects models, viral load, CD4 count, weight, and body mass index were described thr

Virological failure and risk factors among people living with HIV taking second-line ART in Addis Ababa, Ethiopia.

BACKGROUND: Virological failure (VF) presents significant challenges in the emergence of drug resistance, and elevated risk of transmission, higher mortality rates, and a diminished quality of life. Various factors contribute to VF, but documented information on this issue is lacking in Ethiopia. Therefore, this study aimed to assess the prevalence of VF and identify the risk factors among people living with HIV who are on second-line antiretroviral treatment (ART). METHODS: A concurrent mixed-method study using quantitative and qualitative data was conducted at selected hospitals in Addis Ab

Virological Passive Surveillance of Avian Influenza and Arboviruses in Wild Birds: A Two-Year Study (2023-2024) in Lombardy, Italy.

Avian influenza (AI), caused by Alphainfluenzavirus (family Orthomyxoviridae), poses significant threats to poultry, biodiversity, and public health. AI outbreaks in poultry lead to severe economic losses, while highly pathogenic strains (HPAIVs) severely impact wild bird populations, with implications for biodiversity and potential zoonotic risks. Similarly, arboviruses such as West Nile virus (WNV) and Usutu virus (USUV) are emerging zoonoses. WNV can cause severe neurological diseases in birds, humans, and other animals, while USUV significantly affects blackbird populations and has zoonoti

Virological characterization of SARS-CoV-2 BA.2.86 variants by assessing antiviral susceptibility, in vivo infectivity and replicative fitness.

In late 2023, the BA.2.86 and EG.5.1 SARS-CoV-2 variants co-circulated globally, raising concerns about enhanced immune evasion and altered replicative capacity. In this study, we assessed their virological characteristics using live viruses. We evaluated sensitivity to therapeutic monoclonal antibodies and antivirals targeting NSP5 and NSP12, and performed direct competition assays. BA.2.86 displayed a complete loss of neutralization by Sotrovimab, consistent with the presence of the K356T escape mutation, whereas EG.5.1 retained similar susceptibility to its XBB ancestor. Both variants remai

Association Between Sustained Virological Response and Adverse Liver-Related Events in Patients With Decompensated Hepatitis C Virus Cirrhosis.

BACKGROUND & AIMS: Sustained virological response (SVR) improves prognosis in patients with chronic hepatitis C virus (HCV) with compensated cirrhosis, but whether a similar benefit can be obtained in decompensated patients is controversial. We studied the association between SVR and liver-related events (LREs) in patients with decompensated HCV cirrhosis. METHODS: We included patients with decompensated HCV cirrhosis (Child-Turcotte-Pugh [CTP] ≥7 and/or history of decompensation) treated with direct-acting antivirals. The association between SVR and LREs, and between SVR-related change in Mo

Health Disparities in Achieving Sustained Virologic Response Among Patients With Chronic Hepatitis C: A Retrospective Single-Center Study.

Introduction Hepatitis C virus (HCV) infection remains a significant public health concern, though direct-acting antivirals (DAAs) have achieved a cure. Sustained virologic response (SVR) at 12 weeks is the most common metric used to assess the efficacy of antiviral therapy in eliminating HCV from the body. Varying treatment outcomes have been reported across different demographic and socioeconomic groups. Understanding the existence and implications of health disparities in HCV treatment is crucial for designing equitable healthcare interventions. In this study, we aimed to describe the poten
