Risk for colorectal neoplasia in patients with colonic Crohn's disease and concomitant primary sclerosing cholangitis.

BACKGROUND & AIMS: Patients with ulcerative colitis and concomitant primary sclerosing cholangitis (PSC) have a greater risk of developing colorectal dysplasia or invasive cancer than patients with only ulcerative colitis. Therefore, annual surveillance colonoscopies are recommended. We investigated whether primary sclerosing cholangitis is also a risk factor for colorectal dysplasia or cancer in patients with Crohn's disease of the colon. METHODS: We performed a retrospective review of data from a tertiary care hospital on 166 patients with PSC and inflammatory bowel disease; 120 had concomi

Pancreas-preserving total duodenectomy without pancreato-enteric anastomosis.

BACKGROUND/AIMS: Pancreas-preserving total duodenectomy is a challenging surgical technique with organ preservation and has limited indications. We assessed the safety, feasibility and short-term functional outcome of PPTD without the need of pancreato-enteric anastomosis in our surgical technique. METHODOLOGY: During the two-year period from 2005 to 2007, three patients underwent pancreas-preserving total duodenectomy at our center. Two patients had diffuse adenomatous polyposis; another had previous transduodenal excision for polyp with recurrence. In all three patients pancreas-preserving

Screening strategies for colorectal cancer: a systematic review of the evidence.

BACKGROUND: Colorectal cancer is the third most common cancer in Canada. It is well recognized that there are improved survival rates if the disease is treated in its early stages, and indeed this may be a preventable disease. This paper systematically reviews the effectiveness of specific screening techniques for colorectal cancer in asymptomatic individuals at normal or above average risk. METHODS: MEDLINE was searched for articles published between January 1966 and January 2001 by using the MESH terms 'screening' and 'colorectal neoplasia'. The reference sections of review articles publish

A search for germline APC mutations in early onset colorectal cancer or familial colorectal cancer with normal DNA mismatch repair.

Twenty percent of colorectal cancers (CRCs) arise in people who have a family history of CRC in at least one other relative. Although a fraction of these CRCs are explained by two well-described autosomal dominant syndromes-5% by hereditary nonpolyposis colorectal cancer (HNPCC) and 1% by familial adenomatous polyposis (FAP)-the cause of the remaining 14% of familial aggregates of CRC is unknown. Many cases of HNPCC are due to germline mutations in DNA mismatch repair genes, leading to the tumor phenotype of microsatellite instability (MSI), and most cases of FAP are caused by germline APC mut

Brain tumor-polyposis syndrome: two genetic diseases?

PURPOSE AND DESIGN: This report presents a comprehensive and statistical analysis of the brain tumor-polyposis (BTP) cases referred to as Turcot's syndrome in the literature. RESULTS: BTP patients encompass a heterogeneous group that can be classified into two statistically distinct clinical entities based on phenotype of the polyps (P = .0001), presence of colorectal cancer (P = .0001), type of brain neoplasm, ie, glioma or medulloblastoma (P = .0001), presence of skin lesions (P = .0004) and cafe-au-lait spots (P = .0008), as well as consanguinity (P = .0135). CONCLUSION: The first entity

Colonoscopic screening for families of patients with colorectal cancer.

We conducted a prospective study using primary colonoscopy in the families of patients with surgically treated colorectal cancer (CRC) to evaluate its feasibility as a screening method for CRC. The presumed detection sensitivity of sigmoidoscopy in this group was also evaluated. Over a 3-year period, 142 first-degree relatives (73 men and 69 women) of 117 patients with CRC participated in this colonoscopic screening study. The average age at presentation for examination was 49.1 years (range, 31-79 yr). Forty-nine patients had adenomatous polyps (AP) and four patients had cancer, yielding a fr…

Gel Immersion Improves Visualization in Underwater Endoscopic Mucosal Resection for Colorectal Lesions: A Prospective Crossover Trial.

OBJECTIVE: Underwater endoscopic mucosal resection (UEMR) is widely used for colorectal polyp resection, but visualization may be impaired with poor bowel preparation. Gel-immersion endoscopy has been proposed to enhance image clarity. In this study, digital image analysis was applied to quantitatively compare lesion visibility during UEMR with water or gel immersion. METHODS: In this prospective, randomized crossover trial, patients with non-pedunculated colorectal adenomatous lesions measuring 6-19 mm were sequentially examined with water and gel immersion using white light imaging (WLI), b

A complex rearrangement between APC and TP63 associated with familial adenomatous polyposis identified by multimodal genomic analysis: a case report.

