Bone health in patients with fibromyalgia.

OBJECTIVES: To determine whether women with fibromyalgia are at increased risk of developing osteoporosis or osteomalacia. METHODS: Forty premenopausal women with fibromyalgia and 37 age-matched female controls were studied. Broadband ultrasound attenuation (BUA) and velocity of sound (VOS) were measured at the calcaneum and bone mineral density was measured at the forearm and lumbar spine using dual-energy X-ray absorptiometry. Serum calcium, alkaline phosphatase, gamma-glutamyl transferase, 25-hydroxyvitamin D and plasma viscosity were measured in all subjects and parathyroid hormone was me

[Value of ultrasound study and dual energy x-ray absorptiometry (DEXA) for assessment of risk of osteoporosis].

UNLABELLED: Screening postmenopausal women for preventing osteoporosis at an early stage is the main topic of several studies. The present paper evaluates the place of ultrasound measurements in comparison with Dual Energy X-ray-absorptiometry (DEXA). METHODS: 247 women (41 women < or = 50 yrs, 103 women 50-60 yrs, 103 women > 60 yrs) underwent DEXA of the vertebrae and hip and concommittantly ultrasound measurements of the calcaneus. RESULTS: The DEXA values in the vertebral column vary considerably and are lowest in L1 and highest in L4. The femur on the other hand shows lowest values in t

Peripheral bone mass is not affected by winter vitamin D deficiency in children and young adults from Ushuaia.

Low vitamin D levels in elderly people are associated with reduced bone mass, secondary hyperparathyroidism, and increased fracture risk. Its effect on the growing skeleton is not well known. The aim of this study was to evaluate the possible influence of chronic winter vitamin D deficiency and higher winter parathyroid hormone (PTH) levels on bone mass in prepubertal children and young adults. The study was carried out in male and female Caucasian subjects. A total of 163 prepubertal children (X age +/- 1 SD: 8.9 +/- 0.7 years) and 234 young adults (22.9 +/- 3.6 years) who had never received

Osteolysis of the pelvis presenting as insufficiency fracture in a patient with rheumatoid arthritis.

Physician awareness of the risk of osteoporosis and subsequent fractures in a patient with a history of long-term steroid treatment is high. The tendency to assume that a fracture is owing to steroid-induced osteoporosis may result in an unnecessarily intense antiresorptive treatment regimen for a patient who may not have osteoporosis. I report here about a patient with rheumatoid arthritis who presented with bone fracture despite antiresorptive therapy and without evidence of osteoporosis by bone mineral density testing.

Understanding the Bone Mass Measurement Act.

The Bone Mass Measurement Act (BMMA) set forth regulations to provide for uniform coverage under Medicare Part B for bone mass measurements for services provided on or after July 1, 1998. The BMMA authorizes Medicare coverage of "medically necessary approved measurements" performed for a "qualified individual" who falls into at least one of five diagnostic categories: an estrogen-deficient woman at clinical risk for osteoporosis; an individual with vertebral abnormalities; an individual receiving long-term glucocorticoid (steroid) therapy; an individual with primary hyperparathyroidism; and an

Hypophosphatemic rickets accompanying congenital microvillous atrophy.

This report concerns an 11-year-old boy who manifested hypophosphatemic rickets associated with congenital microvillous atrophy (CMA). He had been suffering from vomiting and severe diarrhea from the first day of life and had been treated with total parenteral nutrition (TPN) since he was 67 days old. At 4 years of age, intestinal biopsy resulted in a diagnosis of CMA. He was admitted to our hospital complaining of leg pain at the age of 11. Laboratory data revealed hypophosphatemia, elevated serum 1, 25-dihydroxyvitamin D (1,25(OH)2D) levels, and hypercalciuria. A roentgenogram showed rickets

Total body bone mineral measurements in children with Prader-Willi syndrome: the influence of the skull's bone mineral content per area (BMA) and of height.

Background. The aim of this study was to investigate the influence of body height and growth on total body measurements with dual energy X-ray absorptiometry (DEXA) in children. Material and methods. Seventeen children with Prader-Willi syndrome were studied as part of a clinical investigation of the effect of growth hormone (Genotropin) treatment. Bone mineral areal mass (BMA), in g/cm2, was studied with DEXA at 0, 12, 24 and 30 months after the start of the study. The effect of increased bone volume on BMA was studied by making a rough estimate of bone width, which was correlated with BMA. R

Use of bioelectric impedance analysis and dual-energy X-ray absorptiometry for monitoring the nutritional status of dialysis patients.

Despite recent technological advances, inadequate nutrition has been clearly identified as a significant risk factor to survival of patients undergoing chronic maintenance dialysis therapy. Although body density by underwater weight and residual lung volume, total body water by isotope dilution, bone mineral content, and total body potassium measurements will provide a very accurate multicompartmental analysis of body composition, they are not applicable to routine clinical or field work. Because of availability and simplicity, in addition to anthropometry, bioelectrical impedance and dual ene

Three monthly intravenous injections of ibandronate in the treatment of postmenopausal osteoporosis.

PURPOSE: Oral treatment of osteoporosis with bisphosphonates relies on compliance, the absorption being low and suppressed by simultaneous food intake. Intravenous (IV) treatment with an aminobisphosphonate, pamidronate (once every 3 months) was effective, but required infusions. Ibandronate, a new very potent aminobisphosphonate, can be administered safely as an IV bolus injection, and therefore offers an interesting alternative suitable for outpatient treatment. PATIENTS AND METHODS: To test the efficacy of this bolus IV treatment in postmenopausal osteoporosis in randomized partly double-b

[Long-term follow-up study of bone mineral density in a patient with adult idiopathic Fanconi syndrome].