Genetic testing of the APC gene by sequencing analysis and MLPA is available across commercial laboratories for the definitive genetic diagnosis of familial adenomatous polyposis (FAP). However, some genetic alterations are difficult to detect using conventional analyses. Here, we report a case of a complex genomic APC-TP63 rearrangement, which was identified in a patient with FAP by a series of genomic analyses, including multigene panel testing, chromosomal analyses, and long-read sequencing. A woman in her thirties was diagnosed with FAP due to multiple polyps in her colon and underwent tot

An uncommon cause of acute abdomen in an acromegalic patient: colonic volvulus.

Acromegaly is a chronic endocrine disease, typically caused by a pituitary adenoma leading to increased circulating GH levels and increased IGF-I secretion by peripheral tissues. Gastrointestinal diseases related acromegaly such as colon cancers, adenomatous polyps, and dolichocolon are much less known. We aimed to present a case of complete colonic volvulus in a patient with acromegaly. A 60-year-old male patient presented with the complaints of inability to pass gas or stool, worsening distension, and pain for one week. The clinical exam showed significant tenderness and distended abdomen as…

Correlation of RKIP, STAT3 and cyclin D1 expression in pathogenesis of gastric cancer.

RKIP is proposed as a new metastasis suppressor. Our recent study showed that RKIP inhibits malignant phenotypes of gastric cancer cells. However, the underlying mechanism of RKIP function in gastric cancer is unclear. This study aimed to investigate the correlation of RKIP, STAT3 and cyclin D1 expression in the tumorigenesis of gastric cancer. RKIP, STAT3 and cyclin D1 proteins were detected by immunohistochemistry in tissues of gastric ulcer (n = 27), gastric adenomatous polyp (n = 7), intestinal metaplasia (n = 26), dysplasia (n = 40), gastric carcinoma (n = 169) and metastatic lymph node (

Desmoid tumors do not prevent proctectomy following abdominal colectomy and ileorectal anastomosis in patients with familial adenomatous polyposis.

BACKGROUND: Elective proctocolectomy has been recommended for patients at high risk of desmoids based on the possibility that cancer in a retained rectum may be unresectable because of desmoid disease. There are no data to support the reality of this concern. OBJECTIVE: The aim of this study was to see how often proctectomy was prevented by desmoids. DESIGN: This retrospective, descriptive, database study was augmented by chart review. SETTING: This study was conducted at a hereditary colorectal cancer clinic in a tertiary referral center. PATIENTS: Those presenting for proctectomy after c

Diagnostic performance of quantitative fecal immunochemical test and multivariate prediction model for colorectal neoplasms in asymptomatic individuals.

OBJECTIVE: Quantitative fecal immunochemical test (QTFIT) has the advantage of being able to describe test characteristics on a scaled rather than binary system. The aims of this study were to decide the optimal cut-off points of QTFIT and to make a multivariate prediction model for colorectal neoplasms in asymptomatic adults. MATERIALS AND METHODS: We retrospectively analyzed 1085 consecutive asymptomatic individuals who completed both full colonoscopy and QTFIT at a general health checkup clinic. Advanced adenomatous polyps (AP) were defined as APs of at least 1 cm in diameter; adenomas wit…

Annual endoscopy and urine cytology for the surveillance of bladder tumors after enterocystoplasty for congenital bladder anomalies.

PURPOSE: It is currently recommended that patients with congenital bladder anomalies managed by enterocystoplasty undergo annual surveillance with urine cytology and endoscopy. We reviewed our experience with this protocol and suggest modifications based on this experience. MATERIALS AND METHODS: A total of 65 patients 10 years or more after enterocystoplasty were placed on an annual surveillance protocol consisting of interval medical history, renal-bladder ultrasound, serum B12, electrolytes, creatinine, urinalysis, urine cytology and endoscopy. RESULTS: Of the 65 patients 50 (77%) with en

Poor Knowledge of Personal and Familial Colorectal Cancer Risk and Screening Recommendations Associated with Advanced Colorectal Polyps.

BACKGROUND AND AIMS: Advanced colorectal polyps (adenoma or sessile serrated polyp ≥ 1 cm, adenoma with villous features, adenoma with high-grade dysplasia, or any sessile serrated polyps with dysplasia) are associated with an increased risk of future advanced colorectal neoplasia and confer an increased risk of advanced neoplasia to first-degree family members. Professional societies therefore recommend more intensive surveillance of these polyps and earlier screening for first-degree relatives. The aim of this study was to assess knowledge of personal and familial risk and recommendations am

AGA Clinical Practice Update on Nonampullary Duodenal Lesions: Expert Review.

DESCRIPTION: Nonampullary duodenal polyps are found in up to 5% of all upper endoscopies; the vast majority are identified incidentally in asymptomatic patients. Although most are benign, adenomas are estimated to account for 10%-20% of these lesions. Most international guidelines recommend that all duodenal adenomas should be considered for endoscopic resection; this may be associated with a near 15% adverse event rate (predominantly bleeding and perforation) in prospective studies, with substantial local recurrence on surveillance. The aim of this American Gastroenterological Association (AG