A 64-year-old woman complaining of severe lumbar pain was admitted to our hospital because of the finding of pre-existing mitral valve regurgitation on examination. Laboratory data revealed the proximal type of renal tubular acidosis, renal glucosuria, phosphaturia, generalized aminoaciduria and low-molecular-weight proteinuria. She did not have any cause of these tubular dysfunctions, and was diagnosed as adult idiopathic Fanconi syndrome. Dual energy X-ray absorptiometry (DEXA) revealed a reduction of bone mineral density in the lumbar spine to about 65% of the age-and gender-matched control

[Comparison of bone mineral levels in healthy Japanese perimenopausal women measured by dual energy X-ray absorptiometry and ultrasound methods].

One hundred and seventy Japanese perimenopausal women, 44 to 65 years old (average 55.3), living in the Tokyo metropolitan area were evaluated for bone minerals by two methods of measurement--DXA (dual energy x-ray absorptiometry) measured at the 2nd-4th lumbar spine and three areas of proximal femur, and US (ultrasound bone densitometry) measured at the calcaneal area of the foot. Menopausal status was determined by interview, and anthropometric measurements, grip strength and various foot measurements including length, breadth, girth, area, and angles were taken. The relationship between the

Bone mineral density in middle-aged women with Turner's syndrome.

Bone mineral density (BMD), bone mineral content and body composition were determined in 47 middle-aged (mean age 47.9 +/- 1.1 years) women with Turner's syndrome. Bone mineral density was measured in the forearm, femoral neck and total body. The women investigated had a BMD lower than the normal mean. When expressed as Z scores (individual values compared to normal reference data matched for age, weight and sex), the median Z score of the total body was -1.23. When comparing women with the karyotype 45,X and mosaic women, the latter showed a higher BMD in all sites of measurement. Duration of

Bone mineral density in ambulant, non-steroid treated female patients with rheumatoid arthritis.

Bone mass measurements were performed in a group of 30 ambulant, non-steroid treated female patients with rheumatoid arthritis (RA) of relatively short duration (mean 4.9 years). The bone mineral density (BMD) of the lumbar spine and femoral neck was assessed by dual-energy x-ray absorptiometry (DEXA), and related to parameters of disease activity and severity. Lumbar BMD was within the range of normal while femoral BMD was decreased compared to age-matched controls. BMD values, expressed as the percentage of age-matched healthy controls (BMD%), were positively related to the body mass index a

Effect of promethazine on lumbar vertebral bone mass in postmenopausal women.

OBJECTIVES: Work in mice suggests that the age-related loss of bone mineral noted in that species is caused by an intrinsic defect in a haematopoietic cell population which results directly or indirectly in increased bone resorption. This age-related loss of bone mineral is prevented or reversed by well-tolerated doses of promethazine HCL. The present study was undertaken to determine if promethazine would retard or reverse bone loss in postmenopausal women. DESIGN: Postmenopausal women whose spine (L2 to L4) bone mineral content (BMC) was two standard deviations below young normal values wer

The effect of vertebral collapse on spinal bone mineral density measurements in osteoporosis.

Bone mineral density (BMD) of the lumbar spine (L1-L4) was measured using dual-energy x-ray absorptiometry (DXA) in 57 postmenopausal women with spinal osteoporosis aged 50-82 years (average age 64). For each vertebra between L1 and L4, the BMD was compared to age-matched normal population values and a Z score obtained. Twenty three patients had between one and three collapsed vertebrae from L1 to L4. The average Z score for fractured vertebrae was -1.62 but for uncollapsed vertebrae was -2.26 (P less than 0.001). The difference in Z scores obtained for collapsed and uncollapsed vertebrae was

Assessment of bone mass by image analysis of metacarpal bone roentgenograms: a quantitative digital image processing (DIP) method.

a digital processing (DIP) method for assessing bone mass was developed on the basis of image analysis of roentgenograms. Linearity between DIP values and the actual calcium carbonate content was scarcely affected even if roentgenograms were made with bone phantoms placed in different depths of water or by altering the voltage of X-ray generation. In clinical studies, coefficients of variation (CV) for various measurements were lower than 2.4%. When the correlations between the DIP values and the bone mineral densities in the distal one-third of the radius, and the 2nd to 4th lumbar vertebrae

Screening for postmenopausal osteoporosis.

Postmenopausal osteoporosis is an area of increasing concern to the gynecologist. Fractures will develop in approximately 15% to 25% of women in their later years and preventive therapy is best aimed specifically at this group. Predicting who will be the ones to sustain fracture is a difficult and imprecise clinical task. The clinical, laboratory, and radiologic methods available for this purpose are reviewed. A rational approach to this problem in today's setting is outlined.

Involutional osteopenia: current concepts.

Involutional osteopenia is the most common metabolic bone disorder affecting the elderly population. Improved diagnostic techniques now allow for earlier detection of a decrease in bone mass than is possible with routine skeletal roentgenograms. Methods developed for quantitating bone remodeling have given new insight to the mechanisms involved in age-related skeletal loss. Theories of pathogenesis include [1] inadequate skeletal acquisition during youth, [2] dietary deficiencies of calcium and vitamin D, [3] hormonal imbalance, and [4] local factors influencing skeletal remodeling, particular
